Attachment At the start of the game, Brandi Broke is pregnant with her third child and the last conceived with her late husband Skip. However, due to Skip's genetic file being unlinked, the baby will take Brandi's genetics, and will always be male, essentially always making him an opposite sex clone of his mother. Linking Skip so that he is properly linked before the baby is born will result in the baby being born with the genetics of Brandi and Skip, and the chance of it being female.

There is an error with her memories in that she gave birth to their second child Beau after Skip died, yet her third child is recognized by the game as his. Since Brandi could not have gotten pregnant with the third child when she was already pregnant with Beau, Skip could not be the father unless he somehow impregnated her post-mortem. This, however, contradicts a picture in the family album that shows Brandi about to give birth to Beau with both Skip and their first child Dustin present.

During the 7th episode of the EA Community Team Cast from April 25, 2019, series executive producer and general manager Lyndsay Pearson claimed that the memory contradictions were an oversight, and that the "cloning" technique was used because the data for Brandi's pregnancy kept getting lost.
No More Heroes

The mini-fridge in Travis' room at Motel "NO MORE HEROES" can be used to heal him, even though it's impossible to lose health outside of ranking matches and assassination gigs and Travis' health is always restored after completing or failing a gig. Both the game and its instruction manual still point out this feature regardless. This suggests that there were either plans for enemies to appear in the overworld of Santa Destroy that could harm Travis outside of his missions, or it was implemented purely for creative effect.
The Elder Scrolls IV: Oblivion

Attachment The "skooma drinking" animation in the final game is buggy and functions incorrectly. When a character is displaying the animation, their mouth clips through their head, and unlike any other animation in the game, they do not stop the animation when entering dialogue with the player.

Attachment The song "OPERATOR" by PAPAYA is mistakenly credited in original Arcade releases of the game as "OPERATOR (Two Gees Mix)", despite the song actually being the original version. This was fixed starting from Dance Dance Revolution 3rd Mix PLUS!.

Attachment During the Warhammer 40,000 crossover promotion in 2020, the game's publisher Wargaming came under scrutiny after it was made aware by Twitter user Darren Bogus that they were sponsoring controversial YouTuber Arch Warhammer to help promote it. Arch Hammer is known within the Warhammer community for his misogynistic, racist, and hateful remarks. In a statement from Wargaming on July 3rd 2020, they issued an apology and claimed that sponsoring Arch was due to "human error" and had since cut ties with him. They even admitted that they were warned beforehand by Warhammer's owners Games Workshop not to collaborate with him but had failed to follow their request.

Although the game takes place at Disneyland in California, the book that Mickey Mouse signs an autograph on is actually from the park's Florida counterpart Walt Disney World, as evidenced by the exclusive park attractions.
Sherlock Holmes Versus Jack the Ripper

Attachment Despite the game's cover artwork depicting Sherlock Holmes wearing his signature deerstalker cap and tweed Inverness cape, throughout the game he's actually never shown wearing it. In fact, when Holmes asks Watson to retrieve the cap for use in solving a puzzle, he describes it as "the one I never wear but everyone seems to think I wear day and night."

In the original Sherlock Holmes stories by Sir Arthur Conan Doyle, Holmes is never explicitly described in text as ever wearing a deerstalker. The first time Holmes had ever been depicted wearing one was in the illustrations for the short The Boscombe Valley Mystery drawn by Sidney Paget and published in The Strand Magazine in 1891, though written by Doyle.
Crash Bandicoot: Warped

Remaining idlr on the game's title screen will cause a Demonstration Mode to play. When the third Demo plays, if the player holds down triangle as the screen fades to black, they can then control Coco during her flying stage. Should the player then pause and select the Warp Room option as if to exit a level early, they will skip the introduction cutscene and immediately begin a new game with all five power ups unlocked from the start.

It is unknown if this is an elaborate hidden cheat, a potential debug function Naughty Dog utilized during development, or just simply an mistake within the game's code.

