Metal Gear Rising: Revengeance
1
Jetstream Sam was primarily designed by Kenichirou Yoshimura who had initially planned on leaving PlatinumGames early in the development process of the game after it was handed over to them. However, three days before he was scheduled to leave, Yoshimura had stumbled onto what some of the concept artists were working on, asking if they needed any suggestions. He was enraged that they weren't taking the Metal Gear concept seriously in the development process and that they "cannot place this in a Metal Gear game!" He decided to stay for half a year beyond schedule until he was certain that it was up to the standards of Metal Gear.

A NES version based on the film was also going to be released and developed by Bandai, however it was canceled.
Team Fortress 2

Attachment In the original poster for the "Dr. Grordbort's Brainiac Pack", the "Eureka Effect" is called "Wrenchmotron 600", and the "Eureka Effect" is the name of the taunt used by the weapon.
Team Fortress Classic
1
The Flamethrower in Team Fortress Classic once used a Gluon gun model from Half-Life during development. It's still possible to make the Flamethrower a Gluon gun by simply replacing the "egon.mdl" file in the "tfc" folder with one obtained from a copy of Half-Life.
Phalanx

Attachment The North American SNES cover of the game depicts an old bearded man playing a banjo with a spaceship flying in the background, an unusual departure for a side-scrolling space-themed Shoot 'em up.

According to Matt Guss of Kemco's American Advertising team, they wanted to make the game stand out more-so than other games of the same genre, saying they all had "monsters or spaceships or something [...] We wanted to create shock value so someone would have to pick the game up. We called it the 'Heavy Huh?!' factor." Art director Keith Campbell says they did the photo shoot themselves, hiring a model for the role of the "80-something hayseed."
Super Mario 64

Attachment After defeating Bowser, a key will appear with a number of coins flying outwards from it, but none of them actually fall onto the ground. This is likely a remnant of a past idea where coins would appear after Bowser's defeat, as seen in early pre-release footage. The idea was likely scrapped as the coins would serve little to no purpose after defeating Bowser, since Mario's health automatically recovers after collecting the key and returning to the hub world.
JoJo's Bizarre Adventure
1
Attachment In the 1993 JoJo RPG for the SNES, Midler was portrayed as a stewardess, making it the first time she had a physical appearance. Seven years later, when Capcom was producing JoJo's Bizarre Adventure, they wanted to include Midler as a playable fighter in the game. However, as she was only shown from the waist-down in the manga, and the game was a more faithful adaptation of the source material, they asked Hirohiko Araki to redesign her into something like a belly-dancer with her face covered up. Since then, as shown in the JOJO A-GO!GO! artbook, this new look is now her default appearance.
NieR
1
The development of Nier started after the release of Drakengard 2. The original concept was for a third entry in the Drakengard series. It was intended to be for seventh generation consoles due to the lessening importance of the PlayStation 2, which Drakengard 2 had been made for. However, as the project evolved, the original ideas were reworked and the game eventually became a spin-off from the main series. Despite this, the game's director Taro Yoko continues to think of it as the third Drakengard game.
Dark Souls II
1
In early concept art Lucatiel of Mirrah is shown using a thrusting sword as a weapon, but in the release version of the game she uses a greatsword and shield.
Super Mario World

Attachment The grassy pond located half way up "Kappa Mountain", found near the start of the game on Yoshi's Island, is named after the mythological "Kappa" from Japanese folklore, which is often depicted with a small pool of water on its head. The name "Kappa Mountain" only appears in the game's instruction manual.
Doom

Attachment The 3DO port of the game, which is widely considered to be one of the worst, had suffered due to the limitations of the system and misinformation from the publisher. According to Rebecca Heineman, who had worked on the port, the game's publisher, Art Data Interactive, told her before she agreed to work on it that the version already had new levels, weapons and features and that it only needed to be 'polished'. This was because they thought that to port the game, it was simple enough to take it from one platform to the other and compile the code, whilst adding weapons in it would be as simple as inserting artwork.

Heineman had to begin from scratch starting in August 1995 with a target to complete it by October. With no time to port the original music, she had to record new tracks with a band hired by the publisher. Heineman even had to write specific apps for the 3DO and Logicware in-game logos then delete them from the memory because the console couldn't handle logos. The floors and ceilings in the game were drawn with software rendering due to the 3D limitations of the platform.

Heineman released the source code for the port in 2014 on the website GitHub.
Ico
1
After two years of development, the team ran into limitations on the PlayStation hardware and faced terminating the project altogether. However, the team decided to remain true to Ueda's vision, and shifted the project's development to the PlayStation 2.
God of War: Ascension
1
Some reviewers issued complaints regarding the title for one of the game's trophies, named "Bros before Hos", which is received after graphically face-stomping a female villain. In response to the criticisms of misogyny, Santa Monica released a patch changing the name of the trophy to "Bros before Foes".
God of War
1
The game's developers described the gameplay as "merging the action of Devil May Cry with the puzzle-solving of Ico".
God of War III
1
Director Stig Asmussen revealed in an interview that it was originally intended that, if the player attempts the sex mini-game with Aphrodite a second time, the camera would pan back to the bed to reveal Aphrodite trying to kill Kratos, leading to a QTE where Kratos kills her. This idea was scrapped.
Franchise: darkstalkers

During the early stages of development, Lilith was meant to be a "male daughter" (Japanese term used to describe a man who dresses as if they were woman, known in the western world as a cross-dresser), as Lilith was designed to complement Morrigan. There was also a possible idea to have her gender labeled "unknown."
Super Smash Bros. Melee
1
8-Player Smash (a mode featured in Super Smash Bros. for Wii U) was originally planned for this game. However, due to technical performance, Sakurai stated "it was more than impossible."
Franchise: Uncharted
1
In Uncharted 2: Among Thieves and Uncharted 3: Drake's Deception, Drake will use the term "Kitty got wet". This is a phrase said in the co-op mode of Uncharted II, and within Chapter 9 of Uncharted 3. The phrase was improvised by Nolan North, and originates from a term used by his then-eight year old son while they were playing Wii Sports. After beating his father, Cooper North exclaimed that "Kitty got wet", referring to the fact that cats dislike getting wet, just as people don't like losing.
Uncharted 4: A Thief's End

Attachment Drake's costume went through a number of different designs, all specifically designed to not stand out. The designer wanted to have all of Drake's accessories and his outfit to not distract from the motion actor's work and Drake's facial features but also to portray that he had not been going on adventures for some time.
