In the healing rooms of Mole Mania, you can pester the old mole after having your life bar healed, who will grow increasingly annoyed. If you try to speak to him 6 times in a row, the relaxing healing area music will be replaced with the tense music that plays when entering the final castle level, and the old mole will exclaim:

"Woo! You ungrateful mole! Just wait 'til I get my claws on you!"

The game will then cut to the Game Over screen.
Ghost of Tsushima

There is a hidden questline in the game that is not trackable and only progressed by finding letters, each one a Key Item referred to as "A Family's Escape", hinting to where the next one would be. This questline follows a family that is fleeing the island of Tsushima who are leaving notes for an unaccounted family member named "Toku". Spoiler:The fate of the family and of Toku are left ambiguous by the quest's end.

Attachment A secret message can be found on the title screen of the NES version of the game with the use of two controllers. At the title screen, select "CONSTRUCTION" on the menu. Press Start on Controller 1 to enter the mode, then press it again to return back to the title screen. Repeat this seven times. After the seventh enter-and-exit, hold Down on Controller 1 and press A eight times on Controller 2. Then, hold Right on Controller 1 and press B twelve times on Controller 2. Finally, press Start on Controller 1 to activate the message featuring an animation of a green droplet falling.

The most likely person as to the identity of "Open-Reach" is Tomcat System's founder and programmer Ryoichi Ookubo because of the similarities between "Reach" and "Ryoichi". It should be also noted that his name is hidden twice in the game's ROM, and from one fact pointed out by a Russian LiveJournal user, that the name Open-Reach appears as "OPR" on the title screen of another game Tomcat System developed, the 1991 Famicom game Castle Quest published by Hudson Soft.

As for Noriko, her identity remains unknown. In the comments section of the sourced video, one commenter claimed that Noriko was Ookubo's daughter who was three years old at the time of the game's development, while the aforementioned LiveJournal user claimed that Noriko was a romantic partner, claiming her to be a former Namco graphic designer named Noriko Ikegawa (whose name would later appear in the credits for the Nintendo game Animal Crossing as a model designer), but there is no known substantial evidence to support either of these claims.

King's Field: The Ancient City

In the King's Mausoleum, there is a very dark room you can go to that has several stone faces hanging from the ceiling, and a stone pedestal in the center of the room that can be set on fire to brighten it (to better hear this Easter egg, you should not light it, and turn off the music). If you stand in front of the pedestal, several distorted voices can be heard throughout the room. Reversing them reveals that they are actually voice clips of the Radio Operators from the English dub of Armored Core 2: Another Age.

Attachment In the level, "Wild Nile Ride", there is a hidden message hidden underneath the floor directly before the second Checkpoint crate; while impossible to view without a graphical viewer tool, the Prima Official Strategy Guide features it.

The message reads, "TATTA" with a Vicarious Visions logo and a crude smiley face.

Attachment There's a pretty mean-spirited item in the game simply called "ALF book", which is sold for $100 in one of the game's shops. The book displays a block of text explaining the game's plot, as well as some strange fourth-wall-breaking story about the making of the game. Although this item might seem harmless, it also boots the player back to the title screen without warning and effectively restarts the game.

Ratchet & Clank: Size Matters

In the Dream level, there is a Titanium Bolt that must be obtained by shooting a giant floating crab at a certain point in the level to get warped to a different portion where the Bolt is. It's initially unclear that the player is supposed to shoot the crab specifically due to the hitbox for the warp going beyond the crab, which already floats and disappears out of frame in a loop. However, scoping in on the crab will cause the crosshair to turn red, indicating that it can be hit. Sitting and doing nothing while aiming at the crab for about a minute will cause a hint to appear on the bottom of the screen: "Attack its weak point for massive damage."

This Easter egg went undiscovered until 2022, and is a reference to an infamous gaff during Sony's press conference at E3 2006. During a demonstration of Genji: Days of the Blade, the spokesperson presenting the game claimed that its battles were based on actual battles that took place in ancient Japan, just as a "giant enemy crab" appeared in the demo soon after, with the spokesperson awkwardly commenting on the crab's weak point.

There may also be a second reference to the event when returning to Pokitaru after completing the Dream level, where a lone Scuttle Crab will now spawn in a new spot on the beach.
