Hey You, Pikachu!
1
The Nintendo 64 in the Japanese version looks rather unpolished and appears to have Super Famicom cartridges with it, which doesn't make sense since N64 cartridges are the same in design between America and Japan and Super Famicom games can be played when the Tristar 64 is attached but nothing seems to be attached.
Dark Souls

Attachment It is commonly speculated that Gwyndolin was a son of Lord Gwyn raised as a woman due to his affinity to the moon. Unsurprisingly this leads to him having feminine traits such as long hair, fair skin, and feminine clothing. But surprisingly he also has breasts. This is because the character model used for Gwyndolin is a female character model, despite the fact the character is actually male. This trait is also present in the Dark Souls Art Book.

Croc: Legend of the Gobbos
1
The game started out as a prototype for a new 3D Mario game in which the player played as Yoshi.

"The end came when we pitched to do a 3D platform game, the likes of which had never been done before. We mocked up a prototype using Yoshi. It was essentially the world's first 3D platform game and was obviously a big risk. [...] Nintendo had never let an outside company use their characters before, and weren't about to, either. This is the moment the deal fell apart. We later made that game into Croc: Legend of the Gobbos for the PlayStation, Saturn and PC, which became our biggest ever game in terms of sales and also in royalties, since we owned the IP."
New Super Mario Bros. Wii

Attachment By getting 99 lives at any point in the game with Mario, he will start playing with his hat missing until he loses a life. This is just cosmetic as it changes nothing to the gameplay. This also only works with Mario as collecting 99 lives with any other character does nothing to them.

• The books in the Shinra Mansion library shelves form the letters "VII".
• The red plane in Costa Del Sol harbor has "VII" painted on the side.
• The Gold Saucer has seven areas of attractions.
• The Ancient Forest has seven treasures.
• The Turtle's Paradise awards seven items for completing the flyer-hunt sidequest.
• Aeris and Ifalna were held captive by Hojo for seven years.
• It's been seven years since Cloud left Nibelheim to join SOLDIER.
• Tifa spends seven days unconscious before waking up after the Weapons awake.
• Red XIII tells the party that Meteor will reach the Planet in seven days.
• Cloud levels up to level 7 in the first battle.
• If a unit's HP hits 7,777 during battle they enter All Lucky 7s status.
• The "lucky" handicap reel on the Battle Square is called Lucky 7.
• Most characters have seven different Limit Breaks. The exceptions are Cait Sith, who has two but with various different outcomes, and Vincent, who has four, but each form has two special attacks. This makes for seven characters who have seven Limit Breaks.
• There are nine main playable characters, but only seven are mandatory.

Attachment In the Japanese release, the game featured an e-reader option allowing players to unlock characters, courses and challenges in the game using special e-reader cards.

This feature was removed in the western release due to the unpopularity of the e-reader device in the west. All vehicles, courses and challenges were instead made available to unlock in the game via the completion of cups and challenges.

Attachment In the Arkham Penitentiary Control Room where Clayface is being contained, there is a strange inmate pacing around muttering to himself in one of the cells. Later in an interview with IGN, Miyamoto clarified that he did in fact like Donkey Kong Country, and that he worked very closely with Rare on the game.

Mt. Krakatroda's temple has pictures of Trode, from Dragon Quest 8, caring for his daughter Medea in her horse form. This is also the location where you receive the alchemy pot, Krak Pot, implying that this is the same pot used by the hero during Dragon Quest 8, and as such is the same world.

Zelda's Adventure

Attachment Nearly all of the scenery in the game were of shots taken of the Los Angeles area. Some of the background terrain textures were vacation type pictures taken from a helicopter in Hawaii by one of the developers before the game's actual development had started. The image's file sizes took up much of the game's RAM usage resulting in the slow scrolling backgrounds.
Sonic X-treme

Due to the stress involved with the game's production, Director Chris Senn actually grew physically ill, stating that he "took on far more than was healthy... and after 2 years I became extremely ill... a nurse told me he thought I had 6 months to live, actually. I lost 25 pounds, was sick all the time, had cramps... and still went in to work... all due to too much stress."

The alternate name of the Patriots, the La-Li-Lu-Le-Lo, was chosen because the English 'L' sound does not exist in the Japanese phonetic alphabet, making it impossible to say. However, since there is no such sound that cannot be pronounced in the English phonetic alphabet, the concept could not be translated over.
