Tails' full name was originally only going to be either "Miles Prower" or "Tails" but not both.

Yasushi Yamaguchi, who originally designed Tails for an internal competition, named him "Miles Prower" as a pun on the term "miles per hour" but the Sonic 2 development team changed his name to "Tails". As an act of rebellion, Yasushi snuck the Miles Prower name into various concept artwork and even in-game tiles. Eventually the developers reached a compromise: "Miles Prower" would be the fox's real name and "tails" would be the nickname he commonly goes by. Sonic 2 also features a cheat code that changes any in-game display of "Tails" into "Miles."

Early designs of the Casino stage had heavy quantities of blue. This was changed as the developers through that it may be difficult to see Sonic.

Vanitas' name is derived from the Latin word for "Vanity" or "Emptiness". The name was chosen for its meaning, the similarity to the name "Ventus", and as it's a written pun on "Sora" in Japanese.

Tetsuya Nomura: I wanted to give him a name that meant "empty", and so I looked for something that would be appropriate. When I was searching I found a word that meant "emptiness" in Latin, "Vanitas". It looked a lot like Ventus's name, and in Japanese it has the kanji for "sora" in it, so I thought it was perfect. Of course in Vanitas's case it doesn't mean "sora [sky]" but "kara [empty]" [These two words are written with the same kanji, but are said differently and have different meanings.]
The Beatles: Rock Band

Plans for a Beatles themed Rock Band game happened before the first Rock Band game was ever released thanks to a meeting between Dhani Harrison (George Harrison's son) and MTV Executive Van Toffler at a Christmas Party in 2006.
Mortal Kombat

Attachment There is a sprite of Johnny Cage doing some sort of flip or flip kick left in the game. He does have a similar move in Mortal Kombat: Deadly Alliance as part of his Karate fighting style, and in Mortal Kombat (2011) as a special move. It's possible this was meant as a special move or Fatality.
Chrono Trigger

Two soldiers called Vicks/Biggs and Wedge appear in Chrono Trigger, as employees in Norstein Bekkler's Tent of Horrors. This a reference to both Star Wars and Final Fantasy VI. A V-Jump issue in Japan humorously state that after Tritoch made them disappear in Final Fantasy VI, they ended up in the Chrono Trigger dimension and were found by Norstein, who gave them their new jobs.

It was originally intended for Daniel Cross Spoiler:to have a final set of dying words when Desmond assassinated him. His last words were for Desmond to kill him as the pain of the Bleeding Effect had overwhelmed him, and was driving him insane, claiming that he constantly hears voices asking to be released.
Super Mario Bros.: The Lost Levels

According to an interview with Shigeru Miyamoto in Family Computer Magazine, World 9 of Super Mario Bros.: The Lost Levels, which is only accessible by playing the entire game without using Warp Zones, is based on a glitch in the original Super Mario Bros, which could allow the player to access 256 extra worlds.

Attachment The Space Bash mini-game arena is a based on the levels "Future Frenzy" and "Gone Tomorrow" from Crash Bandicoot 3: Warped. The background music is also a remix of these two levels' themes.

Attachment The developers hid a secret slideshow of their families and friends in the game's code in order to fill up extra memory. This can only be accessed through a button combination, but nobody at the company could remembered the code. It can still be accessed through Gameshark codes.
System Shock 2

System Shock 2, according to its pitch document, was originally titled "Junction Point". It was intended to expand upon the role-playing element of the original System Shock, which itself was intended to bring the gameplay of another of their games, Ultima Underworld: The Stygian Abyss, into a science fiction setting.

The first System Shock was generally perceived to be a Doom clone, and the developers blamed this for its limited commercial success. With Junction Point, their goal was to add significant role-playing elements and a persistent storyline so as to distance the game from Doom.
PlayStation All-Stars Battle Royale

According to Omar Kendall (lead game designer of PSASBR), at one point during development, Cole MacGrath and Evil Cole MacGrath were going to be one character, who would start out neutral, and turn good or evil based on the player's actions. After this idea was dropped, they decided to just use the Hero version of the character.

However, "the IP holders felt really strongly that the way that the Cole character manifests, he really is two different guys, and they felt the character would be best served as two different and they really pushed for it."

While Snake collects many weapons throughout the book, which describes him as a "walking arsenal", he never shoots anyone with them. Seth Godin, creator of the "Worlds of Power" book series, "wanted to minimize the death" seen in many of the NES games.
