Deus Ex: Human Revolution

Attachment The drop ship used to get around has the call letters "B-EE 008" on the side, but in the prerendered cutscenes (That were presumably made earlier in the game's development) the designation is "800 B-EE". It seems this was changed due to the fairly strong representation of the number 8 as a "B", meaning 800 B-EE would be pronounced "Boobie".
Mega Man 7
1
Attachment During the intro sequence, Dr. Wily is mostly covered by other graphics. Extracting his sprite from the game will show that he has distinct breasts. The orientation of his shoulders and arms means they couldn't be pectoral muscles. This could simple be poor rendering by the artist but given the precise nature of pixel art, it is more likely be intentional or possibly a joke by the developers.

K.K. Slider is based on the game's sound designer Kazumi Totaka. K.K. Slider's Japanese name "Totakeke" is taken from the fact that in Japan, last names are written first, so it would be Totaka Kazumi. That can be shortened to "Totaka K" very similar to "Totakeke". Totaka has also hidden music in every Animal Crossing game, and at least twelve other games. People now search for the hidden music in his games, which is known as "Totaka's Song".

Fallout 2
1
Attachment There is a special encounter that consists of a group of "Unwashed Villagers" fighting a character named "The Grim Reaper". This is a real world reference; one of the first fallout communities ever was called "The Unwashed Village Community" and this encounter is based off them. The people who you find in this encounter are members from the community, and actually fought a spammer name "Grim Reaper" who plagued the community for a long time, and was seen as an evil rival to the Unwashed Village Community.
Duke Nukem
1
Attachment Duke Nukem was temporarily renamed to Duke Nukum due to a concern that he shared the same name as a villain in the 90s cartoon "Captain Planet and the Planeteers". He was renamed back to Duke Nukem after it was confirmed that the Captain Planet show held no copyright to the name.
Tom Clancy's Ghost Recon

Released in 2001, but based in 2008, the game's plot revolves around Russian ultra-nationalists attacking 4 countries: Ukraine, Belarus, Kazakhstan and most notably Georgia. Oddly, tensions escalated between Russia and Georgie in real life, in 2008.

The Gyroids bear a strong resemblance to Haniwa; clay figures made for ritual use and buried with the dead. This explains why Gyroids can only be obtained by digging, and also implies that you have just dug up someone's grave. This likeness is referenced in the Japanese version of Animal Crossing: City Folk, where the auction house owner Lloid is named "Haniwa-Kun".

Attachment The twins' house in Kokiri Forest actually has two pots, but the game background only shows one. While technically invisible in-game, Link can still roll in between the two pots, which will result in parts of Link disappearing behind where the second pot should be.
The Elder Scrolls IV: Oblivion
1
In the final Thieve's Guild quest, you're tasked with a seemingly dangerous escape in which the game recommends you equip the Boots of Springheel Jak (enchanted boots provided by the quest that add 50 to your acrobatics). Doing so will protect you from the fall due to a scripted event but the boots are destroyed in the process. However, should your actual acrobatics skill be high enough and/or you have sufficient health, the fall is easily survived without them and you can keep them permanently if you don't equip them before you jump.

Attachment The inscription on Jenova's helmet is a reference to the real world, and read:

It wasn't until 2005 (When a high-res image emerged) that this was found, 8 years after the game was released.

According to "Inside the Vault: The Art & Design of Borderlands 2" that comes with the special edition of Borderlands 2, when the boss "BNK-3R" makes a noise, it's actually the game's sound designer Joshua Davidson yelling about tea.
