Attachment Pokemon Gold & Silver started out as Pocket Monsters 2. It was planned to come out in 1997, but due to delays and other issues the name was dropped. One year later the game was re-announced as Pocket Monsters: Gold & Silver.
Metal Slug 5

Attachment In an unused debug level, you can find an assist character named Acchiles. He was left out due to Metal Slug 5 being rushed. In one of Metal Slug 5's flyers, he is seen along with an unknown character.
Super Smash Bros. Melee

Attachment The Pokemon, Ditto, was meant to be a possible helper when a Pokeball was thrown. He was supposed to transform into the player that threw the ball and would help fight. However, there were problems with its programming and it was removed. By using an action replay, Ditto can be put back into the game, but he is lacking his "transform" ability. Instead, all it does is say "Meta!" (it's Japanese name being Metamon) and disappears. Anyone who runs into it is dealt 7-8% damage.
Franchise: Halo

During the development of Halo 3 and Halo Wars, Microsoft gave the rights to different companies to make a Halo MMO, as well as and a handheld Halo game. Both of these were never finished.
Portal 2

Attachment The original idea for the game was to replace Chell with a new protagonist named Mel. Cave Johnson was to replace GLaDOS as the antagonist, and Mel was going to have many different personality spheres follow her (such as the Adventure Sphere, Betty, and one referred to as the "Morgan Freeman Sphere") instead of just Wheatley. Play testers were up for the idea of Mel until they became disappointed when GLaDOS woke up and didn't recognize the player as the character that defeated her in the first game. Mel was then put in the multiplayer before being scrapped entirely and replaced with Atlas and P-Body.

Platform: Philips CD-i

When the Phillips CD-i was first introduced in 1991, it was not advertised as a gaming console, but as a multimedia product to promote the CD format. Low sales caused Philips to try promoting it as a game console three years later.
Pokémon Crystal Version

Attachment Even though Pokémon Crystal is incompatible with the Game Boy, Game Boy Pocket, Game Boy Light, Super Game Boy and Super Game Boy 2, there is an unused Super Game Boy border programmed into the game. This can be seen by setting a flag in the game's RAM to enable the Super Game Boy features. Cheating devices such as the Game Genie can also be used to achieve the same effect temporarily. The border is a leftover from the Japanese version of Pokémon Gold; the design is the same and the text reads Pocket Monsters Gold Version, rather than the English version's title of "Pokémon Gold".

When the player attempts to play the game on a model of the Game Boy prior to the Game Boy Color, the error message "This Game Pak is designed only for use on the Game Boy Color." is output by the game, with the word Crystal spelled out using Unown. There is an unused color palette associated with this message, using an orange tint, which appears when triggering the unused Super Game Boy border.
Minecraft

Attachment The Leather chain armor is a sprite from Indev (an older test version of Minecraft's Survival mode) 0.31's items.png armor. It never made it to the release versions.
Resident Evil 2

Attachment The S.T.A.R.S. group photo has an unidentified member on the far left. It is speculated that he is either the predecessor of Rebecca Chambers or an early concept of Kevin Dooley, an RPD pilot who sometimes helped the S.T.A.R.S. team on their missions. The latter would make sense as there is an RPD helicopter in the background.
Resident Evil Zero

In the N64 beta of the game, STARS Bravo Team member Edward Dewey is found missing a right hand, after being attacked by Cerberus dogs. This would explain the severed hand holding a gun that would be found by Joseph Frost in the original Resident Evil.

In the final game he still has both hands as the remake of Resident Evil no longer has Joseph finding a severed hand.
Resident Evil 2

Two of the characters in the game, gun shop owner Robert Kendo, and police officer Marvin Branagh, originally had much larger parts in Resident Evil 1.5, with Robert Kendo (Originally named "John") helping Elza Walker out and Marvin accompanying Leon and Ada throughout his story. At one point in 1.5, it appears that Marvin, rather than Sherry in the final game, is infected with a virus and the player has to get him a cure.

In the final game, both die fairly early on, and the player even has to kill a zombified Marvin in their character's "A" scenario.
Prince of Persia: The Two Thrones

The Two Thrones started out as "The Kindred Blades". It had the same dark tone as Warrior Within, a different model for the dark prince, and some changes to the story (Spoiler:such as Kaileena unleashing the Sands of Time by killing herself in order to save the Prince, instead of being killed by the Vizier for him to gain god-like powers).
Final Fantasy XIII

Serah was planned to be the 7th member of the team, but due to issues during development, she was turned into an NPC. The idea of making her playable turned into a reality with Final Fantasy XIII-2.
Mass Effect 3

Attachment The Illusive Man was first intended to turn into a Reaper creature in Mass Effect 3. This plan was later scrapped as the developers wanted to give players the satisfaction of fighting a character they knew rather than a random creature, highlighting the fact that the Illusive Man's weapon is his intelligence, not his physical strength.
Final Fantasy XIII

According to the Final Fantasy XIII Ultimania Omega, at one point in time, Vanille was considered to be the official main character. The idea was scrapped as the development team had already released a trailer and art featuring Lightning taking the role.
Pokémon Ruby Version

Attachment Gastrodon and Shellos were supposed to appear in Pokemon Ruby & Sapphire. Due to time constraints they weren't added. Ken Sugimori only did one version of the Pokemon (there are two varieties in Diamond and Pearl).
Luigi's Mansion

Using an Action Replay code, it's possible to actually walk around E. Gadd's lab. There are two doors, one of which says "Training" that most likely went to the Training room, and one that likely went to the Ghost Portrificationizer room. There is also a functional ladder, but it doesn't lead anywhere. This suggests that in an earlier version of the game the player could travel to E.Gadd's lab by means of a ladder somewhere in the mansion.
Pokémon Red Version

Attachment The original Red and Green Versions were supposed to have a female trainer along with the male trainers, Red and Blue. She was most likely removed due to hardware restrictions, but is still seen on the official strategy guide and artwork. The character was used as the character 'Green' in the comic and 'Leaf' in the remake, FireRed and LeafGreen.
Metal Gear Solid 2: Sons of Liberty

Revolver Ocelot was going to Spoiler:have a mechanical hand after Ninja chopped it off in the last game. However, Liquid Snake was very popular with the fans, so Kojima decided to change the story so Ocelot received a transplant from the deceased Liquid Snake, creating Liquid Ocelot.