Alternatively, completing the "level" as normal will spit Crash back out into Warp Room 4. Due to the gate being locked without standard progression the player is now stuck, only able to access those five respective levels.

In the PAL version, this same feature is performed with Start instead of Triangle.
Pokémon Emerald Version

The player can completely softlock their game if an extremely complex and absurd series of actions are undertaken, forcing them to have no other choice but to start a new game. Specifically, if this is done, then the player will be stuck at either the Pokémon League building, Pacifidlog Town, Dewford Town, or Sootopolis City with only an Electrode that only knows the move Selfdestruct.

This is not possible in the Generation VI remakes.
Jeopardy!

One of the clues in the "TV Twosomes" category falls victim to a glitch. The clue in question is "Number of Agent 86's Partner on TV's *GET SMART*". The correct question, "Who is Agent 99?" is not accepted as correct by the game, not even from a computer player. This is because 9 is used as a separator byte in the game, leading to the computer misreading the answer as simply "Agent".
Mario Kart Wii

Once Orochi is defeated on the PlayStation 2 version, a crescent moon comes out in Shinshu Field. However, a full moon comes out in the Japanese version due to a programming mistake.
Sonic Heroes

A lot of Team Rose's levels end at a much earlier point than Team Sonic and Dark's. However, through the use of glitches, exploits, or cheats, it's possible to go past the goal ring with Team Rose and go to areas that are only accessible to the two other aforementioned teams. The same thing can also be done in some of Team Chaotix's levels as well.

Interestingly, the characters of both Team Rose and Chaotix have fully voiced and functioning dialogue for these normally inaccessible areas.

Attachment The game's story set-up in the Nintendo 64 and PC versions is that the wizard's accident is caused due to him mixing the wrong potions causing him to turn to stone, however in the PlayStation intro the incident is instead depicted as being deliberately brought about by the glove that becomes the game's villain, Cross-Stitch. The manual for the PlayStation release retains the incident as being an accident and not deliberate as seen in the other versions.
Aliens: Colonial Marines

The reason the Alien AI in the game is broken is due to a one letter spelling error in the game's .ini file. The line is written as

"ClassRemapping=PecanGame.PecanSeqAct_AttachXenoToTether -> PecanGame.PecanSeqAct_AttachPawnToTeather"

when it should read;

"ClassRemapping=PecanGame.PecanSeqAct_AttachXenoToTether -> PecanGame.PecanSeqAct_AttachPawnToTether"

Removing the letter A from this line fixes many of the issues with the game's AI.
Super Mario Sunshine

Attachment During the game's opening cut scene, multiple cuts of Peach on the airstrip seem to have an error where the texture for her earrings use the texture for her eyes. This mistake is seemingly fixed from her first close-up onward.

The infamous Red LED lights that surround the 360's power button actually represent different problems depending on how many are alight. One red light meant a hardware failure. Two red lights signify overheating. Three red lights meant a general hardware failure, and four red lights meant and AV cable error. The notorious "Red Ring of Death" is actually, contrary to it's name, a 3-Light problem (General Hardware Failure).
Animal Crossing

In an early announcement trailer for the Nintendo Gamecube, Animal Crossing was called (or at least promoted as) "Animal Forest for Nintendo Gamecube", outright referencing the original Japanese title of the game (Dōbutsu no Mori or "Animal Forest" in English). This suggests that either the English localizers were thinking of keeping the game's original title, albeit translated, or the title "Animal Crossing" was not yet finalized.
Platform: Nintendo Switch

As of the 4.0.0 Update, which originally brought USB headphone support to the Switch, it was discovered that, via the use of the Wii U's Gamecube Controller Adapter, that almost every Switch game can be played with a Gamecube Controller. However, since the Gamecube Controller lacks a left Z-button the player does not have access to all of the controls of games that normally would utilize the Switch's ZL button.
