Attachment According to Ken Sugimori, the Pokémon that appeared in the front page of the 1997 edition of MicroGroup Game Review showcasing Pokect Monsters 2 were not supposed to appear in the game at all. While some were never to be seen, Tyranitar ended up being used. This explains why Tyranitar wasn't in the leaked 1997 prototype ROM.
Sonic 3D Blast

According to programmer Jon Burton, Knuckles' bonus stages were planned to have their own music. However, due to memory constraints, the song couldn't fit onto the cartridge, and Knuckles' bonus stage simply use Tails' bonus stage music in the final game. The song was eventually repurposed in Sonic Adventure as "Pleasure Castle" (Twinkle Park 2).

In an interview, Noboru Sugimura stated that Leon was originally supposed to start out in civilian attire. There would've been a scene between him and Marvin at the police station where he'd change into his police uniform.

"It’s true that with Racoon City destroyed, the police wouldn’t be actively on duty, so there’d be no real need for Leon to change into his uniform… but we wanted to show his determination to protect both the city and his principles by having him change into his uniform regardless. It was a scene with a lot of meaning. Unfortunately, due to a problem with the CG it got cut."

According to an interview with Neil Druckmann, Cassie Drake was originally going to be a male character, but this was change their female lead character concept artist. The artist wanted Nate & Elena's child to be a girl, and Neil was convinced by her decision.
Tomba!

The game's director, Tokuro Fujiwara, decided to make Tomba! a 2D side-scrolling game because because of how many 3D games that let you move anywhere were on the market. He felt that 2D allows players to feel the fun and excitement in a straightforward way.

In an interview, Naoto Oshima was asked if the blue birds were a direct reference to the game Flicky. Oshima said yes, and also mentioned that he asked game's designer Yoji Ishii's permission to use Flickies in his work.

Sefton Hill stated on reddit that they were originally going to include an interactive combat battle against Rocksteady employees, in the game's credit. And each time the player got a knockdown, the developer's names and roles in development to appear during the slow-motion. It would've been used for the new game plus, but due to time constraints, they have to remove this feature.

Animal Crossing

In an early announcement trailer for the Nintendo Gamecube, Animal Crossing was called (or at least promoted as) "Animal Forest for Nintendo Gamecube", outright referencing the original Japanese title of the game (Dōbutsu no Mori or "Animal Forest" in English). This suggests that either the English localizers were thinking of keeping the game's original title, albeit translated, or the title "Animal Crossing" was not yet finalized.

Attachment The Wart Boss Fight was originally going to sport tomato projectiles like its original incarnation in Super Mario Bros 2 featured. They were replaced with cabbages in the final release, but the tomato sprite is still present in the game, albeit unused.

Attachment The Baby Yoshi seen in an egg half hatched on the game's Japanese title screen has an unused graphic of it's full body, suggesting that the game was originally intended to show Baby Yoshis both fully hatched and half hatched.

At one point, Mega Man Zero 4 was almost a completely different game. Rather than continuing where Mega Man Zero 3 left off, Takuya Aizu wanted to create "Mega Man Zero 1.5" in order to fill in the gaps between Mega Man Zero and Mega Man Zero 2. However, Capcom insisted on a true Zero 4.

"The development of Sonic 2 was informed by two factors: the things we couldn’t do in the first game, and the response/feedback from players. The idea of controlling Tails came from programmer Yuji Naka, who wanted a kind of “1.5 player” game—something where it would be ok if the second player died. He wanted to allow for a tandem experience where both skilled and unskilled players could enjoy the game together."
