Attachment In the E3 trailer of Epic Mickey, the Mad Doctor is seen in the black cape uniform like in the cartoon 'The Mad Doctor'. This was changed in the final game to avoid confusing him with the shadow blot.

The chance for bread to spawn from the Teleporter is 1/20, with a hardcoded exception for the Soldier where the chance is a much higher 1/3.

Super Mario World

Attachment An unused 5-up logo can be found in the game's data. When used in game, the logo doesn't show up correctly. You can get a 5-up by bouncing on Wigglers, or by using a Game Genie code. It's fully coded, but the actual way the graphics are displayed is incorrect, as their properties are pulled from the code after the table with the score palettes. The 8x8 tiles used are incorrectly flipped horizontally and vertically.
Super Smash Bros. Brawl

An unused graphic of the Mario icon from Super Smash Bros. Melee can be found in the game's files. The Mario icon could mean that all the icons were going to be reused from Melee, but it also could have simply been a placeholder graphic.

The Earth in the series logo is actually the famous "Blue Marble" photograph of Earth flipped upside down. Interestingly, the original photo itself was upside down. It was rotated 180 degrees so it had the same orientation as our maps, making it easier for us to identify.

The only Wormsong in the series to not originate from a game is Wormsong 2011. It was Team17's gift to their fans posted on their official YouTube channel. The song was later added to the PC version of Worms 2: Armageddon, known as Worms Reloaded.

Worms Armageddon

Worms Armageddon had two versions of the Wormsong included in the game. The main version, Wormsong '99, can be heard in the main menu playing in the background. The second version can be heard when viewing the statistics screen after a battle and letting the background music roll, but only in the PC version. This version has somewhat different lyrics spoken in a Scottish accent, similar to the Angry Scots soundbank available for the worms.

Wormsong '95 can only be heard if the original game disc is played as an audio CD in a CD player or the like. Despite essentially being the theme of the game, it cannot be heard during gameplay. However, the later versions (Reinforcements and United) added a Wormsong Instrumental Remix as the theme of the game, yet that version still lacked the lyrics.

While Wormsong 2003 usually only plays during the end credits of the game, you can still hear the beginning of the song in the title screen when using the official frontend music tweak. However, the demo starts rolling before the lyrics can be heard.

Wormsong '99 is the only Wormsong without a chorus, and is the only song that doesn't end with "WE'VE WON THE WAR!" This could be because it's the last wormsong chronologically, and the song in which Boggy B (the series' protagonist) supposedly dies.

Franchise: Worms

Wormsongs, essentially the theme songs of the games, have a chronological order different from their order of release. Wormsong '95 is logically first, since it is the start of Boggy B's warpath. Chronologically the last song is Wormsong '99 in which Boggy B (supposedly) dies. Though the order of Wormsongs 2003 and 2011 is debatable, they both take place in between Wormsongs '95 and '99.
Wario Land II

In the Game Boy Color enhanced releases of the game, while it's still possible to play the game on a Game Boy or a Game Boy Color, the save files for both systems were not compatible with each other. For this reason, when trying to play a Game Boy Color save file on a Game Boy (or vice versa) the game will go through a set of menus to delete the old save data to accommodate that of the other system.
