There's a secret ending in the game. If you talk to a man atop a climbable wall in stage 3, he will offer you a chance to partake in the Battle Arena. Doing so will warp you to an arena playing a remix of Vampire Killer from Castlevania, where you fight a three bosses:
A disco robot which explodes into fish pastries when defeated.
A zombie dinosaur robot pushing a machine-gun-toting carriage with a large slime monster inside.
A brain-robot which warps around the room.

After defeating the third boss, the player gets sucked into a portal and warped to the Mesozoic Era and becomes the king of monkeys after several years pass. The credits then roll.
Ogre Battle 64: Person of Lordly Caliber

It is possible to have one of the main characters, Diomedes, leave your party early in the game based on how you answer in game questions. He will then replace a boss later in the game.
Kingdom Hearts

The secret ending, "Another side, Another story," was not meant to be a real game. It was only there to show off ideas that Tetsuya Nomura had for the sequels. It was not until Kingdom Hearts 358/2 Days that the fight shown was playable.
Pokémon Colosseum

Spoiler:After you've caught every Shadow Pokémon except one, a trainer appears at Outskirt Stand. His entire team, except his Shadow Togetic, will be made up of the Pokémon that replaced the Cipher Admins' Shadow Pokémon during their rematches. Before the battle begins, he also states "I am you and you are me and we are all together," a line from The Beatles' song "I Am the Walrus".
Metal Gear Rising: Revengeance

On the title screen, before clicking start, inputting the Konami Code (up, up, down, down, left, right, left, right, B, A) will make Raiden say the title of the game, and all difficulties will be unlocked.
Banjo-Tooie

Attachment There was originally a mode called "Bottles' Revenge" which would allow for a second player to antagonize Player 1 in the main game.

In it, the evil spirit of Bottles is able to free himself after being killed by Gruntilda. Whilst this mode is enabled, Player 2 can take control of a nearby enemy to attack Banjo and Kazooie.

While this mode was cut from the game, it can still be accessed through the use of a cheat device. Devil Bottle's dialogue portrait also still appears in the Tower of Tragedy quiz game at the end.
Super Mario Bros. 3

Attachment For every 80,000 points you score, a matching card game will appear on the board. This is a chance to receive items such as coins and power-ups. There are only a total of Eight (8) different patterns.
Duck Hunt

There is actually a two player mode in which the second player controls the duck that player one is trying to shoot. This option is only available in game A. This was stated in the game manual, on page 3.
Star Wars: Jedi Knight - Dark Forces II

Attachment Early on in Level 5 "Barons Hed: The Fallen City", if you scare the villagers in Barons Hed, they will run away, and you can follow one fleeing woman who will open the locked door of a house near a bridge. If you can get inside the house fast enough, Max from Sam & Max, a franchise whose first game Sam & Max Hit the Road was developed and published by LucasArts, can be found inside sitting in a chair wielding a modified Bryar pistol (which is ironic since the Bryar pistol is already a modified Bryar rifle) that shoots highly-damaging green bolts instead of red. Max also has a high amount of health similar to that of a Dark Jedi.

Max does not play a death animation when killed. Instead, he just plays the same idle animation when he is first found, but takes on the attributes of a corpse, such as lack of collision. However if he is activated like a switch or door, he will get out of the chair and approach the room's door to exit, waiting for you to open it for him. Upon doing so, he will then wander the level, killing enemies and villagers alike. After Max has been let outside, the player can activate him again, letting them see from his view. A HUD message is also displayed reading, "It's MaxVision™!" If he is attacked by the player, he will return fire. If the player pushes Max outside instead of activating him, and attacks him, he will not fire back. If he is killed after being activated, he will freeze standing instead of playing any animation, but still becomes a corpse.

Max makes a second cameo appearance in Level 9 "Fuel Station Launch". During the final jump at the end of the level, you need to quickly snap back the mouse just before you land so you are facing backwards, which will allow you to see Max's face plastered three times on the front of the platform you just jumped off.

