Space Invaders
1
Even though Space Invaders used some of the latest hardware for the time, its creator, Tomohiro Nishikado, discovered that the processer wasn't powerful enough to run the game as intended (with the aliens moving at a constant speed), and instead rendered the alien graphics faster when fewer were on screen. Rather than design the game to compensate for the speed increase, he decided to keep it as a challenging gameplay mechanic.
Grand Theft Auto IV

Attachment The official artwork for Lola Del Rio (which was used to promote the game) shows her with six digits on one hand; five fingers and a thumb underneath.
Franchise: Mario
1
According to Mario Bros. developer Takashi Tezuka, there was a stage in Super Mario World's development when Yoshi was a Koopa. A relic of this idea is Yoshi's shell-like saddle.
Plants vs. Zombies
1
Plants vs. Zombies was at one point going to be called "Lawn of the Dead", a pun on George A. Romero's Dawn of the Dead. The name was changed for legal reasons.
Resident Evil 4
1
Devil May Cry started out as Resident Evil 4. Producer Shinji Mikami felt it strayed too far from the series' survival horror roots and gradually convinced all the staff members to make it an independent game. The story was eventually rewrote to be set in a world full of demons and the hero's name was changed to Dante.

The Legend of Zelda: Ocarina of Time
1
Ocarina of Time was originally planned to be played in a first-person perspective, but it was changed to third-person because Miyamoto wanted the change from Young Link to Adult Link to be visible on screen. He also thought players should see Link because "he is so cool looking!"
Super Mario Bros. 3
1
The Koopalings were based on seven of the game's developers. They were all later named after musicians by the American localization team, as no distinct names were given in the Japanese version. It's also speculated the musical names were chosen because their scepters resemble microphones.
Collection: ToeJam & Earl
1
Rumours circulated that ToeJam & Earl were originally named FlowJam and Whirl. The story goes that when designer Greg Johnson told programmer Mark Voorsanger their names, Mark misunderstood what Greg was saying and coded them in the game as Toejam and Earl. It wasn't until they showed the game to SEGA that Greg realized the named were incorrect, but since SEGA actually like the name, they decided to keep them.

This rumour was confirmed as being false by Greg Johnson, who frequently joked with games press, giving them intentionally incorrect information, and was reaffirmed by the official ToeJam & Earl Twitter account in 2020.

In an interview between Shigeru Miyamoto and "Gamekult", Miyamoto states that he loves Disney, and their interpretation of Peter Pan inspired Link's design. This inspiration could also be why the Kokiri never age.
The Legend of Zelda
1
In an interview between Shigeru Miyamoto and "Gamekult", Miyamoto revealed that at the start of The Legend of Zelda's development, the pieces of the Triforce were actually intended to be electronic chips. The game was to be set in both the past and the future. And as the hero was the link between them, they decided to call him "Link".
Franchise: Mario
1
Attachment Shigeru Miyamoto had originally intended for Bowser to be an ox, and based him on the main antagonist from the animation "Alakazam The Great". This is likely why his modern design has horns and a defined muzzle, despite him being a turtle.
EarthBound
1
The main antagonist of EarthBound, an alien known as "Giygas", was made by Shigesato Itoi and described as "a living being that deserves love." While the character first debuted with a different design as the final boss of Mother, Giygas' appearance in EarthBound was inspired by a childhood incident where Itoi accidentally walked into the wrong movie at a theatre and watched the 1957 Japanese horror film "Kenpei to Barabara Shibijin". Itoi was traumatized by the film, where he confused a lovemaking scene that ends in murder as a rape scene. He says it had a "sense of terror having atrocity and eroticism side-by-side, and that's what Giygas's lines at the end are":

Ness, Ness, Ness, Ness, Ness, Ness, Ness Ness, Ness...
I...feel..g..o..o..d...
It hurts... It hurts...
I'm so sad, Ness...
Friends...
You cannot grasp the true form of Giygas' attack!
Go b..a..c..k
I am...H...a...p...p...y...
It's not right, not right...
Ness!

The Legend of Zelda: Twilight Princess
1
Attachment The 7th dungeon of Twilight Princess, "City in the Sky", is speculated to be based on a painting called "Another World" by M.C. Escher. The creatures, seen on the walls in the painting resemble the Oocca from the game. In the Twilight Princess dungeon, the Oocca are found running on the walls and floor.

SEGA was the second company (after Nintendo) to be in negotiation with Sony to make a new console, but the idea was eventually rejected by SEGA. It was confirmed by Tom Kalinske (an employee of SEGA) that the console specs SEGA had proposed were used for the PlayStation. It was also the technical achievements of SEGA's "Virtua Fighter" that made Sony focus on 3D rather than 2D. SEGA essentially sealed their own fate in the console market by rejecting the plans and inspiring Sony to focus on 3D.
Star Wars: The Force Unleashed
1
The Apprentice's nickname is "Starkiller". This name originates from early drafts of the first film, where the protagonist was named Annikin Starkiller.
Company: Sega

Mega Man Legends

In the Japanese version of the game, you have more freedom to "abuse animals". You are given total freedom in kicking animals and shooting birds without penalty. This was toned down in the international release, so that you could only kick the dull-colored dogs at the Old City area. Presumably because they are the only animal that can assault you, which would justify a retaliation.

Attachment There are at least two cases of art plagiarized from deviantart.com appearing in Sonic games. A piece of art of Sonia was mistakenly added to Sonic & Sega All-Stars Racing, and some fanart that was made for a Sonic and the Black Knight contest turned out to be a replica of someone else's artwork. Sega apologized both times.
Franchise: Mario
1
Mario was not Mario's original name, and nor was Jumpman. When Miyamoto created the prototype of Mario, he called him "Mr. Video" and planned to put the character into many games (possibly as cameos, as he did with Mario). But even before then Mario was called "Ossan" which means middle-aged guy.
