During the game's development, two beta builds were shown off to the public. The first was aired on an episode of the kids' game show Nick Arcade; the other was shown off at a New York toy show and was actually stolen from the event. The ROMs for both the Nick Arcade and Simon Wai prototypes (the latter being named after the person who discovered the ROM image) are available online and feature tons of unused content, including the elusive Hidden Palace Zone.

Andrew Rannells, the award-winning actor who played the lead character Elder Price in the play The Book of Mormon, was the voice director for the English dub of Sonic X. He played characters such as Bokkun, Decoe, Gamma, and Mr. Stewart. He also played the G.U.N. agents in the game Shadow The Hedgehog. Rannells actually worked on quite a few 4Kids shows during his time there, including Pokémon, Pokémon Chronicles, Kirby: Right Back At Ya!, F-Zero GP Legend, and voiced characters in 4Kids-acted games in the Yu-Gi-Oh and One Piece series.

An outro for the Glimmer stage exists. The first time you play through the level it will lead you onto the following cutscene in the game, but completing and obtaining the Talisman a second time will show you the outro cutscene.

When you first start the game, Spyro isn't given a chance to introduce himself. Despite this, Pogo the Gemcutter already knows his name. In the Japanese version, however, he refers to him as 'Dragon-san' instead.

The 2008 E3 trailer for the game contains a Vault-Tec commercial which advertises a phone number. Calling the 1-888-4VAULT-TEC number will play an automated message saying:

"Thank you for calling Vault-Tec, your first choice in post-nuclear survival! We're sorry, but due to unexpectedly high call volume, all representatives are currently busy. Please, stay on the line and someone will be with you as soon as possible. There are 101 million callers in front of you. Estimated wait time is 78,643 hours. Thank you for calling Vault-Tec! Have a wonderful day!"
Fallout 3

When creating the gun-system for Fallout 3, one of teams first tests was taking a bow from Oblivion and making a Bow and Arrow "gun." It shot rapid-fire arrows and apparently "didn't work quite right", so the new gun-system for the game was built up from scratch.
The Elder Scrolls IV: Oblivion

The 2005 E3 demo video showcased a new system called "Radiant AI", which gave them general goals to achieve, how the goals were to be achieved was totally up on the NPC and the environment they were in.

This, however, presented problems when it was first implemented, as there were no "rules" for the NPCs to follow, only goals. This would cause many problems for the player including quest-lines being broken. Some examples are an unseen NPC going around town, buying up all the armor from the town stores, and another was an NPC important for the Dark Brotherhood questline would end up being found dead, as he was also a Skooma dealer and was killed by other NPCs needing the Skooma. As a result, many of the behaviors of the RAI had to be toned down significantly in the final release.
The Elder Scrolls III: Morrowind

Attachment There are three naked Nord barbarians that can be encountered on Vvardenfell that have each had a bad run-in with a witch that they escorted. In each case, the Nords lost all of their possessions, including their clothes, to the witches, and ask the player to help them with their predicament.

Later on in Tribunal, yet another naked Nord named Forstaag the Sweltering can be found in the western end of Mournhold's Plaza Brindisi Dorom. Upon talking to him, however, he will reveal that is he is not paralyzed, nor has he ever escorted a witch in his life. In fact, he claims he is naked merely because he feels hot (ostensibly the source of his nickname, "the Sweltering"). He even gets rather offended, claiming "you people think that every time you see a naked Nord barbarian, he's been tricked by some witch. So narrow minded."

If you continue to interrogate him, he will fiercely defend himself, claiming: "I told you I don't need any help. I'm just trying to cool off a bit. I've never even seen a witch! You people and your stereotypes...."

Later on in the game, after the attack on Mournhold, he will continue to defend his stance: "You'll notice I'm not blaming this on any witch. And it's still too hot around here."

Attachment The mysterious characters written on all the Dwemer pipes in the game are actually the word "Wormgod" repeated over and over again in Dwemeris Script. "Wormgod" was the nickname of Bethesda developer Gary Noonan on the Elder Scrolls official forums.
The Elder Scrolls IV: Oblivion

The NPC 'Jakben, Earl of Imbel' found in the Imperial City is actually an anagram of "Jack be nimble", from an old mid-19th century English nursery rhyme about a boy named Jack who would jump over candle sticks. He also owns a pair of unique boots important to the Thieves Guild questline which boosts the wearer's acrobatics skill by 50 points, allowing them to jump much higher than before. The name of the item, the "Boots of Springheel Jak", is itself a reference to a character from English folklore.
Fallout

Attachment There were plans to include a race of mutated raccoons called the S'Lanter, who had built a small community known as the Burrows. The S'Lanter and their home village ended up not making it into the game, for reasons such as not fitting in well in the Fallout world and also for time constraints, as the time needed to build and then animate the various versions of them was simply too much. The only remaining in-game mention of them is a Holodisk about FEV experiments located in The Glow.

The trap card "The Selection" erroneously states "By paying 1000 Life Points you can destroy all Monster cards in face-up position on the Field and prevent any cards of the same type from being Summoned, Flip Summoned, or Special Summoned."
Despite this being claimed in-game, the card still uses its actual effect.
