In the level Dexter Industries, by the Factory Compound section, you should be able to enter the main building. Inside, you are able to see a hanging nuke. Shooting at the tip of the bomb will trigger a special cutscene and an instant game over.

A message in Japanese will show up with creepy sonic heads in the background. The message translates to "Fun is infinite with Sega Enterprises. - Madin".

The direct translation for the word at the end is Majin, but the context of the word makes it more likely to be spoken as Madin.
Super Mario RPG: Legend of the Seven Stars

Attachment There is a optional hidden boss in Monstro Town named Culex, who is an homage to the Final Fantasy series. His appearance purposely breaks the style in Super Mario RPG to reinforce this homage.

To fight Culex you must go to Moleville and purchase fireworks, then trade them at the little mole's Pur-Tend Store for the Shiny Stone. Use this stone on the sealed door in Monstro Town. You'll end up in a battle with Culex, and if you beat Culex you'll get the Quartz Charm.

Culex's battle, victory, and post-battle conversation themes are (respectively) the boss theme from Final Fantasy IV, the victory theme from the Final Fantasy series, and the Final Fantasy main theme. Also, the Elemental Crystals that Culex uses are the same as in Final Fantasy and Final Fantasy IV.
Mass Effect

Attachment The Shifty Looking Cow is a Space Cow found on the planet Ontarom, in the Newton system of the Kepler Verge. An otherwise normal member of the species, the Shifty Looking Cow likewise has a dialogue option that does nothing. When Shepard turns around, however, the Cow promptly begins stealing Shepard's credits slowly, and will even follow the Commander around. It can be found near the southwestern corner of the map, near a deposit of gold.
Crash Bash

In the Japanese version of Crash Bash, in Battle or Tournament mode, hold down R1 + R2 + Left + Down at the same time on the character select screen to unlock Fake Crash as a playable character.
Crash Bandicoot: The Wrath of Cortex

If you start a new file with the name "WOMBAT" you will get an immediate 106% completion rating. This is a reference to Crash's prototype name, Willie the Wombat. This does not work on the PlayStation 2 version of the game.

When you meet with Major Ocelot at Bolshaya Past Crevice, equip your Single Action Army Revolver, and begin to use the right analog stick to twirl the revolver. A hidden cut scene will occur where Ocelot will come out and twirl his revolver as well, and show off his freestyle tricks.

Attachment There is a hidden "Red Man" in the Corel Prison. The model is hidden from view in normal gameplay. There are no other models like this in the game, and it is thought to be a signature from one of the designers.

Attachment There are actually several floating doors in the Frigate level that are accessible if you perform a few glitches. The doors are left over from when the developers decided to turn them into doorways, but chose not to delete the doors for some reason. They still exist in the level's geometry, but only from certain angles.

In order to see them, turn on Tiny Bond from the cheat menu, and head for the first set of stairs heading down to the engine room. If you do the floating Tiny Bond glitch (crouch with R and C-Down) and walk down the stairs, you'll float over them and into the geometry of the level. Eventually you will see a door floating in space. If you open this door in mid air, go through it, and turn right, you will see a second door floating in mid-air as well. You cannot reach the second door without falling down, however.

If you plant remote mines on the floating doors, then go back around to the hallway and detonate the mines, they'll explode in front of you without any visible fireball. The explosion will merely send bits and pieces of debris flying everywhere.
Braid

Throughout the world of Braid are 7 extremely well-hidden stars, with an eighth one in the final level. Collecting the first seven stars will unlock an alternate ending to the final level, giving you access to the final star. The stars are so difficult to obtain that one star requires the player to wait on a screen for two hours just to get it, and another cannot be acquired if the World 3 puzzle has already been completed.
Fallout: New Vegas

Attachment On the eastern part of Camp Searchlight you can find a church with an accessible basement. Inside of the basement you can find, while having the Wild Wasteland trait, three Holy Hand Grenades (really called Holy Frag Grenades) that do massive damage. These grenades are a reference to Monty Python and the Holy Grail.

If you wait roughly 47 minutes on Sonic the Hedgehog 3's file select screen, the music changes. It's not fully known if this is an Easter egg or just a glitch.

Attachment In a dungeon of the game, if you pull a lever and stare off into the water for about 30 seconds, a monstrous squid with glowing eyes will slowly swim by. If you pull it and wait again, the squid will raise a tentacle to attack Ezio.
Sonic & Knuckles

Attachment When you arrive at the Sky Sanctuary Zone, Knuckles will jump over a big gap and create a bridge for you, allowing you to move on. He then sits down, exhausted, having you run after Dr. Robotnik without him. But if you instead turn to Knuckles and press down, having Sonic (or Tails) duck, Knuckles will see this as a bow, as if you're thanking him, and he will proceed to wave you off, as if he's saying "just go!".
Resident Evil 2

Attachment Normally, when the player searches the leftmost desk in the STARS office (which belongs to Albert Wesker), the player character will remark that the desk has been searched by someone else already. Should the player interact with it 50 times in a row, however, a roll of film can be obtained that, when developed, shows a photo of Rebecca Chambers from the previous game, wearing a basketball outfit. The picture is labelled "Recruit."

Once you beat the game 100%, you can find Fake Crash dancing in the levels Toad Village, Makin' Waves and Hog Ride.
Metal Gear Solid 3: Snake Eater

In Metal Gear Solid 3, you hear about a legendary sniper called "The End". He apparently can never miss a shot, will always kill his target no matter what from any distance, and without a spotter.

What isn't known is that his parrot is his spotter. If the player shoots the parrot in the battle, he has a harder time shooting you. He also becomes enraged.
