Attachment There is an unused Kill Icon in the game that suggests the Taunt item 'Shred Alert' was once planned to be a Taunt Kill, meaning that players within it's radius would be killed, similar to the Pyro's Armageddon Taunt.
LEGO Ninjago: Spinjitzu Smash

Attachment This game bears a striking resemblance to the Mega Man: Battle Network series. The first release of the game even contained leftover sprites from Mega Man: Battle Network 6, including Mega Man's speech portrait, various frames of animation from Falzar version's Aqua Cross, and a Mettaur sprite sheet.

There were supposed to be several different types of warthogs to be driven by the player before they were cut from game development. This included an Arctic Model that would later be included in the Avalanche DLC for Halo 3 and a smaller transport ATV that would become the Mongoose in Halo 3.
Super Smash Bros. Melee

Attachment In earlier versions of the game, the lab in Termina: Great Bay could be entered from both sides, and characters could stand on top of it. This idea was eventually canned and the laboratory was closed and moved to the background. However, the lab still has some textures and models of some pipes inside of it, likely remnants from the initial idea.
Super Mario Sunshine

Attachment Early in development, there was going to be an enemy named "Tramplin' Stu", another iteration of Stu in the game. The Tramplin' Stu was supposed to spawn Swoopin' and Strollin' Stu behind it as it walked. To defeat a Tramplin' Stu, you must fill it with water so that the outer shell pops, then you stomp on the eye on top of it. The data for Tramplin' Stu can be found in a test map where it shows up correctly, but without its shell. Tramplin' Stu shows up correctly with its shell when moved to the Delfino plaza map.
Halo 4

Attachment An unused model of Halo Reach's Scarab can be found in Spartan Ops Map "Harvester" data files. It wasn't meant to spawn any where in the map at any point.
Super Mario World

Attachment The Japanese box art of Super Mario World uses the subtitle, "Super Mario Bros. 4". Even though the subtitle isn't used on the Japanese title screen (or anywhere else) there is a graphic for the subtitle in the game's ROM.

Ultimate Mortal Kombat 3

By using a Game Genie on the Genesis, the player can use Chameleon. He is clearly unfinished as he has no combos, special moves, or fatalities, uses Scorpion's sprites (with the sprite changing to Rain's when a decapitation fatality was performed on him), and the announcer does not say his name. This hints that he was planned to be put in the game before he was debuted in Mortal Kombat Trilogy.
Jak II

Sandover Village can be accessed and explored though a glitch in the first cutscene. However, since the area was only meant to be used in said cutscene, the village does not have any characters and the levels Forbidden Jungle, Geyser Rock, Misty Island, Sentinel Beach, and Fire Canyon are inaccessible as there is nothing to call up for these areas.
The Legend of Zelda: Ocarina of Time

It was originally planned for the Great Fairy to be blue and white. The beta model can be found in the game's data.
Super Mario World

The game originally had a flying cage carried by four winged creatures. Mario would start already in the cage and with an autoscroll script, the cage would move along the level. Although the cage can be found in the code, the graphics for it are incomplete. The cage is likely left over from the 'SNES Test Program'.

Attachment There is an unused animated Yoshi egg texture in the game's data, suggesting that Yoshi was going to play a bigger role in the game, similar to Super Mario World.
