Crash Bandicoot 2: Cortex Strikes Back

Attachment Tiny Tiger was originally named Taz Tiger, since it was based off the extinct animal Thylacine, better known as the Tasmanian Tiger. His name was change possibly because it was to avoid any legal issues with Warner Bros., since there was another character that goes by the name of Taz, the Tasmanian Devil.

In the North American release, when you pause the game in the middle of Tiny's boss fight. His name reads Taz Tiger. This was a mistake Naughty Dog left behind and it was fixed in the European release.
Dead or Alive 4

Team Ninja wanted to include Master Chief in the game as a guest fighter. However, Bungie told them it was impossible for him to be in the game due to storyline restrictions but they were still interested in the idea of putting a non-cannon character from the Halo universe in the game. The end result was the character Nicole-458. In keeping with the idea of English being the language of the United Nations Space Command, Nicole is also the only character in the game that speaks English.
No More Heroes 2: Desperate Struggle

In an interview with Nintendo Power, SUDA51 revealed that Nathan Copeland was originally meant to appear in the first No More Heroes, but dropped him in favor of Death Metal. SUDA commented that, "When I started working on this installment, I knew right away that I wanted to bring Nathan back."
Shadows of the Damned

Attachment Shadows of the Damned originally started development as Kurayami. The story and setting were heavily inspired by Franz Kefka, and the game was originally meant to be free-roaming and have a "heavy focus on action".

The playable characters of Shadow and Relm are father and daughter. Never stated directly in game, it is told through flashbacks when you sleep at an inn with Shadow in the party. Later, in 1998 a developer confirmed this in an interview, saying that the scene was to go as follows:
Strago: I have one request... Show me your face. Even if you are him, I have no intention of wasting time trying to talk you into staying. I just want to know... for Relm's sake...
Shadow: ...... (He takes off his mask and shows Strago his face. However, his back is turned so that the player can't see.)
Strago: Thank you... Shadow. ...Come, let's have a drink.
Star Wars: Knights of the Old Republic II - The Sith Lords

Attachment Due to LucasArts rushing the game's development to release it on time for the holidays, a large number of content and story was either cut or unfinished with numerous bugs remaining in the game. M4-78, a planet inhabited by droids, was one of the worlds originally intended to be visitable by the player where they'd find one of the Jedi Masters and her apprentice.

Ripper Roo was originally going to speak fluent English. This was dropped for unknown reasons and was replaced with his manic laughter along with subtitles. The original recordings can still be found hidden within the game's code.

Attachment Mother 3 was originally going to be a 3D game for the Nintendo 64 DD add-on. Dubbed "Earthbound 64" by fans, the game was delayed several times due to the little experience the development team had with 3D games. The game would have possibly utilized the N64 DD's features and would also possibly use Mario Artist to create new in-game character faces. The Rumble Pak was also considered to be used during battle sequences. Due to the commercial failure of the N64 DD, the game was canceled but later recreated for the Game Boy Advance.

Rayman Origins

Attachment In the level Golly G. Golem, there is a Golem boss you must fight to progress. In the first teaser trailer for Rayman Origins, Rayman and Globox can be seen fighting a green, mossy version of the Golem which never appears in the final game. The cover of the game still features this early design for the character.
Street Fighter IV

Attachment Rufus' original concept art is completely different when compared to his final design. He was shown to be much slender, African American and wore martial artist robes with the words "King Cobra" on the belt. The final design was chosen because the developers wanted to leave an impression on players with his obese appearance and set him apart from the other characters.
Kid Kirby

Attachment A prequel to the Kirby series titled "Kid Kirby" was being developed for the SNES by DMA Design, now Rockstar North. The game was a platformer featuring a younger Kirby which would've utilized the SNES Mouse peripheral, however the game was canceled supposedly due to the mouse not selling well.
Prince of Persia: The Two Thrones

The last lines of dialog the Prince says are supposed to be the same as what he says in the first game, The Sands of Time, meaning he was telling the story of all three games to Spoiler:Farah after they defeated the Dark Prince and Zurvan. However, the last lines the prince says were changed. In the Sands of Time, his last lines were, "...You may wonder who I am or why I say this. Sit down and I will tell you a tale like none that you have ever heard..." In the Two Thrones, he says, "...You may wonder who I really am or why I say this. Come, and I will tell you a tale like none that you have ever heard..."
Twisted Metal

There were originally different endings for each of the racers. They were filmed with real actors and some differ to the final endings. The endings were dropped for numerous reasons, most notably, the campness and cheesiness of the endings themselves, the poor production quality, and the fact that they were using the working title of 'High Octane'. The endings can be found in Twisted Metal: Head-On Extra Twisted Edition.
Viewtiful Joe 2

In early trailers, a multiplayer mode was shown off. One player would play as Joe while the other player would play as Silvia. However, sometime during development, the mode was dropped.
Minecraft

Attachment Gears were a type of block that were available in Indev 0.31 which could be used in a similar way to redstone. They could only be placed on the side of blocks, and were never available in the release versions. In earlier builds players could hack the game to make them accessible, but the files were removed in later updates.
Super Smash Bros. Brawl

Attachment The Pokemon Trainer has an unused jumping animation. Some fans have speculated he may have been playable on his own, but it seems more likely that this jumping was planned to help him traverse large areas like The Subspace Emissary.
Metroid Prime

Attachment Kraid was intended to return as a boss, but was scrapped due to time constraints. Kraid's character model was completely finished, but the there wasn't enough time left to create a fully polished boss fight.
