Attachment The character design for Claudia went through many changes, including one where her head was shaved and her body was covered in tattoos. Spoiler:It was deemed that this made her malevolence too obvious, and they simplified her design.
WWF No Mercy

It was originally intended for the game to have Transfer Pak functionality, allowing the player to take their created wrestler in the Game Boy Color version and place him into the Nintendo 64 version. The Game Boy version was cancelled, but the menu can still be accessed using Gameshark codes.
FIFA 14
1
Attachment Inside the game's files is an unused preview image of the Adidas Brazuca, the ball for the 2014 FIFA World Cup. However, the ball uses the texture and model of the Adidas Cafusa.
BioShock Infinite

Attachment The Old Man Winter Plasmid used in the "Burial At Sea" DLC was created by Joe Trinder. He posted the design on the game's official forums. Trinder's design was immediately praised by other fans, quickly gaining attention on Reddit and even earning the approval of BioShock creator Ken Levine. Within a day, Irrational contacted Trinder and asked for permission to publish the poster on their Facebook page. A month later, the team bought the design from Trinder, and told him they planned to use it in the game. His initial version was for a Vigor, but it was changed into a plasmid to fit with the game's DLC.

Attachment Fire Blast's animation is based on the Daimonji of the Japanese festival Gozan no Okuribi, where a large bonfire is lit in the shape of the character 大 (meaning "big" or "great").
Silent Hill 2

Attachment Masahiro Ito, who designed the monsters for the game, said that he was inspired to create the Lying Figure when he saw a coworker walking toward him from a distance. The coworker was wearing a sweatshirt with a hood and had his hands in his pockets. He was listening to his Walkman and was walking in a "cool way."

Attachment Masahiro Ito's initial concept for the monster that would become Pyramid Head did not include the iconic helmet. The helmet was added to suggest pain.

According to an "Iwata Asks" interview, the reduction of Mr. Resetti's duties to an optional event (where they could only see him by developing the Reset Surveillance Center, which only appears as a Public Works Project after resetting the game once) was not only to mirror the Japanese government's budget revisions, but also because Resetti was considered annoying by many players, and made younger ones cry on some occasions. Yet, he still appeared due to the amount of positive attention towards him from the fanbase.

For The Legend of Zelda's 25th anniversary, Nintendo of America wanted to do a compilation release featuring various classic titles from the series, however series creator Shigeru Miyamoto and Nintendo's president Satoru Iwata were against the idea. This was because they already released the Super Mario All-Stars 25th Anniversary Edition a year prior and didn't want to do something similar twice in a row.
WWF Raw

There is an unused character found in the SNES version's data named "??Mystery??". He appears to be a placeholder for a 13th wrestler who wasn't finished. His sprites are borrowed from Shawn Michaels, but the palette used gives him grey hair and boots, pale skin, and black pants; and when performing certain moves, his sprites change to Bret Hart. He also lacks a finishing move, a theme song, portraits and a biography on the wrestler selection screen, and graphics for the Titantron that appears when winning a match. The Genesis version contains a variation, but this one resembles Razor Ramon.
Persona 4

The battle form of Spoiler:Mitsuo the Hero, the boss of the Void Quest dungeon, is in the style of 3D Dot Game Heroes, which was in development at the time.

One of it's attacks (Command) changes the battle command screen to that of the original Shin Megami Tensei.
Borderlands 2
1
Attachment The Bane side mission, and weapon, are both based off of the 1950's Western movie "Winchester '73" starring James Stewart. The movie revolves around a cursed rifle that leads to the death of anyone possessing it.

Attachment Tiny Tina's lazy eye is actually a bug in the software. Instead of fixing her eye tracking, the developers thought that it fit with her character, so they decided to leave it in the game.

"But since the team understood and liked the joke of Tina being hyperactive and crazy, they decided, fuck it, that's an awesome bug, and they kept it." -Anthony Burch

Mister Torgue is censored by a device in his voicebox, rather than being edited later. In his own words:
A LOT OF PEOPLE BEEN ASKING ME WHY MY VOICE BEEPS ALL THE F*CKIN TIME. THE TORGUE SHAREHOLDERS WIRED MY VOICEBOX WITH A DIGITAL CENSOR SO I CAN'T SAY STUFF LIKE SH*T, C*CK, OR P*SSY F*CKIN' D*CKBALLS! THAT'S LIKE HALF MY F*CKIN' VOCABULARY, IT'S GODDAMN BULLSH*T!
DuckTales: Remastered
1
Attachment A sprite of Scrooge in the costume he wore in the "Masked Mallard" episode of the Ducktales cartoon series appears in a documentary about the making of the game. This could possibly indicate that the developers originally planned to give the player the option to change Scrooge's outfit.

The GameCube is actually capable of rendering stereoscopic 3D. The GameCube was going to have an add-on that would attach to a TV and enable it to properly display 3D games. The first planned game for the add-on was Luigi's Mansion. The add-on was eventually cancelled as it would have cost more than the console itself.
Sonic the Hedgehog
1
In early screenshots of the Genesis game, a UFO could be spotted, but was not present in the final game. Yet when Sonic the Hedgehog was released in 2005 for mobile phones, the UFO sprite was found hidden away in the files, but still unused.
Primal Rage
1
The Primal Rage design team implemented an unusual and largely unknown security method into the arcade machine's coding that prevented the full features of the game from becoming active when emulated. Since the original programming team has since moved on to other things, they could not be reached for questioning regarding the game's security lock-outs. Those who have been contacted expressed no interest in unlocking the ROM BIOS for freeware distribution or security corrections. For these reasons, no perfect emulation of the original arcade game exists.

In the absence of these security protections, blood would be "censored" in that it would be colored a yellowish color with a brown shading. Also, fatalities could not be performed, and many special moves were not available. Another emulation problem was the player could no longer chain attacks, which greatly decreased the chance of performing a combo. These issues even affect the Midway Arcade Treasures 2 collection, which would supposedly feature an arcade perfect rendition of the game, but instead provides a censored and incomplete version.
Luigi's Mansion

Attachment An early, unused, model of Bogmire can be found in the game's files. This early model shows Bogmire with a slightly larger mouth than that of the final version.
Final Fight

Attachment Guy's name was originally spelled "Gay" in early development versions. As director Akira Nishitani put it, the development team wasn't "really aware of English spelling and pronunciation conventions." An unused graphic with the names "Gay, Haggar and Cody" can still be found in the game's code.
