Attachment In the instruction manual, there is a character listed as Black Ears. This appears to be Kuromimi, based on the list of characters and their stars later in the manual, where Black Ears is listed as the Chitan Star.
The Legend of Zelda: Majora's Mask

Attachment After receiving the Romani's Mask, if you protect Romani's Ranch from the invaders on the First Day, and help Cremia deliver the milk to Clock Town, you'll receive 1 of 3 rewards. Link can get either a Huge Rupee (worth 200 Rupees), some Chateau Romani, or a hug.

If Link receives the hug, Cremia will hold Link up close to herself and a message will appear saying "You did it! You helped Cremia!" "You feel all warm and fuzzy! inside! Sigh... You could get used to this!" The extra exclamation point after fuzzy was removed in later versions of the game.

Collection: ToeJam & Earl

Rumours circulated that ToeJam & Earl were originally named FlowJam and Whirl. The story goes that when designer Greg Johnson told programmer Mark Voorsanger their names, Mark misunderstood what Greg was saying and coded them in the game as Toejam and Earl. It wasn't until they showed the game to SEGA that Greg realized the named were incorrect, but since SEGA actually like the name, they decided to keep them.

This rumour was confirmed as being false by Greg Johnson, who frequently joked with games press, giving them intentionally incorrect information, and was reaffirmed by the official ToeJam & Earl Twitter account in 2020.

Attachment There are at least two cases of art plagiarized from deviantart.com appearing in Sonic games. A piece of art of Sonia was mistakenly added to Sonic & Sega All-Stars Racing, and some fanart that was made for a Sonic and the Black Knight contest turned out to be a replica of someone else's artwork. Sega apologized both times.
Deus Ex: Human Revolution

Attachment The drop ship used to get around has the call letters "B-EE 008" on the side, but in the prerendered cutscenes (That were presumably made earlier in the game's development) the designation is "800 B-EE". It seems this was changed due to the fairly strong representation of the number 8 as a "B", meaning 800 B-EE would be pronounced "Boobie".
Mega Man 7

Attachment During the intro sequence, Dr. Wily is mostly covered by other graphics. Extracting his sprite from the game will show that he has distinct breasts. The orientation of his shoulders and arms means they couldn't be pectoral muscles. This could simple be poor rendering by the artist but given the precise nature of pixel art, it is more likely be intentional or possibly a joke by the developers.

Attachment The video reveal trailer ends by displaying a barcode. The numbers on the barcode teased a date (June 7th, 2011) when more about the game would be revealed. However, the company who made the trailer seem to have overlooked the barcode itself, as scanning the barcode takes viewers to an Amazon listing for a bag specifically designed to store sex toys.
Astal

Attachment Astal's spine did not feature the game's name, which may have lead to its poor sales in the US. Retailers would have had to display the case forward facing, rather than to its side as standard.
