In Japan, the game was known as "New Legend of the End-of-Century Savior: Fist of the North Star". Since the international version of the game did not retain the Fist of the North Star license, the characters names were changed and minor alterations were made to the graphics, generally limited to changing the color palette of the characters.

Attachment As the game was based on the popular manga ''Hokuto no Ken'', enemies would explode violently in a cloud of blood when attacked in game. For the overseas version, "Last Battle", enemies simply fly off screen when hit.

Certain bosses were also recoloured to look more mutant in nature.

Two extremely obscure ports of the game for the Amiga and Commodore 64 were developed and released by Elite in Europe in 1991.
