Tina's hair color was originally meant to be blonde. However, the Model 2 hardware had difficulty rendering yellow in the game. This is why Tina's hair is brown. The issue was fixed in subsequent installments and updates to the game.

Attachment During the battle tutorial in Chapter 1, if the player repeatedly selects "FIGHT" and misses Kris's attack, Ralsei will grow increasingly agitated, eventually abandoning the tutorial altogether after ten misses and blaming himself for being a poor instructor.

Additionally, the third time Kris's attack misses, Ralsei's response will contain a typo in it, featuring an 8 mistakenly typed to the left of the asterisk that usually starts a line of in-game text.

Attachment In the Tatooine Jabba's Palace map there are two unused models out of bounds. The first one is outside one of the hallways and it is a figure in a robotic suit that looks to be bound by one hand and sitting immobile and in a voluptuous position. The model's suit is the only thing fully rendered and its unfinished "skin" and "body" just appear as a black mass. The second one is an unused Command Post that's "deactivated". Interestingly, in the level's reappearance in this game's sequel, the sitting figure is completely removed but the deactivated command post is not.

Attachment In Super Street Fighter IV, Hugo from Final Fight can be seen in the background in the Metro City stage, 'Skyscraper Under Construction'. When the Ultra Street Fighter IV announcement trailer was released, two Hugos could be seen together both in playable form and in the background. This was later fixed in the final version, if the player selects Hugo to play in 'Skyscraper Under Construction', the other Hugo in the background is replaced by a different character.

The N64 Controller that was released for Nintendo Switch Online's Expansion Pack can be used on any Switch game. However, it lacks X and Y buttons leading to some games being partially unplayable when someone attempts to play them with one.
The Sims 3
1
Attachment In June 2010, several players noticed that a package file was attaching itself to other packages in The Sims 3. The file was a corrupted girl doll object titled "Girl Doll Dressed", which was incorrectly cloned from the Teddy bear. Game installations became "infected" by players downloading lots, houses or Sims with the hidden doll attached, similar to how computer viruses spread, and the doll was determined to cause longer loading times and game crashes. Players then unknowingly uploaded these infected packages to The Exchange, the game's online custom content workshop, causing it to spread to even more players.

Many Sims community sites began reporting the issue and one user posted a guide on how to manually remove the doll from the game to prevent the glitch from spreading, and a SimGuru on EA's official Sims forum eventually responded with a stickied post informing the community about the issue, and stated that EA would patch the game in the future to institute measures preventing corrupted objects from being uploaded to The Exchange.

Attachment In the initial release of Chapter 2, interacting with Undyne's desk after goading her into breaking it generates the placeholder text "[NO TEXT] (obj_npc_room)"; a subsequent patch resolved the issue by making the desk impossible to interact with after it's broken.

In the early version of the game, Leon was able to air throw an opponent into the bouncing walls and the flammable fuel tanks in 'The Show' and 'Fuel' stages, respectively, an infinite number of times, making him overpowered in these stages. This was patched in a later version.

In Chapter 2-3: Breaking the Bank, it is possible to work off your debt of one million Rubees without finding Slim and stealing the needed currency from the mansion's vault. Although there are no differences in the stage's ending if you do this, working off the debt acts as a lengthy sequence break as traveling back to the vault after working and paying off the full debt will reveal that the counter on the vault door now displays 0, and the vault can be opened freely without entering the password to reveal that it is empty.

On the other hand, manually working to collect over 9,999,999 Rubees will result in the Rubees counter in the HUD overflowing and appearing to start back at 0 due to it only displaying seven digits, but in reality the game does register the collection of more than ten million Rubees and allows you to complete the level.
Super Mario Galaxy

Attachment In the Super Mario 3D All-Stars port of the game for the Nintendo Switch, the game's cutscenes were re-rendered into HD. However, an error slipped through where Princess Peach can now be seen T-posing during the game's opening sequence just after Bowser's airships drop their anchors into Peach's Castle.

Chrono Trigger
1
In the DS version, it's possible to defeat the special boss Spoiler:the Dream Devourer found in Darkness at the End of Time by healing. The boss has 32000 HP; the game's HP value is a 16-signed integer (it cannot be higher than 16 bits, and one of those bits only indicates the sign of the number it represents). By using powerful magic that the boss absorbs during its second phase, it can be temporarily healed past 32767 HP (the maximum HP limit for all characters in the game), which overflows the HP counter and turns the boss's HP into a negative number. Since the boss has less than 0 HP, it will be considered "dead" and the battle will end.

During the motion-capture scene where David Anderson knocked out Donnel Udina by punching him in the face so that Commander Shepard and his crew can go after Saren Arterius in the Normandy SR-2, the game's cinematic lead Shane Welbourn (suited up as Anderson), accidentally clocked the actor who played Udina in the jaw.
