Attachment During the opening, you are shown inkblots in order to test your mental health. Fans thought that one of the inkblots looked like two bears high-fiving, but this was not an option in the game. A fan decided to make a mod for the game which made 'Two Bears High-Fiving' an option. Obsidian referenced the fan reaction and the mod by putting a character called 'Two-Bears-High-Fiving' in the game's Honest Hearts DLC.
Donkey Kong 64

Attachment The Kong's weapons were originally real guns, as opposed to the comical fruit and nut based weaponry seen in the final game. It's thought this change was made to avoid controversy and ensure the game received a child friendly rating.
World of Warcraft

A 12 year old boy called Jørgen Olsen saved himself and his sister from a moose attack using skills he learned from World of Warcraft. Hans and his sister accidentally entered the moose's territory, and the moose attacked. Hans taunted the moose to take its attention away from his sister, and then proceeded to play dead, making the moose lose interest. In the game, 'Taunting' is used to get an enemy's attention, and 'Feign Death' tricks enemies into ignoring you.
Portal 2

Early in development, Portal 2 was planned to feature no portals at all. Valve focused on a new gameplay mechanic called "F-Stop", and though the new mechanic received some positive feedback, every single play tester was disappointed with the absence of portals. Valve then revised the game to include portals.
