Crash Bandicoot 4: It's About Time
1
Attachment In Crash Bandicoot 4: It's About Time, the relics only appear as 2D images. However, complete 3D models can be found for all of them inside the game, including three variants of the N. Sanely Perfect relic that seem to be tiered (one being a skull, another being a skull with crossbones, and the last being a skull with crossbones; wings; horns; and a more menacing face which is closer to the final N. Sanely Perfect Relic aside from the wings), all with their own animations.
Super Mario 3D World + Bowser's Fury
1
Attachment Super Mario 3D World + Bowser's Fury has data for an unused Giga Mushroom, which resembles the Mega Mushroom but with a red-yellow color palette and an angry expression with a cross-popping vein, a common anime trope used for anger. It has seven animations, but each one contains no data. Touching it plays the "item into inventory" sound. If Mario touches it, he will become Mega Mario, and if Mega Mario touches it he will become Giga Cat Mario, suggesting it would have been used for Bowser's Fury.
Super Mario 3D World + Bowser's Fury
1
Super Mario 3D World + Bowser's Fury has unused character select screen voice clips from Toadette ripped from Captain Toad: Treasure Tracker, suggesting she was going to be a playable character at some point.
Twisted Metal: Small Brawl
1
Attachment A prototype for Twisted Metal: Small Brawl has an unused 9/11 memorial screen.
Lemony Snicket's A Series of Unfortunate Events
1
Attachment In the ending of the PC version of Lemony Snicket's A Series of Unfortunate Events, the Count Olaf police mugshots are 3D squares that the camera zooms into. If you change the camera to look at the sides of these mugshots, you can see an anthropomorphic turtle holding a trophy with the caption "CONGRATULATIONS! A WINNER IS YOU!" (a reference to Pro Wrestling for NES).
Ico
2
Attachment In the area beside The Watermill, there is a tree with a small low-poly representation of a Japanese Rhinoceros Beetle attached to it. This pairs well with a bit of cut content recently found within the game's code for the same area, where Ico could have gained a secret weapon that took the form of an Insect Net.

How these two elements could have come together remains unknown, but it was likely part of a reference to Ico's voice actor, Kazuhiro Shindō, and his prior role as 'Boku' in the first installment of the Boku no Natsuyasumi series.

Boku no Natsuyasumi had an extensive bug-catching and beetle fighting system. These features reappearing in ICO were potentially meant to be an elaborate reference, playing on the idea that Kazuhiro Shindō is once again voicing a young boy, who might refer to himself with the first-person pronoun 'boku', if he were to speak Japanese.

This video displays both the Japanese Rhinoceros Beetle and the wireframe of the unused Insect Net weapon:

The credits for both ICO and Boku No Nastuyasumi show that both protagonists' voice actors have the same name. I have highlighted both names in a combined screenshot attached.

Additionally, Kazuhiro Shindō's Japanese Wikipedia article confirms his video game roles. Providing evidence that he really was the voice of both characters:

Nickelodeon All-Star Brawl
subdirectory_arrow_right Nickelodeon All-Star Brawl 2 (Game)
1
Attachment In launch versions of Nickelodeon All-Star Brawl, Garfield's stage Sweet Dreams was discovered before his official announcement in an unfinished form which takes place at day time, unlike the final version which takes place at night. Additionally, a thumbnail was discovered for an extremely early version of the stage that takes place at sunset and has an opaque lasagna tray. When Sweet Dreams returned to Nickelodeon All-Star Brawl 2, under the name Food Dream, the daytime setting for the stage would be restored.
Mario Forever
2
Attachment Mario Forever, between versions 3.5 and 4.4, used an audio track that was a "fart version" of the Super Mario World 2: Yoshi's Island castle theme as the Hardcore World 1-4 music. Prior to its addition, some versions of the game had an unused version of the fart castle theme with Luigi impressions by developer Michał Gdaniec non-melodically interspersed.
Ed, Edd n Eddy: Scam of the Century
1
Attachment Ed, Edd n Eddy: Scam of the Century uniquely uses flat character heads on 3D bodies. However, Ed's texture contains segments from a 3D head, suggesting that the game may have originally used fully 3D characters.
Snoopy's Magic Show
1
Attachment Based on evidence from the book Video Game Quest and unused graphics within the game's code, Snoopy's Magic Show was likely originally going to be an original IP, with the title of either Death Ball or Heart Attack.

Disney's Tarzan
1
The Game Boy Color version of Tarzan has rumble support in its code, but was not released on a cartridge capable of using rumble.
PaRappa the Rapper 2
subdirectory_arrow_right PaRappa the Rapper (Game)
1
Attachment There is an unused 7th stage in PaRappa the Rapper, which seems to be an encore of the final stage where audience members - including PaRappa's rival Joe Chin - come on stage and copy PaRappa's rhymes instead of the other way around. This concept would be used in PaRappa the Rapper 2 as an encore following the final stage's Cool Mode.
PaRappa the Rapper
1
Attachment Cheap Cheap the Cooking Chicken has unused front-facing sprites. It is not known what these are for, but it's been speculated that she was going to use them inside the TV and then switch to her used in-profile sprite when PaRappa gets a Bad rating and she comes out of the TV to scold him.
Wii Fit
1
Attachment Inside the files of Wii Fit is an early model for the trainer who less resembles a mannequin and moreso appears to be distinctly white with blonde hair and almost fully realistic. The exceptions to this being her feet which have no toes, and her clothes that - while roughly the same color as the final character - are textureless (note that the attached image appears to be wearing a white onesie due to a rendering error). Based on the structure of the model and the full version of her body texture, it is apparent that the model was originally nude and the developers simply retextured parts of her body to create her clothing. The clothing meshes have their UV data removed, making them unable to use the body texture. The trainer also has a few unused animations of some warm-up stretches and yoga poses, but her shoulders get distorted and inverted due to how they are rigged. Additionally, while the warm-up poses flow together, the yoga poses (different steps of the Tree pose) are almost completely still and have no transition between them.