Attachment There is a hidden circuit in Punch-Out!! called the "Another World Circuit", which can be accessed by inputting the code 135 762 4680 on the password entry screen, then pressing A, B and Select simultaneously. The fights (in order) are King Hippo, Great Tiger, Piston Honda (Second Fight), Soda Popinski, Bald Bull (Second Fight), Don Flamenco (Second Fight), Mr. Sandman, Super Macho Man, and finally, a title bout with Mike Tyson/Mr. Dream, strangely labelled as "ANOTHER MCHAMPIONN." There is no extra content in this mode, but it does serve as a way to get to the later fights more quickly.
Crash Bandicoot

Attachment 13 years after the release of the game, it was discovered that there is an unused, fully complete level in the code of the game. The level is named 'Stormy Ascent', and was removed because of its difficulty.
The Legend of Zelda

If you name yourself "ZELDA," you can start the second quest right away.

If you play as either Jin or Xiaoyu 50 times, they will be wearing a school uniform. If both of them are wearing the uniform when chosen, the fight will take place on a school stage called "Mishima High School".

Attachment Even after unlocking all 23 cheat options through the single player campaign, there is still an empty 24th space left in the cheat menu. This space is left for nine cheat options which cannot be unlocked during the course of the game. They can only be added to the menu using a GameShark. The cheat options include:

Super Mario RPG: Legend of the Seven Stars

Attachment There is a optional hidden boss in Monstro Town named Culex, who is an homage to the Final Fantasy series. His appearance purposely breaks the style in Super Mario RPG to reinforce this homage.

To fight Culex you must go to Moleville and purchase fireworks, then trade them at the little mole's Pur-Tend Store for the Shiny Stone. Use this stone on the sealed door in Monstro Town. You'll end up in a battle with Culex, and if you beat Culex you'll get the Quartz Charm.

Culex's battle, victory, and post-battle conversation themes are (respectively) the boss theme from Final Fantasy IV, the victory theme from the Final Fantasy series, and the Final Fantasy main theme. Also, the Elemental Crystals that Culex uses are the same as in Final Fantasy and Final Fantasy IV.
Crash Bash

In the Japanese version of Crash Bash, in Battle or Tournament mode, hold down R1 + R2 + Left + Down at the same time on the character select screen to unlock Fake Crash as a playable character.
Fallout: New Vegas

In New Vegas, there is an enemy called the Y-17 trauma override harness. The suit was designed to make sure that the person inside would go back to the base he was stationed at in the event that he was unable to himself.
However this suit has some problems. It doesn't know if the user dies, and if it doesn't have a home base, it will walk forever until it's given one.

If the player has a special perk called "Wild Wasteland", they can find odd and out of place things in the game, and when fighting this enemy you will sometimes hear them say, "Hey, Who turned out the lights?"

This is a reference to the Doctor Who episode "Silence in the Library", in which the Doctor went and found some researchers who had special suits. The suits had a function in which it would keep the user alive after he died through a link called "ghost data", and could make them speak by retaining their consciousness. One of the first people who died would repeat the words, "Who turned out the lights" over and over again since the lights inside his visor were turned off.
Devil World

Attachment The main character of the game, Tamagon, appeared as a trophy in the Japanese version of Super Smash Bros. Melee. Tamagon was removed in the American version of the game but the trophy can still be found with Action Replay, complete with a translated description. Stranger still, the Tamagon trophy was removed outright from the European version of the game and cannot even be found with a cheat device, despite the fact that Devil World was released in European countries.
Resident Evil 4

Should the player repeatedly shoot the lake just before battling Del Lago, the monster will burst out of the lake and eat Leon in one gulp, killing him instantly. The HD and Steam editions of the game award the player with the trophy/achievement "Don't Shoot The Water!" for doing this.
Portal

Attachment In the Normal Test Chamber 17 there is a piece of the level that's out of bounds. It was only meant to be accessed in the Advance Test Chamber 17, which is unlocked after the game is completed. It contains a momentum test with a glass panel that has a hole in the middle. This room can be accessed with cheats by typing 'noclip' into the console. It is also possible to access the room without cheats by using the 'Portal edge Glitch' on PC or it can be accessed on console and pc using a glitch called 'Portal clipping'.
