The Grafted Blade Greatsword, which can be obtained by killing the Leonine Misbegotten boss at the end of Castle Morne in the Weeping Peninsula, is a reference to the Iron Throne from the book series "A Song of Ice and Fire" by George R.R. Martin, as well as its television adaptation "Game of Thrones." The sword was likely included in the game as tribute to George R.R. Martin's work as he worked with Hidetaka Miyazaki to create the lore of Elden Ring's world.

The sorcerous NPC character Selvius may be based on the enigmatic wizard Severus Snape from the Harry Potter franchise. Both have similar first names and Selvius speaks in a posh, judgmental, and condescending tone just as British actor Alan Rickman does frequently in his performance as Snape in the Harry Potter film series.

The description for the Blaidd's Armor accessory ends with the line "...but the cold never bothered him anyway." This line is an homage to a lyric from the song "Let It Go" from the Disney film Frozen.

The looter Patches, a recurring character in the Souls series, can be found as a secret boss fight Spoiler:near the large lake in Limgrave after following the connected river north to a small cave on the left.

As of Version 1.04.01, there is a glitch in which Malenia will regain health while attacking a player character that is in their long death animation.
