Dee Vasquez was originally designed to be male, but this was changed after the developers realized Spoiler:that all of the killers were ending up as men during development.
Phoenix Wright: Ace Attorney
2
Attachment In the original art, Phoenix Wright has brown eyes as he does in-game. However, in the official art created for the DS release, he has blue eyes.
Adventure
2
Adventure was heavily inspired by Colossal Cave Adventure, a text-based game for many early personal computers.
BioShock
2
Attachment Before the Little Sisters became the "gatherers" in the game, early designs included a sea slug, a frog with a funnel in its anus, and a dog in a wheelchair.
Deadpool

Attachment Deadpool had a different costume in the teaser trailer.

Attachment Japanese rock artist Gackt once claimed in a TV program that Squall was modeled after him, even referring to the protagonist as "Gackt #2". However, the character designer, Tetsuya Nomura, says that he based Squall's appearance on the late actor River Phoenix.
Animorphs: Shattered Reality

According to Jay Barnson, the former senior programmer of SingleTrac, the game did not start out as a platformer and had a completely different plot. Due to the Columbine massacre and the media's claims of the video game industry being partly to blame for the tragedy, Scholastic and K.A. Applegate, the author of the book series, asked for the game to be redesigned so that they could distance themselves from anything resembling "violent video games", despite the book series commonly featuring violent acts.
Final Fantasy VIII

Attachment Tetsuya Nomura, the game director, said that Deling City's design was heavily influenced by Paris.
Danganronpa: Trigger Happy Havoc

Although the executions for Spoiler:Sayaka, Kiyotaka, Yasuhiro, Byakuya, Chihiro, Touko, Aoi, Sakura, and Spoiler:Hifumi were never seen in the game, each character did have their own personal executions designed for them, which can be found in the Official Danganronpa Art Book.

Spoiler:Hifumi's execution has one more detail of note; while it was never put into the game, it did appear in the manga adaptation of "Danganronpa 1 (Trial Version)", a demo released exclusively in Japan. Referred to as "Kiwotsukete, Buko-hime! Dai Kaijū Shinkō!!" (translated to "Watch Out, Princess Buuko! Great Monster Invasion!!), it involves a giant Princess Buuko and Monokuma fighting each other, with Spoiler:Hifumi caught in the middle, ending with him dying after being hit by 'Certain Kill' beams from both simultaneously. This makes it the only execution to never be put into the game, but still be put into an official source outside the artbook.

The design of the bad guy, Evil Otto, is based on the smiley face made popular in the 1960's. The developer, Alan McNeil, despised the image, because he felt it depicted a kind of "fake friendliness" he associated with businesses and salesmen. He made Evil Otto into a Smiley just to add a little sarcasm to the game, imagining the tagline "Have a Nice Day!", which frequently accompanied the smiley face, as here meaning "Have a Nice Day while I beat you to death."

Evil Otto's name, however, comes from the security manager at Alan McNeil's previous employer, Dave Nutting Associates: Dave Otto. Nobody at DNA liked Dave Otto, because he instituted strange and annoying policies, which Alan collectively described as "you are all guilty until proven innocent".
Final Fantasy VII
2
Final Fantasy VII heavily references the Lurian Kabbalah [kah-ball-a]. Kabbalah is a Jewish school of thought that also refers to the mysticism of Judaism and Gnosticism, a collection of ancient religions. Not only does FFVII draw from the beliefs of these religions, but it incorporates what actually happened to the followers in real life, as well as using Hebrew in the formation of character names. A few examples are:

• The Gnostics were persecuted, like the Cetra.
• Sephiroth's name comes from the term Sefirot (meaning "counting" or "enumeration"), which refers to the ten aspects of God in the Tree of Life.
• The sixth sefira in Tree of Life is beauty, known in Hebrew as Tif'eret {ti-fah-ret}. It is possible that this is how Tifa's name was derived.

Attachment A later publication of the game by Eurosoft features a cover taken from the book "The Howling III: Echoes" by Gary Brandner.
Maniac Mansion

In the Japanese version, entering "おわひ" (Apology in Japanese) will reveal the hidden message "ぱすわーどがながくてすみません" (Sorry the Password is so long), presumably a message from the developers, apologising for the game's up to 104 character long passwords.
Metal Gear

Attachment The book cover for the novelization of the original Metal Gear removed the gun from Snake's hand. While Snake collects many weapons throughout the book, which describes him as a "walking arsenal", he never shoots anyone with them. Seth Godin, creator of the "Worlds of Power" book series, "wanted to minimize the death" seen in many of the NES games.
Final Fantasy VII
2
Attachment Cait Sith resembles the Esper "Stray" from Final Fantasy VI, which was also called Cait Sith in the original Japanese version. The name itself comes from Celtic mythology, and means "faerie cat".
Q*bert
2
Q*bert's cube pyramid gameplay came from an "Escher Pattern", which looked like a screen full of cubes. Warren Davis saw it as a pyramid of cubes and as a programing exercise. Once Davis had a game design in mind, he asked Jeff Lee if he could use a few characters, which Lee was planning to use for another, unrelated game. According to Davis, Lee's idea for Q*bert's design was for him to shoot things out of his nose, which is why he has a long nose.
Final Fantasy VIII

The characters of Fujin and Raijin were initially designed to appear in Final Fantasy VII, however, the inclusion of the Turks made their presence unnecessary and they were shelved until Final Fantasy VIII. They were also most likely inspired by shonen-manga of the past, which ended up fitting much better into Final Fantasy VIII's cast.
Silent Hill 3
2
Heather Mason was named after Heather Morris, the person who provided the voice and motion capture for the character.
Silent Hill 3

Attachment Shingo Yuri, the character designer, based Heather's appearance off of Sophie Marceau and Charlotte Gainsbourg. Douglas Cartland was based off of Father Vito Cornelius from The Fifth Element.
