Street Fighter II
The "Combo" system developed for the game was a bug. A bug in the game's code enabled the player to "cancel" during the animation of some moves by performing another move, allowing for a combination of several basic and special moves. This "combo" system was later adopted as a standard feature of fighting games, and was expanded upon in subsequent Street Figher installments.

The Iron Knuckles enemies are actually Gerudo Thieves in armor. Removing the chest plate reveals a curvy feminine figure, and since the Gerudo are all female it seems that this was intentional, and Nintendo didn't just aimlessly use them as a base.

The hotel in Celedon City has an invisible PC in the corner. If you stand one space down from the top right corner of the first room and press A, you'll be able to boot it up. It's assumed that this is a slight oversight, as the hotel resembles a Pokemon center, and the programmers removed the PC graphic but not its function.

Fallout: New Vegas
The project director for Fallout: New Vegas, J.E. Sawyer, released a mod for the game which brings more difficulty, shifts certain NPC alignments and modifies a few character stats.

The reason this wasn't released as an official patch was partly due to its technical difficulties and difficulty curve, however it was mainly due to the development for the game and all DLC having ended.
