In the game's opening cutscene, Amigo uses his own maracas as a racquet and Dr. Eggman uses multiple racquets from his backpack. However, both of them use regular racquets during the actual game.
SpongeBob SquarePants: Battle For Bikini Bottom

Attachment Squidward's Clarinet can be found in the game's files, but Squidward is never seen holding or using it.
Rockman EXE WS

The very first line in the first level's text archive contains a message that says: Stage 1: Under Construction!

This message does not otherwise appear during the level and is the only level to have such a text. As it suggests, it was likely used when the stage was being constructed.
Super Mario 64 DS
1
Attachment Found within the game's data is an unused Red Koopa Troopa that can only appear through hacking methods. Unlike the green Koopa Troopas found in the game, the red ones will attempt to run at the player and knock them around similar to Bullies. When knocked out of their shells, they'll slowly walk back into them instead of panicking like the green ones do. In addition, their shells cannot be ridden. Instead, they will only slide along the ground like in previous Mario games, killing enemies in their path until they break against a wall. If a Red Koopa Troopa is eaten by Yoshi, he'll spew out fire like he does when eating one in Super Mario World.
Call of Duty 4: Modern Warfare

The game contains an unused perk called "Expose Enemy". It's code name is "specialty_exposeenemy".
Phantasy Star
1
The game contains unused items named "M SYSTEM" and "ZILLION" that have no effect. They are probably a reference to the Sega Master System and the game Zillion respectively.
Star Trek: Voyager - Elite Force

Attachment There are two unfinished items in the game's memory that go unused in the final game:
• Tricorder - The tricorder gives you a sensor display to the bottom right with dots representing friends and foes, and describes what you point at in more detail with the left mouse button. The item was finished up late and made officially available in the Virtual Voyager expansion.
• Hypospray - The primary trigger fires a spray of blue particles. Using the alternate fire shows it's clearly unfinished by only spraying at an extremely high rate without sound. It also has an early icon, showing a different font. Ironically, attempting to obtain one by cheating will parse "health" and the letters as a negative number, killing the player in the process.
Mario Kart Wii
1
Attachment There are 3 unobtainable item boxes located under the S curve of DK Summit.
BS Super Mario Collection

There are a few unused items from Super Mario Bros. 3 that can be spawned as the first item in your inventory using a Pro Action Replay, but using most of them will crash the game:
• An anchor
• A hammer
• A warp whistle
• A music box
Day of the Tentacle

There are item graphics for a diamond and a noose that are never used in-game. The diamond is likely related to the one used for the time machine, though it never actually appears in your inventory. A noose never appears in the game, however.
Mario Party Advance
1
The game contains an unused gaddget called "Toad Force V".
Final Fantasy II
1
The game files contain data for an unused weapon called "The Killer Bow".
Paper Mario: Sticker Star
1
The game contains several unused stickers:
• A bigger version of the Leaf sticker.
• A Piranha Plant sticker.
• A Rock sticker.
• A Spiny Egg sticker.
• A Super Leaf sticker.
S.T.A.L.K.E.R.: Clear Sky
1
The game contains leftover code for an unused ".223 pistol". This was most likely intended as a reference to a similar pistol in the Fallout series.
Tomba!
1
The games code contains over 20 unused items, 4 of which also have full descriptions.
Fallout 3

Attachment Concept art and unused models exist for cut weapons referred to as the "Kryolator" and "Kryo-Grenade." While the grenade itself made it into the Mothership Zeta DLC with a new texture and mesh, the "Kryolator" was ultimately cut in the final version of the game, existing as only a mesh with textures. However it did later appear as a usable weapon in Fallout 4.
Mega Man Battle Network 6: Cybeast Falzar
subdirectory_arrow_right Mega Man Battle Network 6: Cybeast Gregar (Game)
1
The game's Japanese release made use of the Game Boy Advance E-Reader, but the feature was removed in the international release due to the lack of the device's popularity. In the Japanese release, the Request BBS required the use of E-cards and it was possible to obtain Gregar and Falzar battle chips. In the international release, not only is it impossible to obtain these without hacking, but they were broken, causing them to crash the game if used.
Fallout 4
1
The games files contain unused, cut or unfinished weapons. Among these are;
•A mesh for a Ball-peen hammer, which was intended to be used as a weapon.
•An untextured, high-poly model of a kalashnikov rifle, similar to the Chinese assault rifle, as seen in Fallout 3.
•A rough Harpoon gun composed of Syringer and Assault rifle parts.
•A pistol variant of the cryolator.
•A mortar.
•A cannon which fires nuclear bombs.
•A pipe shotgun receiver, a shotgun variant of the other pipe weapons seen in game.
Fallout 4

A Vault 101 jumpsuit can be acquired via console commands, but it does not appear anywhere in the game otherwise. This jumpsuit's vault number is the same one that the protagonist of Fallout 3 starts in.
