When the player is inside Richard's villa, waiting around for two and a half minutes will trigger an instance of the famous Totaka's Song.

Totaka's Song is a small ditty that composer Kazumi Totaka likes to hide in many of works, including Mario Paint, Yoshi's Story, Luigi's Mansion, and most famously, the Animal Crossing series.
Wing Commander

The game crashes with an error every time you exit.

Unable to track down the error in time for release, one of the programmers simply hex edited the error text to read "Thank you for playing Wing Commander."
Black & White

In the game there's a mechanic where you praise or punish creatures. Praising them encourages a behavior, while punishing them discourages a behavior. Prior to a patch being released, punishing your creature after it had defecated caused it to become permanently constipated.

Attachment There is a man who is located at Cinnabar Island's Pokemon Laboratory who requests a Raichu in order to trade for an Electrode. After the trade is complete, the man then says 'The RAICHU you traded to me went and evolved!'

This is a translation error where the Japanese version of Pokemon Blue has a Machoke in place of Raichu where as Pokemon Red and Green have Raichu.

Attachment TM67 Retaliate & TM94 Rock Smash both have a sprite oversight where the TM Sprite does not match the Type in the HUD.
Retaliate is a Normal move but is shown as a Dark Type in the Sprite.
Rock Smash is a Fighting move but is shown as a Rock Type in the Sprite.

In the dam level, there are three guard towers on the main dam, and one across the lake on the hidden island. The ones on the dam are marked 2, 3, and 4 in order, and the one on the island doesn't have a number. This leaves guard tower 1 missing from the game.

It's possible that this tower originally appeared immediately to the right of the locked gate to the dam. When it was eventually removed, the developers may have simply forgotten to change the other numbers.

When the game was released in English speaking territories, the developers forgot to translate some of the text, including the trading cards and the sign above the Drill Tunnel entrance.

The S.T.A.R.S. office is way too small for a team of 12 and for some reason only has the desks of Barry, Chris, Jill, Rebecca and Wesker. They may have made some changes after the Mansion Incident but Brad's desk is missing and he had still been alive at the time the office was last used.
Tomb Raider: Legend

The original Xbox version doesn't have the introductory cutscene because the team forgot to put the video in the final build. Rather than suffer the delay and expense of re-submitting the build, they elected to simply release the game without the intro.

Attachment At the beginning of the Facility level, players are required to open a door with a computer switch in order to progress to the next area. Unfortunately, the computer can be destroyed, leaving players stuck in the starting area. In order to get around this, the developers added a scientist that eventually rushes to the bathroom. He is carrying keycard A, which will open all of the doors in the level (with the exception of the bottling room, which requires the door decoder).

Attachment Each guardian Bakugan that is buyable by the player has something missing. Drago and Delta Dragonoid II are missing their horns, Delta Dragonoid II is also missing the blades on his tail, Tigrerra is missing her blades and the diamond on her forehead, Skyress is missing her horns, Gorem and Hammer Gorem are missing their shield and horns respectively, Preyas and Preyas II Diablo are missing their collar, Preyas II Angelo has differently shaped eyes and missing half of his hair, and Hydranoid is missing his horn. These edits were probably made so these Bakugan would not look like the owners' guardian Bakugan from the Anime.

When a party member dual wields two weapons, even though there is an animation of the weapon being used, the game only calculates damage based on the first weapon. This is fixed in later versions.

The spell Temper (TMPR) was bugged in the original NES version. In later releases, it raises the attack of an ally by 14.
PokéPark Wii: Pikachu's Adventure

A couple of small typos were fixed in later versions of the game. The two corrected pieces of dialogue were:

"He works for Venusaur in the Meadow Zone. He blocked the bride to stop anyone playing, but really he loves the Attractions to be found there!"
Correction: bride -> bridge

"Not so far away are your Empoleon?"
Correction: your -> you
The Legend of Zelda: Skyward Sword

After completing the Thunder Dragon Quest, if the player talks to Golo, the Goron outside of the Lanayru Mining Facility entrance, at least twice, Golo tells the player that he is jealous of his discovery. From this point forward, the game becomes impossible to complete and the events at Eldin Volcano and Faron Woods will no longer occur.

Nintendo has since tried to fix this glitch by releasing "The Legend of Zelda: Skyward Sword Save Data Update Channel", a free channel from the Wii Shop.

Arin Hanson, also known by his YouTuber alias Egoraptor, provided the voice for Dr. Bruce Banner during the development of Marvel: Ultimate Alliance after winning a contest held by Activision. Hanson described the experience recording Banner's lines as "humiliating" as it was the first time he had to work in a professional recording booth let alone for a AAA video game, and believed it was a bad performance. Hanson initially claimed in 2013 that his voice was used in the PlayStation Portable and Nintendo DS versions of the game, but a DS version does not exist and he is not credited in the PSP version. In a later email correspondence with a user from the Lost Media Wiki, he stated he recorded his voice after Activision informed him that it would be used in select versions of the game, those being the Wii, PSP and PlayStation 3 versions. Since this was also a local contest, Activision assumed that all auditions were coming from Los Angeles and in the surrounding areas. Hanson got a "congratulations" email and was invited to Los Angeles for recording, but he was unavailable at the time. His recordings ultimately went unused in favor of another voice actor due to time constraints from Hanson only getting a chance to record late in development. However, his unused recordings would end up being added in the 2016 re-release of the game.

Super Mario Sunshine

Attachment In Noki Bay, once inside the bottle in the Red Coins in a Bottle level, Mario should fall to the bottom of the bottle and then locate a small rock formation there. If Mario maneuvers into a small square opening towards the back right of the rock formation, he can find a small door inside the opening which is impossible to open. Therefore, the player must rotate the camera to see what is on the other side of the door. In doing so, Mario can see a brown book on the floor of the room behind the impassable door, though its purpose is unknown.

The book itself may be a reference (or vice versa) to the level's background music. In the Original soundtrack, the title of the level's theme is in fact "The Book in the Bottle". It's also been said that this mission was originally different, and that Mario would collect the book rather than 8 red coins.

Attachment There were suppose to be four playable characters, Crash, Coco, N.Cortex, and Nina. However Coco was removed as a playable character and made an NPC. Her game play was rumored to consist of her hacking into security systems. Although she is not playable in the game anymore, the back of the box still says there are "4 playable characters."
Super Mario Sunshine

Attachment In the beginning of the game, when FLUDD is scanning Mario, there are videos playing in the bottom-left corner of the screen. These videos are Mario fighting Bowser in "Super Mario Bros", Iggy Koopa in "Super Mario World", and Bowser again in "Super Mario 64." In the opposite corner is a list of backwards names which represent past Mario games. Looking closely reveals that both Super Mario Kart and Mario Kart 64 were written with the word KIRT instead of KART.

In the original arcade version, Ryu had a winning quote that said, "You must defeat Sheng Long to stand a chance." This was a mistranslation that should have said "You must defeat my dragon punch to stand a chance." Sheng Long has become a running joke with Capcom that has turned into the characters of Akuma and Gouken
