Disgaea: Hour of Darkness

If the player loses in any of the fights with Vyers, also known as Mid-Boss, they are treated to a hidden ending, and are awarded with New Game+.
Mega Man X3
1
There are two hidden armor parts that X can receive.

The first is hidden in Doppler Stage 1, which is only available if X has found all items and has NOT taken any of the enhancement chips. The capsule contains a Hyper Chip that grants him gold armor and the effects of all the other chips.

The second is in Doppler Stage 2. If the player has killed Vile before this stage and has allowed Zero to survive, using Zero through the stage makes him engage a miniboss. The fight ends with him getting damaged, and he gives X his Z Saber before retreating. Without arm enhancements it works basically like how Zero uses it in game, but with the arm parts the slash becomes a flying beam like Zero uses in Mega Man X2.

The reason the boss cannot be fought when Vile lives is that the second Doppler stage is left in derelict condition, all the windows blown out by explosions. Presumably, this is because Vile, in his insanity towards X, did a tremendous amount of damage to the building before his arrival, and the flying miniboss escaped out the open window.
Dead Island
1
Attachment After starting a new game, if you choose to turn left instead of right as the game tells you to, you will see another hotel door open. Inside the room are the couple from the first Dead Island trailer, in which the couple fought off zombies to protect themselves, and their daughter, who tragically turned into a zombie during the fight.

Attachment In the first three versions of Street Fighter II, "World Warrior", "Champion Edition" and "Hyper Fighting", Ryu and Ken's Hadoken sprites would sometimes appear orange. Developer Akira Nishitani confirmed that this was added as an Easter egg, and wasn't a glitch as previously thought.

In later versions of the game, starting with "Super Street Fighter II: The New Challengers", Ryu would receive a legitimate orange Hadoken special move called "Shakunetsu Hadoken"
Star Wars: Rogue Squadron
1

There is a white cat that appears twice during the game. The cat appears first in File 1 at the beach. If Raiden attempts to attack the cat, it will dodge the attack and can even be forced into the ocean until it's completely underwater. The cat appears later during File 3. During Monsoon's pre-battle monologue, if Raiden looks to the right the cat will appear again and one of the assassins accompanying Monsoon will pet and play with the cat.

In StarCraft 2: Wings of Liberty's third campaign mission, there is a Tauren Marine hidden on the map. Clicking on him three times starts a countdown which will cause him to lift off in a portable toilet.

He can then be seen flying through space from the bridge of The Hyperion and on certain missions.

Dark Souls
1
In the Prepare to Die Edition, the player has an opportunity to save a young Sif from the Abyss. After doing so and summoning Sif for a boss fight deep in the Abyss, if the player has not yet defeated Sif in the present, the opening cutscene will change.

Sif appears to recognize the player, and is reluctant to do battle with them, but finds it necessary, likely due to Sif swearing to protect the grave or the Covenant of Artorias. This cutscene makes the hobbling animation when Sif is at low health even more heart-wrenching for some players.

In the second Sigma stage, once you get to a fork where you can go up or down a ladder, use your ice shot to freeze the bat and climb up the ladder at the top. Pass the obstacles, and when you are falling down, hang to the left. If you make it without losing energy (you can use an energy tank), there should be a Dr. Light capsule that will teach you the Shoryuken.
Street Fighter X Mega Man
