Attachment There was going to be a peripheral for the Atari 2600 called the "Mindlink" that made it possible to play games with your mind. Atari said the device would allow users to "think" the movement of objects on the screen. The product however simply read electronic impulses in the forehead whenever the user moved their eyebrows, rather than actual brain waves.
Uncharted 3: Drake's Deception
1
Attachment After the big bar fight in the beginning of the game, if you walk over to the other side of the bar instead of following Sully, you can find a newspaper that reads, "Scientists are still struggling to understand deadly fungus." This is referencing the fungal infection in the game "The Last of Us." This easter egg was planned to be removed by the development team before the game was shipped (as they were not planning to reveal the game until much later) but forgot about it and released the game with the easter egg still in.
Mega Man 3

Keiji Inafune considers this the least polished of the Classic series, as they didn't have that long to work on the game (roughly a year), and many things kept going wrong during development. Inafune told Nintendo Power, "If we had more time to polish it, we could do a lot of things better, make it a better game, but [Capcom] said that we needed to release it."
Metal Gear Solid 2: Sons of Liberty

According to the first volume of Metal Gear Saga, Liquid Snake, Spoiler:having faked his own death from the FOXDIE virus, was meant to be the main antagonist of the Tanker Chapter (an aircraft carrier during this part in development). However, this, along with most of the original plot, was scrapped due to tensions rising in the Middle East.

The original Xbox prototype was built using dismantled Dell laptops.

"In 1998, Berkes and his team ordered a few Dell laptops, took them apart and built the first prototypes of a Windows gaming console.

Ed Fries was leading Microsoft's games publishing business when the four Xbox founders pitched a "Direct X Box" based on the Windows DirectX graphics technology that was developed by Berkes' team."
Grand Theft Auto: Vice City

During early development, there were once bus shelters around the city. The model still exists in the game's code.
Oddworld: Abe's Oddysee
1
Originally, the scene where Abe escapes Rupture Farms was suppose to show a meteor shower hitting the moon which would slowly create the hand print seen in-game, which hinted at the greater powers guiding Abe through his adventure. Due to budgeting problems, the scene was shortened.
Mega Man Soccer
1
Attachment An early sketch from Rockman's Soccer shows that Guts Man was once planned for the game. He was replaced with Proto Man on the cover art.
Metal Gear Solid V: The Phantom Pain
1
This game is named after a real medical condition. Phantom pain is pain that feels like it's coming from a body part that's no longer there. Doctors once believed this post-amputation phenomenon was a psychological problem, but experts now recognize that these real sensations originate in the spinal cord and brain.

Attachment In the early Sonic games, The Chaos Emeralds appeared to be 'Emerald Cut'. Eventually, the emeralds in the series became 'Brilliant Cut'. In reality they should now be Chaos Diamonds, as a brilliant cut is reserved for diamonds.
Franchise: Fallout
1
Attachment From the ideas carried over from Black Isle Studios' Van Buren to Fallout: New Vegas, there were two specific characters that made their way into New Vegas: Arcade Gannon and Joshua Graham. In Van Buren, both were recruitable companions for The Prisoner. Arcade has barely anything about him written, suggesting that he was written in only a short time before the project was cancelled, but Joshua Graham-- then called "The Hanged Man", was much more fleshed out. He was to be first encountered in Fort Abandon as a man completely wrapped in bandages and being hung by the neck from a pole, and was described as being one of the most evil characters in the game. With Graham's appearance in Honest Hearts, it seems that The Hanged Man's personality in Van Buren is what inspired Graham's particularly atrocious behavior as the Malpais Legate in his backstory.
BMX XXX
1
BMX XXX was originally to be the third title in the Dave Mirra series, however when Acclaim started to make the game more adult-oriented, Mirra filed a lawsuit against the publisher claiming the title damaged his image and that his likeness and name were still being used with the game promoted as "Dave Mirra BMX XXX" even after he had separated from the project. According to Mirra, the game was supposed to be a mature, tongue-in-cheek game similar to the film "Airplane!".

The project was instead split into two with the second game "Dave Mirra Freestyle BMX 3" being released for the Game Boy Advance.

Franchise: Sam & Max
1
Lucasarts originally planned to make a sequel to Sam & Max Hit the Road entitled "Freelance Police". The game would have introduced both Sam and Max to complete 3D gaming animation. Many of the developers wanted to make Freelance police as a series of episodes. Due to creative differences between the developers on the game Lucasarts scrapped the game completely. It also led for Steve Purcell's copyright contract on Sam and Max to expire. The cancellation of Freelance Police led to the creation of the company of Telltale Games, who would later bring back Sam and Max through episodic gaming as well as introducing a new method of storytelling in video games.

Attachment The area where Punk Noodles is was originally just a small grass park with two statues. The statue model still exists in the game's files.
