Secret of Mana

Attachment There are two unused items in the game's code. The first being a brownish orb which is named "?". Its animation is that of a curing item, but has no visible effects.

The second item is called "Ruby Armet". It is only equippable by Purim (the girl), and offers 46 magic defense and defense. The official Secret of Mana Guidebook still lists it as an available item, even giving it a price and concept art. It also includes this description:

重みを肩で支えることができる構造になっているため,力がなくても動きやすい中量級へルム。また中央にあしらわれた真紅のルビーは,炎のダメ一ジを軽減する効果をもっている。

Which translated says:

This is a midclass helm that does not take energy to move in easily because the structure is such that the weight is supported by the shoulders. Also, with the deep red ruby in its center, it reduces the effectiveness of flame damage attacks.

Attachment In Minecraft Pocket Edition, there's an unused camera. If hacked into the player's inventory, it can be used to take pictures.
Final Fantasy X

Cloud's Buster Sword was originally intended for use by Tidus but was removed before the game was released. It is still in the game's code, though, and using hacking tools, it can be obtained and used in the final version.

It is possible that fire arrows were originally intended to be in Twilight Princess. Fire arrows can be found in the game's data. Though they can only be obtained with a cheat device and they do not function.

Attachment Twilight Princess is one of the few Zelda games that does not have a magic meter. As such, there is no need for Green Chu Jelly, which is an item used to refill magic in other Zelda games. However, in the Wii version, it is possible to obtain Green Chu Jelly through normal means. But it is only possible once, in the "Cave of Ordeals", room 19. Carefully kill all of the purple Chus and let a Blue Chu and and Yellow Chu merge. Kill it and then scoop up the remains with a bottle. Link will now be holding a bottle of Green Chu Jelly. It has no text, description or effect whatsoever. However, performing this trick in the GameCube version produces a Purple Chu instead.
Pokémon Gold Version

In the code for Gold & Silver, there are various pieces of unused dialogue that refer to an item called "Sweet Honey." This implies that the honey mechanic was planned for Generation II but scrapped to return later in Generation IV.
BS Zelda no Densetsu: Inishie no Sekiban

After collecting everything in the game, two item slots remain empty - the Magic Cape and Book of Mudora, while coded into the game and fill these slots when hacked into the player's inventory, cannot be obtained normally.

The Magic Cape works as usual (you become invisible and invincible at the cost of magic power), but the Book of Mudora does nothing; as it fills the last slot in the A-button portion of the status screen, you were likely supposed to find the Book and either use it yourself (as in ALTTP) or give it to Zelda.
Super Mario Sunshine

Attachment In Noki Bay, once inside the bottle in the Red Coins in a Bottle level, Mario should fall to the bottom of the bottle and then locate a small rock formation there. If Mario maneuvers into a small square opening towards the back right of the rock formation, he can find a small door inside the opening which is impossible to open. Therefore, the player must rotate the camera to see what is on the other side of the door. In doing so, Mario can see a brown book on the floor of the room behind the impassable door, though its purpose is unknown.

The book itself may be a reference (or vice versa) to the level's background music. In the Original soundtrack, the title of the level's theme is in fact "The Book in the Bottle". It's also been said that this mission was originally different, and that Mario would collect the book rather than 8 red coins.

There are a number of unused items that can be acquired through the use of Action Replay. These include, among others, the "Weavile Fig" and the "Mime Jr. Fig", which are awarded during the game but never appear as items in the menu.

During development, the Silo mission was originally slated to be a Casino level to tie-in better with the movie. Various items can be found though the games code such as a gold bar, a token, and money.

Attachment There is an unused stone in the game's code called the God Stone. A gray recolor of the Light Stone/Dark Stone sprite, with the description "A phantom stone."
The Legend of Zelda: Majora's Mask

Attachment In a prerelease trailer for Majora's Mask, there seems to be an unused mask that looks like Adult Link for Ocarina of Time. This mask was rumored to be a transforming mask which would turn Link into Adult Link, allowing him to use certain items. The mask was removed before the final version and young Link is capable of using all of the items directly.
Dead Rising 2

Attachment The stuffed toy bear from the first Dead Rising game was apparently going to be an in game weapon again as it is seen in one of the reveal trailers and in the Fortune City Commercial trailer. It was later removed for unknown reasons (possibly replaced by the robot bear).

Attachment There are some The Wind Waker leftovers in the game. These are the Wind Waker item itself, Bomb Flowers and some other items. The file 'itemmdl.arc' contains models of 21 items from The Legend of Zelda: The Wind Waker which suggests Twilight Princess uses The Wind Waker's engine.

Attachment There are 9 unobtainable items within Pokemon Diamond & Pearl. Some were removed entirely, some were replaced with updated versions, and some are only available under certain conditions.

GoldenEye 007

The GoldenEye 007 instruction booklet lists a Soviet weapon called the Spyder. However, no weapon by the name of the Spyder is found in the final game. The name Spyder was actually dropped due to legal reasons, and the gun was renamed the Klobb after Nintendo of America's Ken Lobb, who helped the game during its development. The manual was simply printed without the correction made, or it was printed before the name change took place. Before being named the Spyder, the gun was also called the Skorpion, based on its real-world counterpart, the Skorpion VZ/61.
The Legend of Zelda: The Wind Waker

There's an item hidden in the game's code called the "Water Boots." They have no effect, but putting them on uses the same animation and sound as the Iron Boots. It's speculated that they may have been used for underwater swimming, or sinking underwater like the Iron Boots from Ocarina of Time.

Attachment If you you use an Action Replay, you can access the paper airplane item in your inventory. If you place the item on the ground, walk off screen, and come back, the paper airplane will duplicate itself. Pressing the A button while on top of the paper airplane will make it disappear visibly, but will still exist in the system's memory. Letting the airplane multiply too much can crash the game.
Sonic the Hedgehog

Attachment In Sonic 06's manual, there is a section about shields however they cannot be accessed during normal game-play. By modifying the game's code, they can be accessed but can not protect against damage.
