Bass's name is mispronounced throughout the game. In the game, his name is pronounced "bass" (like the fish), however his name is meant to be read as "base," since his and Treble's names come from the bass and treble clefs. This has lead to much confusion on how to pronounce his name.

The SNES version is missing the character of Guy. A later version of the game was released called Final Fight Guy, which replaced Cody with Guy and made up a new story to explain Cody's absence to the game. The game was sold in stores in Japan and was a rental only at Blockbuster in America.

The rock star Nikki's name is a reference to the rock star Nikki Sixx from Mötley Crüe. In Japan, the character's name is Slash as a reference to the English name of Soysau from Chrono Trigger, which is also a reference to the guitarist, Slash, from Guns N' Roses.

Attachment In 1993, Sonic the Hedgehog became the first video game character to appear as a balloon in the Macy's Thanksgiving Day Parade. The balloon popped on it's one and only flight by being pushed into a lamp post by strong gusts of wind, hurting a kid and a police officer on it's descent. Because of this incident, the balloon was retired.

A second Sonic balloon based on his modern redesign was used between 2011 and 2013 to celebrate the series' 20th anniversary, and was brought back one more time in 2021 in accordance with the series' 30th anniversary.

Aeris, after being brought back via a cheat device, has dialogue in discs 2 and 3. This seems to imply that either Spoiler:she was not meant to die at the end of disc 1 or that Spoiler:she was intended to be revived later in the game.
The Guy Game

One of the girls who appeared topless in the game was 17 years old at the time of development, making The Guy Game illegal to sell in countries where the age of consent was 18. However, this did not come to light until four months after the game's release, when the same girl sued the game's developers Topheavy Studios, Gathering of Developers (a subsidiary of 2K Games' owner Take Two Interactive which included Topheavy Studios), Microsoft and Sony (on which the game was released for the Xbox and PlayStation 2), as she was not informed that the footage of her would be used in-game and on the game's promotional website. A Texas county judge granted a temporary injunction against the production of copies of the game until her voice, name and image were removed from the game entirely.

The following year, Topheavy Studios released a DVD version of the game with additional content not featured in the original game titled "The Guy Game: Game Over", while taking the controversy in stride stating on the game's store page:

"The rumors are true! The most controversial video game ever created - The Guy Game - is no longer available!"

Attachment During a cutscene where 'The Chaos' attempts to grab Lightning, there is a hidden message among the yellow lines as she disappears. The message says "Pray for Japan" which was not known because it was written in a different script (Etro Script, a 3rd type of language used in Final Fantasy XIII). This is most likely a reference to the 2011 Tohoku Earthquake in Japan.
Mortal Kombat

In the audit menu of the arcade version, there is a useless row of text that says "ERMACS" (which stands for Error Macros) and is listed under "Reptile Battles." Because of this, players thought that Ermac was a hidden boss like Reptile. Ermac became a running joke for the developers, so much so that they even made him a real character for Ultimate Mortal Kombat 3, with his BIO saying he was hiding in the first Mortal Kombat.

Attachment The game was intentionally given controversial press coverage thanks to the game's PR expert, Max Clifford. Paid by Rockstar, negative press was used because they knew the controversy would help boost sales.
