Grand Theft Auto III

Officially, the protagonist's name in this game is "Claude". Hacking into the game's files, one can find a text string saying "DEFNAME = CLAUDE" which not only made it clear his name was Claude, but also hinted at a removed mechanic allowing the player to rename the main character. As a result, many also believe he is Claude Speed, the protagonist of GTA2.

Sam Houser claimed that the GTA3 protagonist did not have a name as he was merely the player's avatar, but would later be named "Claude" as of San Andreas, as everyone had taken to calling him that.

Attachment The "Goblin Mask" is based on Blanka from Street Fighter.
This is furthered by a combo weapon utilizing this mask and a battery to create a weapon called "Roaring Thunder" which shocks all zombies in radius to the target.

Attachment The Famicom's final design incorporated this red color because of an order from Hiroshi Yamauchi, the president of Nintendo at the time. Yamuchi often wore a scarf of a similar color and decided to include this favorite of his in the system as well.

It should also be noted that at the time of manufacturing, red and white plastics were the cheapest color of plastic to produce.

Early in development, the developers made a replica of Crescent Island from Diddy Kong Racing to see if they could make a racing track of that size on the PlayStation.

Bass's name is mispronounced throughout the game. In the game, his name is pronounced "bass" (like the fish), however his name is meant to be read as "base," since his and Treble's names come from the bass and treble clefs. This has lead to much confusion on how to pronounce his name.

Mortal Kombat
1
In the audit menu of the arcade version, there is a useless row of text that says "ERMACS" (which stands for Error Macros) and is listed under "Reptile Battles." Because of this, players thought that Ermac was a hidden boss like Reptile. Ermac became a running joke for the developers, so much so that they even made him a real character for Ultimate Mortal Kombat 3, with his BIO saying he was hiding in the first Mortal Kombat.
Super Smash Bros. Melee
1
Because of Super Smash Bros. Melee's status as a launch title, it was specifically designed to showcase new features that the N64 wasn't capable of. This is why the game's opening sequence is a high-quality, full-motion video, which the N64 was incapable of producing. The developers at HAL even worked with 3 other production companies in Tokyo, Japan, to make Melee's opening sequence as outstanding as possible.

Melee was in development for 13 month's, and the game's director, Masahiro Sakurai, had no holidays and cut short his weekends to work on the game. He even went far enough to say that his lifestyle while developing Melee was "destructive", due to how he felt pressured to deliver a greatly improved game. In the end, Sakurai and his team's efforts paid off. Even after the release of Super Smash Bros. Brawl's release, Sakurai still dubs Melee to be the sharpest game in the series.
Franchise: Pokémon
1
Attachment The blue Pokemon mini was modeled after Wooper.
Duke Nukem Forever
1
Duke was originally going to have a sidekick in the form of a gay robot. This wasn't to make fun of gay people, but to add depth to Duke's character. The idea was scrapped during development because the robot had a tragic ending, and the developers felt that Duke was more about action than tragedy.

"'Sexuality is part of the Duke personality.' The head of Gearbox explained that the original idea behind the sidekick was 'to explore how Duke would relate to a peer that might have a different sexual orientation.'"

"Ultimately the robot was in a situation where for in order for victory against the aliens to be accomplished, the robot's inner workings - a nuclear generator - had to be sacrificed."

"Duke is all about having a good time. Do you want to pay $60 to feel like crap? No!"

Attachment Rufus' original concept art is completely different when compared to his final design. He was shown to be much slender, African American and wore martial artist robes with the words "King Cobra" on the belt. The final design was chosen because the developers wanted to leave an impression on players with his obese appearance and set him apart from the other characters.
Silent Hill 2
1
There is persistent butterfly and moth symbolism surrounding Maria, who has a butterfly tattoo on her abdomen. Spoiler:Butterflies are seen as a symbol of rebirth in some cultures. Since she is supposed to be the reborn image of Mary, she could be seen as a butterfly. Likewise, moths are seen as a symbol of death, and Maria transforms into a swarm of black moths for the final boss. This parallels how James killed Mary.
