The Punisher

Bone Crusher, the third boss of the game, has three attacks, one where he uses his flamethrower, one where he launches multiple missiles, and another where he rushes into the player. The flamethrower attack has been removed from the Genesis version.

The Punisher

Attachment There's a short cutscene that plays after the player defeats Scully, the first boss of the game, where Punisher interrogates Scully to find out about Kingpin's location, and once he's done he disposes of Scully. In the Arcade version Punisher kills Scully, while in the Genesis version he just tosses him to the side.

Attachment Nick Fury smokes a cigarette in the arcade version, with the cigarette being part of his character select portrait and his in-game sprites. In the Genesis version, he doesn't smoke, with the cigarette being completely removed.

Valentine's design was officially changed on May 13th, 2014 via a patch update. The update changed all parts of her design that involve a red cross against a white background to be altered so the cross is colored pink instead. This was done in order to prevent legal action by the International Committee of the Red Cross.

In the Japanese version of World Heroes 2 Jet, Zeus will bleed during the cutscene that plays after his defeat. In the international release however, the blood has been removed.

Attachment In the North American and European releases of the game, the cigars being smoked by Flip and the Gorilla are removed. Despite this, the game's manual in these regions features artwork of the Gorilla smoking a cigar, and the Gorilla's sprite still has curled lips as if it were holding a cigar in its mouth.

Attachment Within the data for Chapter 2 is an unused alternate version of Spoiler:Spamton NEO's wasted turn, in which his phone fires a Pipis that explodes in his face and covers it in soot, rather than launching a volley of flying heads at him. Spamton NEO's head remains covered in soot until his next attack. It's likely that this event was changed due to Spoiler:its accidental resemblance to blackface, which historically was often paired with similar explosion gags in animation.

Attachment In the Tatooine Jabba's Palace map there are two unused models out of bounds. The first one is outside one of the hallways and it is a figure in a robotic suit that looks to be bound by one hand and sitting immobile and in a voluptuous position. The model's suit is the only thing fully rendered and its unfinished "skin" and "body" just appear as a black mass. The second one is an unused Command Post that's "deactivated". Interestingly, in the level's reappearance in this game's sequel, the sitting figure is completely removed but the deactivated command post is not.

Attachment In the original version of Grand Theft Auto: Vice City, a confederate flag can be seen on Phil Cassidy's muscle shirt. In the remastered version, the flag on Phil's muscle shirt has been changed to a skull.

Attachment Several clothing items were changed between the Japanese and international releases of the game:

•The Three Arc Shirt (a.k.a. the Familiar Shirt) is replaced with the Fortune Shirt, due to the former's resemblance to Charlie Brown's shirt in the American comic strip Peanuts (which had ended its print run in 2000, shortly before the game's release).
•The Tomato Juice Shirt is replaced with the Fishbone Shirt, due to the former's design resembling bloodstains.
•The W Shirt is replaced with the Houndstooth Tee, due to the former's resemblance to the logo of American fast food chain McDonald's.
•The Puzzling Shirt is redesigned to use more pastel colors, also changing the bottom-center square from orange to purple, to reduce the design's similarity to the Rubik's Cube brand of toy puzzles.
•The I Love GC Shirt (itself a redesign of the I Love 64 shirt from Dōbutsu no Mori) is replaced with the Cherry Shirt, due to the former being a parody of the I ❤ NY logo, which is trademarked by the New York Department of Economic Development.

These redesigns are also carried over to Dobutsu no Mori e+.

The German rating board deemed the game's gore and violence excessive and refused to rate the game at all. Since this had the potential to place the game on Germany's "List of Media Harmful to Young People", banning shops from either advertising or selling the game to customers of 18 years or older, Tecmo censored the PAL version to obtain a rating. A year later, Tecmo managed to obtain an 18+ rating for the uncensored release of Ninja Gaiden Black.
Super Smash Bros. Ultimate

Attachment When Kirby absorbs Sora and then uses his Magic, it is immediately clear that Kirby does not get a Keyblade of his own. Instead, Kirby uses an ethereal glowing weapon that resembles a keyblade but is entirely made out of light and sparkles that changes color depending on the spell. While this can be interpreted as being a restriction by Disney over the Keyblade's imagery, Masahiro Sakurai on Twitter chalked it up to being done for lore reasons as he said "...only Keyblade Wielders can use Keyblades..." implying that Kirby isn't "chosen" as barer of one and thus has to use the three spells differently.
Kingdom Hearts

Disney heavily restricted the use of Mickey Mouse in the game especially as the central character like Square initially wanted. This was because Disney was nervous about the success of the game and did not want its possible failure leaving a black mark on the company and its mascot. Tetsuya Nomura stated that these contractual restrictions went so far as to have Disney allow Mickey to only make a minor cameo appearance (such as "in the background of a crowd" as Nomura put it). Spoiler:Nomura and his team got around this by having Mickey have an extremely important but brief role at the end of the game in the form of Mickey helping Sora close the Door to Darkness so no more Darkness gets out of it, separating the two. King Mickey even appears mostly in silhouette in this scene. When the first Kingdom Hearts became a humongous success for both Disney and Square, the former allowed the game developers to feature their mascot in a much more prominent role in the franchise from then onward.

In the Western version of the game, when an enemy is killed, it is possible to cut off their limbs during their death animation. This feature was censored in the Japanese version of the game to be inline with Japan's rating system.
Super Smash Bros. Ultimate

Attachment When the Hollow Bastion stage turns into the Dive to the Heart segment, the different Stations of Awakening in the background that reference key Kingdom Hearts characters appear to be censored of any reference to Disney characters such as Mickey Mouse, Donald Duck, and Goofy. However, these censored Stations are actually close replicas of the designs used on coasters that Square Enix produced in celebration of Kingdom Hearts' 15th anniversary, which also omitted the Disney characters.
