There were two cut random character missions in The Ballad of Gay Tony. They both involved Luis running into ex-girlfriends. One mission has Luis taking the ex-girlfriend home and defending himself from her brothers, while the other involved the ex planting a bomb in Luis' car. It is unclear if these missions involved the same girlfriend, or two different ones.
Grand Theft Auto IV

The Rhino was originally going to appear in the game, but it was cut. There are still lines of code within The Lost and Damned files that show this.

Obsidian's Scott Everts attributes some of the performance issues and simplifcation present in the game to the fact it wasn't a PC exclusive.
Mortal Kombat

Attachment Data for an unfinished version of Skarlet can be found within the game's data. This version uses Jade's stance and Mileena's profile picture as placeholders, along with an additional costume not seen in both the home console or PC versions' DLC. This costume would later be made available in the PS Vita releases of the game.
Crash Twinsanity

Attachment There are several unused animations which can be found deep within the game's files. Most of the animations are unused attacks for Crash, nearly all of which have animations and are usable by modding the game's code. They were most likely removed due to time constraints.

There is an unused human zombie enemy in the game files for all three games. It is thought they were meant to be the end result of zombified stalkers who have lost the ability to operate mechanical devices.
The PDA Encyclopedia also contains an unused entry for them;
"The Brain Scorcher strips its victim of mind and reason, leaving only an empty, mindless shell."

While not included in the games, all the necessary files are contained in the installed data, making them easy to mod back in.
Sid Meier's Civilization V
1
Attachment Three unused World Wonders exist in the game's data. These are the Large Hadron Collider, which only has a completion theme and placeholder image, the Panama Canal, which only has an icon and completion image, and the Three Gorges Dam, which has an icon, image, and unused space for a completion theme.
Call of Duty 4: Modern Warfare
1
The Nintendo DS version of Modern Warfare began development as a remake of Geist for the GameCube titled Geist DS. Evidence of this can be found in the game's code.
Globetrotter 2
1
Attachment Within the game's files are three unused cities: Oslo (Norway), København (Denmark), and Stockholm (Sweden). Despite being unused, they appear to be fully playable if hacked into the game, have their own musical intros, and have batches of trivia questions like all the other cities.
The data for these cities seems to be left over from the original Globetrotter, suggesting they were planned to return like the other reused locations.
Demon's Souls
1
An early prototype of the tutorial level is still on the game's disc, and can be accessed via hacking tools. It's essentially the same level as the tutorial level in the final version, but with various differences.
The Elder Scrolls V: Skyrim
1
There are two scrapped features found in the game's files that would've occurred if you killed an NPC. The first was if you killed a unique NPC that has a family, then later you would see the ghost of that NPC follow any of their relatives, however only NPCs with generic voice files could do this, and only a single NPC in the whole game could turn into a ghost.

The second feature was after you killed a unique NPC who had a family, later talking to any of the NPC's family members will have them comment to you about their death in their hello conversation.

Attachment Inside of the Sixth House base of Sharapli (which can be found a bit east of Ghostgate), there is an interesting pool there named the Pool of Forgetfulness. Activating the pool does absolutely nothing, and there is no script associated with the item. It's possible the pool was once meant for a quest, or that the developers simply forgot about it.
