The DLC, "Legacy of the First Blade, Shadow Heritage", was the subject of controversy and backlash from the LGBTQ+ community. At the end of the DLC, the player character was forced to be part of a heterosexual relationship, ending with the player character having a child with another character. Many players saw this as a disservice to the LGBTQ+ community, and Ubisoft going back on their commitment of diversity and inclusion within the game. Despite allowing the player to choose their sexuality through multiple romantic encounters throughout the game, the DLC gave no option to decline the relationship, forcing players to engage in the heterosexual relationship. Furthermore, completing this unlocked an achievement titled, "Growing Up". The developers apologized and released a patch for the DLC. In the patch, the ending is still the game, and a baby is born, but the player character is allowed to clarify that they choose to do so to "further their bloodline", and not for romantic reasons. The title of the achievement was also changed to "Blood of Leonidas".

After footage of the game's final boss surfaced from pre-release copies, players expressed concern to the developers that some intense rapidly flashing lights and other visuals during the final boss may cause serious seizures to players that have Photosensitive epilepsy, and in general may be an eyesore to players that are not epileptic. The developers understood and significantly changed the battle effects in a day-one patch that was heavily recommended by Square Enix to be downloaded by future players.

Although Terry Bogard from Fatal Fury had made it into the game as a downloadable character, Masahiro Sakurai had initially planned to add Mai Shiranui alongside Terry. In the Japanese Nintendo Direct presentation of Terry Bogard, he explained further that he worried that CERO (Japan's video game rating board) would raise the rating from A (for all Ages) to a higher one which was why he reconsidered having her in the game, joking that Super Smash Bros. was only for "good boys and girls".

Attachment According to game's director Hideki Kamiya, the Umbrella Corporation can that gets stepped on at the start of the opening cutscene was supposed to have the Capcom logo on it, but when Capcom's management heard about it, they disapproved of the idea of their brand being stepped on, so the developers changed it to the Umbrella logo instead.

Attachment Using up more than 8 credits will set the bad ending for the game. While the original Japanese release displays a credits counter on the Continue screen, this is omitted from the North American release. However, the game has a subtle visual indicator on the Continue screen showing which ending you earned. The photograph in the good ending features Kyle and Erin smiling normally; the bad ending alters Erin's face slightly, giving her fangs and a minorly distorted brow to show Kyle has failed and she has become a vampire.

This also affects the Game Over screen when choosing "No" to end that run of the game. When at 8 or less credits, a cutscene will play showing Nosferatu biting into Erin before showing the Game Over screen. When you have more than 8 credits, no cutscene will play as Erin has already become a vampire in the photo, and will cut straight to the Game Over screen. Some time after the game's initial release, cartridges of the game were produced and released in both Japan and North America with the Nosferatu bite scene censored by fading to a white screen just before he bites her and then displaying the Game Over screen.

Attachment In the Akumajo Dracula X CD liner notes, the game's character designer Toshiharu Furukawa stated that they had to remove some monsters in previous Castlevania games, due to the game's international localizations. Having destructible monsters with humanoid forms (i.e. Medusa, Carmilla, and Gargoyle) went against America's morals. However, for Castlevania: Rondo of Blood, it doesn't need to be localized, due to it being released in Japan-only.

Attachment The Hindu goddess Kali's design was initially more revealing, however during the game's beta, her appearance was changed to have her more clothed, in addition to new voice lines and updated abilities.

It's thought that her redesign was due to receiving backlash from practicing Hindus, who also objected to her inclusion. However, it's also possible this was done due to working with Chinese publisher Tencent to make the game more family friendly.

Attachment Shortly after its release onto the Nintendo Switch, the game was removed from sale from the North American Nintendo eShop due to containing content deemed beyond acceptable for the ESRB rating it was given. It was re-released on March 20th, 2020 as Lust for Darkness: Dawn Edition with the explicit content and sexual imagery toned down. The European release however remained unaltered with the "Dawn Edition" also added to the European Nintendo eShop.

Attachment The game was originally called "Gods and Monsters" when it was shown off at E3 2019, however, Ubisoft ran into some trouble with the Monster Energy Drink company who not only claimed that the original title of the game infringed upon their trademark, but also claimed that the game's title would cause brand confusion. Attachment The game's cover artwork originally featured the American flag and green gaseous fumes, and on the back the words "WANTED: Deadly Force Authorized", "deadly arsenal" and "terrorists". However, due to the then recent 9/11 terrorist attacks and the anthrax scare, the game had been recalled with these removed and the back text changed to make it more politically correct. This pushed back the game from its original September 25th, 2001 release to early November 2001. Some copies with the original box art had already shipped, making that version a collector's item and dubbed the "9/11 American Flag Cover". It also came in a dual case, despite the game only having a single disc.
