Attachment The creepers were based on a failed model of a pig. The length and the height of the pig model were accidentally inverted, resulting in a tall mid section with small legs.

"I don't have any modeling programs to do the models, I just write them in code. And I accidentally made them (the creepers) tall instead of long, so it was like a tall thing with four little feet. And that became the creeper. As opposed to a pig." - Minecraft creator 'Notch'
Sonic Generations

Perfect Chaos' appearance in this game is different to its appearance in Sonic Adventure; in Sonic Adventure, it was a creature made entirely out of water, whereas in Sonic Generations, it has scaly, reptile-like skin. According to an interview with Takashi Iizuka in the Prima Strategy Guide for Sonic Generations, Perfect Chaos' 2011 design is how the team had originally planned the monster; it had had to be changed in Sonic Adventure due to the technical restraints of the Dreamcast.
The Legend of Zelda: Majora's Mask
1
The development for Majora's Mask started out of a lack of enthusiasm to work on the unreleased expansion for Ocarina of Time for the Nintendo 64 Disc Drive, dubbed "Ura Zelda". Eiji Aonuma protested the expansion, so Shigeru Miyamoto challenged him to make a sequel for the game within 1 year, in exchange, they would no longer have to make Ura Zelda.

Resident Evil

Resident Evil's original Japanese name, Biohazard, was changed because trademarking "Biohazard" would have been too difficult, mostly due to a DOS game and a band of the same name. The name "Resident Evil" came from a naming contest held within the company.
Super Mario 3D Land
1
World 5-2 is designed in a Legend of Zelda format, using a top-down view, lots of rooms, and no stompable enemies. Also, if you light all of the torches in a certain room using a Fire Flower, a secret door opens and a Zelda sound effect is heard. The level was designed in honor of LoZ's 25th anniversary, hinted at by the level number: "5-2" is "25" backwards.
Franchise: Pokémon
1
Despite looking black-faced, Jynx wasn't meant to be racist. It is actually based off the Japanese Myth, Yama-Uba. Yama-Uba was described as having bright blond hair and a torn red kimono much similar to the design of Jynx.
Final Fantasy VII
2
During development, series creator Hironobu Sakaguchi's mother passed away. At the time, Sakaguchi wanted to craft a story that would show just because somebody has passed on, it doesn't mean that they are forever lost. He also wanted to present a realistic death, rather than a Hollywood sacrificial end. These two desires resulted in the concept of the Lifestream and Aeris' iconic death. The characters continuing to go on, attempting to save the world, after Aeris' death is a reflection of his own feelings coping with his mother's tragic death.

The "Combo" system developed for the game was a bug. A bug in the game's code enabled the player to "cancel" during the animation of some moves by performing another move, allowing for a combination of several basic and special moves. This "combo" system was later adopted as a standard feature of fighting games, and was expanded upon in subsequent Street Figher installments.
The Legend of Zelda: Ocarina of Time
1
Attachment The Iron Knuckles enemies are actually Gerudo Thieves in armor. Removing the chest plate reveals a curvy feminine figure, and since the Gerudo are all female it seems that this was intentional, and Nintendo didn't just aimlessly use them as a base.
Donkey Kong
subdirectory_arrow_right Mario (Franchise)
1
According to series creator Shigeru Miyamoto, Mario's appearance was the result of graphical limitations in the original Donkey Kong: his overalls allowed his arms to be visible against his body (as there was no room for outlines), his hat substituted for hair and eyebrows (and did not need to be animated when he jumped), and the large nose and mustache made up for the lack of space to depict a mouth.

Miyamoto additionally stated that the design was influenced by the decision to make Mario Italian-American, identifying his large nose in particular as a distinctly western feature.
