Attachment When Kirby absorbs Sora and then uses his Magic, it is immediately clear that Kirby does not get a Keyblade of his own. Instead, Kirby uses an ethereal glowing weapon that resembles a keyblade but is entirely made out of light and sparkles that changes color depending on the spell. While this can be interpreted as being a restriction by Disney over the Keyblade's imagery, Masahiro Sakurai on Twitter chalked it up to being done for lore reasons as he said "...only Keyblade Wielders can use Keyblades..." implying that Kirby isn't "chosen" as barer of one and thus has to use the three spells differently.
Kingdom Hearts

Disney heavily restricted the use of Mickey Mouse in the game especially as the central character like Square initially wanted. This was because Disney was nervous about the success of the game and did not want its possible failure leaving a black mark on the company and its mascot. Tetsuya Nomura stated that these contractual restrictions went so far as to have Disney allow Mickey to only make a minor cameo appearance (such as "in the background of a crowd" as Nomura put it). Spoiler:Nomura and his team got around this by having Mickey have an extremely important but brief role at the end of the game in the form of Mickey helping Sora close the Door to Darkness so no more Darkness gets out of it, separating the two. King Mickey even appears mostly in silhouette in this scene. When the first Kingdom Hearts became a humongous success for both Disney and Square, the former allowed the game developers to feature their mascot in a much more prominent role in the franchise from then onward.
Nioh

In the Western version of the game, when an enemy is killed, it is possible to cut off their limbs during their death animation. This feature was censored in the Japanese version of the game to be inline with Japan's rating system.

Attachment When the Hollow Bastion stage turns into the Dive to the Heart segment, the different Stations of Awakening in the background that reference key Kingdom Hearts characters appear to be censored of any reference to Disney characters such as Mickey Mouse, Donald Duck, and Goofy. However, these censored Stations are actually close replicas of the designs used on coasters that Square Enix produced in celebration of Kingdom Hearts' 15th anniversary, which also omitted the Disney characters.

Pre-release footage of the game's story mode shows Spoiler:the death of the scientist in graphic detail, with the victim being melted. In the final release of the game Spoiler:the screen fades to black during the character's death.

Whip's Desert Eagle was completely censored in international versions of the game. Her scene in the Ikari Team ending was also altered to not have her shoot Zero in anger.

In the original overseas arcade version of the game, Mai's "swaying bosom" animation was disabled to appear stilted. However, in a 1994 developer interview, SNK revealed the animation could be unlocked via a secret code which they themselves did not reveal, but instead directed curious players to ask SNK's American division for it.

Attachment The design of the Anti-Funk, the final boss of the game, originally resembled the hood of a Ku Klux Klan member. This design made it into early review copies of the game before Sega discovered its existence and rejected it, forcing the developers to change it to a skull just before the game's release. In a 2003 Xbox Nation magazine interview with series creator Greg Johnson, he elaborated on the origin of the design:

"When I was doing it, it was actually an accident. We had done a bunch of designs for the Anti-Funk, and the artist had done this unintentionally. I looked at it and said, “Wow, that’s funny. That looks like a Ku Klux Klan hat.” And then I thought about it for a second and I said, “Well hey, what better villain for these black characters who are spreading the funk than the love and this kind of oblique reference?” Because it wasn’t [obvious]—it was open to interpretation."

In an interview with the game's producer/scenario writer Hiroyuki Takahashi and director Shugo Takahashi published in the 1993 Megadrive Fan Attack Special book, they stated that the enemy "Paste" (called "Bubbling Ooze" in the English localization) was originally going to be named "Slime". Its name was not included due to copyright issues, so they tried other options to name it like "Ooze" or "Gel", neither of which got picked. They later came up with the name Paste, while the English release gave the enemies the unused name "Ooze". It's possible that 'Slime' couldn't be used due to a copyright for the iconic Dragon Quest enemy of the same name.

Attachment The Japanese name of the Star Fox series' main antagonist, Andross, is アンドルフ (Andorf).

According to Takaya Imamura in a 2002 Nintendo Dream magazine interview, the name was deprived from “Andromeda”, and his name was changed to Andross in the Western localizations because of fears that his original name had "Nazi overtones", presumably suggesting the name's spelling had similarities to dictator Adolf Hitler, as アンドルフ can also be anglicized as "Andolf".

Night Trap: 25th Anniversary Edition, an expanded 2017 re-release of the game for the PlayStation 4, PlayStation Vita, Nintendo Switch and PC, uses the full uncompressed video footage recorded for the original game. Additional bonus content includes deleted scenes like an introduction scene for the game's story and a death scene featuring Danny (which was most likely cut due to Danny's young age), as well as a behind-the-scenes developer commentary, a "theater mode" to watch all of the game's story, a "survivor mode" that will randomly place Augers in the house, and a playable version of "Scene of the Crime", the first prototype of Night Trap created in 1986 to pitch an unreleased console called the Control-Vision to Hasbro.

Attachment In the United Kingdom, a poster promoting the PlayStation version of Tekken 3 prominently depicted a dead body with a severed leg in a morgue. In response, Britian's Advertising Standards Authority (ASA) forced Sony to remove the poster and promptly banned it, believing the ad was "macabre" and likely to cause offence. Additionally, the ASA forced Sony to have all future poster ad campaigns be examined by them prior to publication. Sony defended the ad, claiming it was meant to show "a surreal situation by positioning the body parts in the manner reminiscent of a cartoon character". Despite the explanation, the ASA stood by their decision.

Attachment In the Japanese release, due to the CERO rating system restricting certain graphic elements from appearing in games released in the country even with the highest rating of CERO-Z, a cutscene of Boozer getting his arm burnt with a blowtorch by the Rippers was altered by shifting the camera angle of the scene towards Boozer's face to imply he is getting burnt off-screen.

In a 2019 "ESPN Esports" video/interview between ESPN commentator Arda Ocal & professional boxer Mike Tyson, Ocal asked him about Balrog (who made his first appearance in Street Fighter II under the name M. Bison, but had his name changed to Balrog in the English localization due to the close similarities to "M. Tyson") and if he knew about the game and his influence on Balrog's character design. Tyson responded that he knew nothing about either Street Fighter or Balrog, but that he was honored by Capcom's impersonation of him upon seeing it for the first time.
Assassin's Creed Odyssey

The DLC, "Legacy of the First Blade, Shadow Heritage", was the subject of controversy and backlash from the LGBTQ+ community. At the end of the DLC, the player character was forced to be part of a heterosexual relationship, ending with the player character having a child with another character. Many players saw this as a disservice to the LGBTQ+ community, and Ubisoft going back on their commitment of diversity and inclusion within the game. Despite allowing the player to choose their sexuality through multiple romantic encounters throughout the game, the DLC gave no option to decline the relationship, forcing players to engage in the heterosexual relationship. Furthermore, completing this unlocked an achievement titled, "Growing Up". The developers apologized and released a patch for the DLC. In the patch, the ending is still the game, and a baby is born, but the player character is allowed to clarify that they choose to do so to "further their bloodline", and not for romantic reasons. The title of the achievement was also changed to "Blood of Leonidas".
Balan Wonderworld

After footage of the game's final boss surfaced from pre-release copies, players expressed concern to the developers that some intense rapidly flashing lights and other visuals during the final boss may cause serious seizures to players that have Photosensitive epilepsy, and in general may be an eyesore to players that are not epileptic. The developers understood and significantly changed the battle effects in a day-one patch that was heavily recommended by Square Enix to be downloaded by future players.
