Sonic the Hedgehog 2

There is a Debug Mode code accessed from the Sound Test in the options menu that is inputted by playing songs in the order 01, 09, 09, 02, 01, 01, 02, 04. This is a reference to the North American release date of the game, which was on 1992/11/24, or November 24th, 1992.

The code to access the Level Select is 19, 65, 09, 17. These numbers reference Sonic developer Yuji Naka's birthday, September 17th, 1965.
Portal

Attachment In the Normal Test Chamber 17 there is a piece of the level that's out of bounds. It was only meant to be accessed in the Advance Test Chamber 17, which is unlocked after the game is completed. It contains a momentum test with a glass panel that has a hole in the middle. This room can be accessed with cheats by typing 'noclip' into the console. It is also possible to access the room without cheats by using the 'Portal edge Glitch' on PC or it can be accessed on console and pc using a glitch called 'Portal clipping'.
Super Mario World

Attachment A hidden level is found in the game's code, which seems to be a test level. All unused level sections are filled with this test level, so over 200 copies of this level are in the game.

Attachment Even after unlocking all 23 cheat options through the single player campaign, there is still an empty 24th space left in the cheat menu. This space is left for nine cheat options which cannot be unlocked during the course of the game. They can only be added to the menu using a GameShark. The cheat options include:

Super Smash Bros. Melee

Attachment Unused character intro loading screens for All-Star mode can be found by using the game's debug mode. They were most likely removed because the player may fight multiple opponents simultaneously in some rounds.
Kingdom Hearts

Attachment Disney Castle was supposed to be playable. It was shown in a beta trailer but the level was cut from the final version. However, through cheat codes, it can be accessed.

There are 2 unused abilities. No Ability and Cacophony.
No Ability is presumed to be an error handler, and is used when the game manages to find a Pokémon with an ability of ID set to 00. The term isn't used in the final game because all Pokémon were given abilities.
Cacophony is an ability similar to Soundproof, which isn't assigned any Pokémon in the game. It's assumed it was meant to be the ability for Whismur, Loudred, and Exploud, given their nature for noise-based attacks.
Animal Crossing

Attachment The NES games Balloon Fight, Baseball, Clu Clu Land, Clu Clu Land D, Donkey Kong, Donkey Kong 3, Donkey Kong Jr., Donkey Kong Jr. Math, Excitebike, Golf, Ice Climber, The Legend of Zelda, Mario Bros., Pinball, [urli=game/punchout/]Punch-Out!![/urli], Soccer, Super Mario Bros., Tennis, and Wario's Woods can be played in the game. They can be acquired through Game Boy Advance connectivity, e-Reader, and secret codes to Tom Nook. However some can only be obtained with the use of a cheat device which can also unlock all of them at once.
Half-Life

In original copies of Half-Life, During Chapter 3, there is a secret room in map c1a1c that can only be accessed by enabling cheat mode and noclipping through the floor. The room is made of tiled pictures of Valve co-founder Gabe Newell. The room still exists in later versions of the game, although the imagery of Newell has been removed and attempting to enter the room takes you back into the normal map.

Moon Knight and Colossus are not on the roster of playable characters in the PlayStation 2, Xbox, and PlayStation Portable version, but they can still be accessed on these versions by using an exploit.
Gradius

While porting the game to the NES, one of the developers snuck in a code that gave you all the powerups, due to his inability to play the game. He then forgot to take the code out.

The code later became known as the Konami Code.
BS Zelda no Densetsu: Inishie no Sekiban

After collecting everything in the game, two item slots remain empty - the Magic Cape and Book of Mudora, while coded into the game and fill these slots when hacked into the player's inventory, cannot be obtained normally.

The Magic Cape works as usual (you become invisible and invincible at the cost of magic power), but the Book of Mudora does nothing; as it fills the last slot in the A-button portion of the status screen, you were likely supposed to find the Book and either use it yourself (as in ALTTP) or give it to Zelda.
Super Smash Bros.

Attachment According to an interview with Satoru Iwata, Masahiro Sakurai intended for the characters to use Final Smashes in this game. It was taken out due to hardware limitations. This concept was held off until Super Smash Bros. Brawl. Going in the debug menu you can find 3 sound files. 2 of them of Ness saying "PK" and "Starstorm!". The third is Captain Falcon saying "Come On!".
Doom II: Hell on Earth

Attachment In the Doom II final level "Icon of Sin", the boss is supposed to be a giant demon head with a fragment missing from its forehead. It says, "To win the game, you must kill me, John Romero!", distorted and in reverse to sound like a demonic chant. One can use the "noclip" cheat to enter the boss and see Romero's severed head which is skewered on a post. John Romero is the creator of the Doom series. The staff didn't like Romero due to his egotistical personality. They decided to get him back by making him the final boss.
Banjo-Kazooie

The idea behind "Stop 'N' Swop" was to do certain challenges/objectives in "Banjo-Tooie," then stop and swap the cartridges while the system was still on. This would have unlocked special items in "Banjo-Kazooie." You can than swap the games again to get the items in "Tooie." This idea was scrapped when the window of time that players could "hot swap" games (the ability to switch games when the power was on) was shortened from 10 seconds to only 1 second. There are still cheat codes that will unlock the special items in Banjo-Kazooie.
The Legend of Zelda: Twilight Princess

It is possible that fire arrows were originally intended to be in Twilight Princess. Fire arrows can be found in the game's data. Though they can only be obtained with a cheat device and they do not function.
Mass Effect 2

It appears that originally the player could recruit Thane, Tali, Samara or Legion a lot earlier in the game; possibly directly after the prologue like Miranda and Jacob. By using an editor tool to unlock them from the beginning of the game, players will discover that the four have extensive dialogue for virtually every mission in the game, including missions which must be complete before their official recruitment missions. Furthermore, there are some unique scenes should the player bring Legion in particular along onto certain missions, such as characters commenting on his presence.

Legion's presence also triggers an exclusive Paragon Interrupt during Tali's recruitment mission. Upon meeting Tali, she will notice Legion and immediately draw her gun, triggering a Paragon Interrupt prompt. If the player doesn't press it, she will open fire at Legion who will run for cover.
Final Fantasy X

Cloud's Buster Sword was originally intended for use by Tidus but was removed before the game was released. It is still in the game's code, though, and using hacking tools, it can be obtained and used in the final version.
The Elder Scrolls V: Skyrim

Attachment In the games files there is an unused Pit or Arena Called "Windhelm Pit Entrance". Entering the Pit spawns 2 Argonians (Both Male), Nords, Dark Elves and Imperials NPCs, They are all called 'Pit Fans', who are supposed to be spectators. It is believed that Windhelm was going to have an arena at some point of development where you would face against animals or NPC Bandits. There are also Pit guards who are fully voiced, but they cannot be viewed during normal gameplay. The place can be viewed by entering the console commands "coc WindhelmPitEntrance" and "coc WindhelmPitExterior".
Half-Life 2: Episode One

Attachment There is an unused weapon with the file name of "#HL2_Hopwire". The weapon sucks up everything in its path into some sort of warp. It's not known how it would fit into the story, but it would probably have been used against Striders, based on some console codes that mention the weapon. It can only be accessed though console commands.
