Luigi keeps his diary in a secret basement under his and Mario's house. If you manage to read the entries, you can find one entry reveals that Luigi is afraid of ghosts, and another where he says he wants to have the lead role in his own adventure. These diary entries are referencing Luigi's Mansion, which was in development at the time.
The Legend of Zelda: Ocarina of Time
1
Attachment The twins' house in Kokiri Forest actually has two pots, but the game background only shows one. While technically invisible in-game, Link can still roll in between the two pots, which will result in parts of Link disappearing behind where the second pot should be.
The Elder Scrolls IV: Oblivion
1
In the final Thieve's Guild quest, you're tasked with a seemingly dangerous escape in which the game recommends you equip the Boots of Springheel Jak (enchanted boots provided by the quest that add 50 to your acrobatics). Doing so will protect you from the fall due to a scripted event but the boots are destroyed in the process. However, should your actual acrobatics skill be high enough and/or you have sufficient health, the fall is easily survived without them and you can keep them permanently if you don't equip them before you jump.

Far Cry 3

There's a loot item named "Ripped Teddy Bear" which has the description:
An old teddy bear, eye missing and a musty smell. Stuffing is visible between the stitches. A worn, ripped label names the bear "Bo B-".

This is a reference to The Simpsons; Mr. Burns' childhood bear, Bo Bo, that was shown in the episode "Rosebud".
Final Fantasy VII

Attachment The inscription on Jenova's helmet is a reference to the real world, and read:
Made in Hong Kong
All rights reserved 1996
Squer Company Limited

It wasn't until 2005 (When a high-res image emerged) that this was found, 8 years after the game was released.
Pokémon Ruby Version
subdirectory_arrow_right Pokémon Sapphire Version (Game)
1
Attachment There is an unused battle theme for the three legendary beasts, suggesting they were planned to make a return after Gold, Silver and Crystal. The legendary beasts made a return in FireRed and LeafGreen, and could have been an implementation of the original idea to put them into Ruby and Sapphire. It's also possible the track was simply used for testing purposes.
Dark Souls
1
When originally announced, the game was titled "Dark Ring". The name was changed to Dark Souls because "Dark Ring" can be seen as a euphemism for an anus.

This was the second time the name changed. The first time was also unintentionally risque; "Dark Race".
Snatcher
1
While Hideo Kojima was developing Snatcher, he wanted to coat the game's floppy discs in a chemical that, when heated in the computer, would give off the smell of blood. This was to "give the stench of a murder scene".
Mortal Kombat
1
Johnny Cage is based on Jean-Claude Van Damme. The creators wanted Van Damme to be in the first Mortal Kombat, but Van Damme was unable to due to his busy work schedule. Johnny Cage's clothes in this game are almost identical to the clothes Van Damme wore in the last match in the movie "Bloodsport", and his Split Punch is taken directly from a move done by Van Damme in Bloodsport. Cage's Nutcracker Split Punch was also inspired by Bloodsport.
Borderlands 2
1
According to "Inside the Vault: The Art & Design of Borderlands 2" that comes with the special edition of Borderlands 2, when the boss "BNK-3R" makes a noise, it's actually the game's sound designer Joshua Davidson yelling about tea.

Ms. Pac-Man
1
Attachment Ms. Pac-Man originally started out as an unlawfully modified version of the Pac-Man arcade game called "Crazy Otto". Crazy Otto was bought by Pac-Man's North American distributor Midway, and rebranded as Ms. Pac-Man.
Resident Evil: Uprising
1
Attachment You can find a sticky note in the Raccoon City Police Department describing a recipe for a "Jill Sandwich". This is a reference to Resident Evil 1, where Jill is almost crushed, but is saved by Barry. Barry then says "You were almost a Jill sandwich!"
Drawn to Life: The Next Chapter
1
At the end it is revealed that Spoiler:all the game's events occurred inside a coma experienced by a boy named Mike. In an early cutscene, Heather tells Mari and Jowee to "Kemi-Kewa", which when unscrambled says "Wake Mike".
Dead Space
1
Attachment There's an achievement called "Live With the Hot Ones" for setting enemies alight with the flame thrower. This is a direct reference to an obscure line in the EA code of conduct. Every employee at EA is contractually obligated to "Live With the Hot Ones". No one had any idea what it meant, so it became something of an in-joke at EA.
Franchise: Pokémon
1
In the Japanese versions of Pokemon, the attack "Night Slash" is called "Blade Testing". This is a reference to an old practice that some immoral samurai held to test their new swords. They would wait alongside a road at night for a random passer-by, then attack them with the intention to kill. Night Slash also has a higher than average critical hit rate, which could be another nod to the story, as the passer by would be defenseless.
Super Mario World
subdirectory_arrow_right Super Mario World: Super Mario Advance 2 (Game)
1
Attachment In the Japanese version, Yoshi can eat the dolphins. This was removed from the international release. It's thought it was removed because of the different cultural views towards dolphins, or simply to make the level easier to finish, as the dolphins can be used as platforms. This was added back into all versions of the Game Boy Advance version.
The Legend of Zelda: The Minish Cap
1
Attachment In the first dungeon, you come across a red-capped mushroom enemy that walks around spraying spores. The enemy is named "Puffstool", and is actually a reference to a boss found in Pikmin also named "Puffstool" who sprays spores to infect Pikmin and turn them against you.
