Attachment Super Saiyan 4 Vegeta's Final Shine Attack is incorrectly colored Blue in this game, in Dragon Ball GT (where the attack comes from) it's green. What's strange about this is the fact that in it's predecessor this attack is the correct color.

During the cutscene where you inquire the location of a jet fighter in Jowai Resort, one of the subtitles for the Resort owner's dialogue reads "Hold on hold on, I can see that you really want to fly our jet so I'll tell you what. Go talk to Bob at his Roboshack. He knows about these locks". The "Hold on Hold on" part of this sentence was omitted in spoken dialogue.

The main menu icon for Duel shows Vegeta in his Majin Buu saga outfit fighting Goku. This is odd as this game only covers the events of Dragon Ball Z from the Saiyan Saga to the end of the Cell Saga nor are these clothes an alternate costume for him.

Right before Teen Gohan goes Super Saiyan 2 in his Dragon Universe his first piece of dialog when talking to Android 16 is not voice acted and the second piece of dialog when he transforms into Super Saiyan 2 is in Japanese.

In the demo version, a cutscene in Blackwater City shows Darla Gratch says she's reporting for "Channel 64 News", but her microphone has a 2 on it. In the official release version, her small dialogue was change to "Channel 2 News", but her mouth doesn't match the words she says. In later games like Ratchet & Clank: Going Commando and Ratchet & Clank: Up Your Arsenal, she was reporting for "Channel 64 News", as was originally intended during development of the first game.
Sonic and the Black Knight

Attachment Sega held a competition for fan artwork of Sonic characters. The top twenty of the US, Europe and Japan were placed in the game for display and their artists would win a free copy of the game

However, one of the winning entries submitted by Antonino Miniero was discovered to actually be a replica of a piece of fan art by artist Mega777. Miniero was disqualified by Sega of Europe and did not receive a copy of the game.

Miniero later issued a full apology on the Sonic Italia message boards stating:
"I thought they would have turned to websites to check for any malpractice. When I found out I won, I cannot deny the fact that I was happy, but having won with that picture I was also afraid of such a reaction.
All this is absolutely not an excuse to justify my actions, I know I have been wrong and I take all the responsibility for what I did, accepting any decision made by SEGA and SonicItalia.
I want to apologize to all those who were offended by this, especially with the “21st” winner, it is my fault they did not win.
I officially apologize to MEGA777 for having soiled her masterpiece. I respect her very much, and for the fact that I had acted against her, albeit unintentionally, I feel deplorable."

Teen Gohan in all of his forms will say "All right Piccolo!" for his opening battle quote to Goku and Krillin. In the first game he would just say "All right" to Goku and Krillin. This mistake was fixed in Ultimate Tenkaichi by cutting off the word "Piccolo" at the end of the Sentence.

One of Gogeta's random opening battle quotes is "I am neither Goku nor Vegeta, I am the man who will defeat you.", but the subtitles say "destroy" rather than defeat.

Attachment For some strange reason, in the Spanish version of the Wii U game, the Temple stage is called "La Cúspide." "La Cúspide" means "Summit," and it's unknown whether the stage from Brawl would return, or just simply a translation mistake.

Some releases fixed this mistake, however, and the map is called "Templo" in those instances. (the correct Spanish name)
Kingdom Hearts: Chain of Memories

There is a wrong volume event in the game that causes the song, The Force in You, to not play one of its channels. This mistake was fixed in the game's remake.

Attachment Dr. Eggman's nose in this game is the same color as his face in this entry. This is noteworthy as Eggman Nega retains his pink nose, so this is likely a mistake as no other designs were changed.

By going to the northwest section of the map where the Ginyu Force resides, you can skip the segment, because there is a misplaced warp in that corner. This appears to be a mistake as you are supposed to fight the Ginyu Force to progress the story, and the warp isn't on land.

Demitri's Midnight Bliss attack turns Zangief into "Zangyura-chan, the little tangerine vending Russian girl". The name is based on a typo made by Gamest magazine while they were covering a Street Fighter 2 Turbo location test, stating that "By pressing all kick buttons, Zangyura can do a very quick Super Uriajjō, without collision detection in his legs" (ザンギュラはキックボタン3つ同時押しで足に判定のない、早めの回転のスーパーリアッが出せる。).

The visual design of Zangyura, as well as the fact she sells oranges is based upon Russian cartoon character Čeburaška, who in one film was discovered in a crate full of oranges.
