Super Mario FX

Attachment Super Mario FX is a game speculated to exist for the SNES that utilized a more advanced version of the Super FX Chip. Although Shigeru Miyamoto has said that he thought about experimenting with a 3D Mario game using the Super FX chip, no evidence of such a game has ever surfaced. To complicate the issue further, it seems the name "Super Mario FX" arose from a misunderstanding about the Super FX chip itself. During its development, the Super FX chip was codenamed 'Super Mario FX', and the original line of chips have the word 'MARIO' printed on them.

You can see HUNK in the original trailer for the 3DS, even though he didn't appear until the HD version where he is an unlockable character.

By looking through the game's disc you can find different victory fanfares signed for each character, there's even fanfares for both Mewtwo and Roy. But in the final game, Meta Knight is the only character with a unique fanfare.

There is a cancelled sequel for Super Mario 64, commonly called "Super Mario 64 2" or "Super Mario 128" (not to be confused with the tech demo of the same name) for the failed N64 DD. Only one demo was ever made, but was never shown to the public. The only information known about the game is that Luigi was going to feature in it, and co-operative play was a speculated feature. Since these two elements feature in Super Mario 64 DS, it's possible that other elements from Super Mario 64 2 were incorporated into Super Mario 64 DS.
Crash Nitro Kart

Attachment Crash Nitro Kart was originally being developed by Traveller's Tales and was intended to be called "Crash Team Racing 2". Nina Cortex was also planned to make her debut, however when Vicarious Visions took over the development, Traveller's Tales instead used her for Crash Twinsanity.
Duke Nukem Forever

Duke was originally going to have a sidekick in the form of a gay robot. This wasn't to make fun of gay people, but to add depth to Duke's character. The idea was scrapped during development because the robot had a tragic ending, and the developers felt that Duke was more about action than tragedy.

"'Sexuality is part of the Duke personality.' The head of Gearbox explained that the original idea behind the sidekick was 'to explore how Duke would relate to a peer that might have a different sexual orientation.'"

"Ultimately the robot was in a situation where for in order for victory against the aliens to be accomplished, the robot's inner workings - a nuclear generator - had to be sacrificed."

"Duke is all about having a good time. Do you want to pay $60 to feel like crap? No!"
Super Mario 64 DS

When first announced, Super Mario 64 DS was named "Super Mario 64 x 4", and seemed to feature gameplay where the player would utilize all four characters at the same time. This early build was shown at E3 2004 before being altered to be more like the original.

Pneumatic Diversity Vent was introduced in a trailer at E3 as a way to move stuff around like turrets. However nowhere in the game can it be found. It was cut because the puzzle aspect was very limiting.

The game started out as an episodic downloadable title. After a delayed release, the game was made into a full-retail title.

According to Portal 2 writer Erik Wolpaw, Valve originally planned to include a gel that let you walk on walls.

Unfortunately, players would become queasy with its use. Valve tried reducing the frame rate and camera movement, but were eventually forced to drop the feature entirely.

Attachment Tiny Tiger was originally named Taz Tiger, since it was based off the extinct animal Thylacine, better known as the Tasmanian Tiger. His name was change possibly because it was to avoid any legal issues with Warner Bros., since there was another character that goes by the name of Taz, the Tasmanian Devil.

In the North American release, when you pause the game in the middle of Tiny's boss fight. His name reads Taz Tiger. This was a mistake Naughty Dog left behind and it was fixed in the European release.
Dead or Alive 4

Team Ninja wanted to include Master Chief in the game as a guest fighter. However, Bungie told them it was impossible for him to be in the game due to storyline restrictions but they were still interested in the idea of putting a non-cannon character from the Halo universe in the game. The end result was the character Nicole-458. In keeping with the idea of English being the language of the United Nations Space Command, Nicole is also the only character in the game that speaks English.
No More Heroes 2: Desperate Struggle

In an interview with Nintendo Power, SUDA51 revealed that Nathan Copeland was originally meant to appear in the first No More Heroes, but dropped him in favor of Death Metal. SUDA commented that, "When I started working on this installment, I knew right away that I wanted to bring Nathan back."
Shadows of the Damned

Attachment Shadows of the Damned originally started development as Kurayami. The story and setting were heavily inspired by Franz Kefka, and the game was originally meant to be free-roaming and have a "heavy focus on action".
Final Fantasy VI

The playable characters of Shadow and Relm are father and daughter. Never stated directly in game, it is told through flashbacks when you sleep at an inn with Shadow in the party. Later, in 1998 a developer confirmed this in an interview, saying that the scene was to go as follows:
Strago: I have one request... Show me your face. Even if you are him, I have no intention of wasting time trying to talk you into staying. I just want to know... for Relm's sake...
Shadow: ...... (He takes off his mask and shows Strago his face. However, his back is turned so that the player can't see.)
Strago: Thank you... Shadow. ...Come, let's have a drink.
Star Wars: Knights of the Old Republic II - The Sith Lords

Attachment Due to LucasArts rushing the game's development to release it on time for the holidays, a large number of content and story was either cut or unfinished with numerous bugs remaining in the game. M4-78, a planet inhabited by droids, was one of the worlds originally intended to be visitable by the player where they'd find one of the Jedi Masters and her apprentice.

Ripper Roo was originally going to speak fluent English. This was dropped for unknown reasons and was replaced with his manic laughter along with subtitles. The original recordings can still be found hidden within the game's code.

Attachment Mother 3 was originally going to be a 3D game for the Nintendo 64 DD add-on. Dubbed "Earthbound 64" by fans, the game was delayed several times due to the little experience the development team had with 3D games. The game would have possibly utilized the N64 DD's features and would also possibly use Mario Artist to create new in-game character faces. The Rumble Pak was also considered to be used during battle sequences. Due to the commercial failure of the N64 DD, the game was canceled but later recreated for the Game Boy Advance.
