Attachment Due to its close resemblance to the ADAM Harvester from the Bioshock series, Valve requested permission from 2K Games to add the Vita-Saw to the game. However, due to copyright reason, Valve can't sell the Vita-Saw in the Mann Co. Store.
Batman: Arkham City

The Joker infects Batman with the TITAN disease by using his blood. This implies that the blood-type of Batman is AB+, or that The Joker's blood-type is O-. Either that, or they share the same blood-type.

This is implied by Batman not showing any signs of rejecting The Joker's blood. The Joker also has a plan to infect all of Gotham City with the TITAN disease by shipping his blood to the Blood Banks that supply the city. This would further imply that The Joker is actually a Universal Doner, otherwise everyone in Gotham with a non-matching blood type would die of Hyperacute Rejection.

Attachment There are six unused announcer calls for Maven, Kenzo Suzuki, and Mr. America (Hulk Hogan). It's likely they were all meant to be playable characters during development, but it's unknown as to why they were removed. It's unlikely that this was due to their contracts expiring during development, as the game includes several characters on the roster whose contracts expired during development, such as Spike Dudley, Mark Jindrak, and Joy Giovanni.

Attachment Marcassin is a long, dark-haired man with a light complexion, considered beautiful by many characters in the game. This could be a reference to "Howl's Moving Castle", an animated film also by Studio Ghibli, as Marcassin shares the appearance of Howl, a character with long, dark-hair, light complexion, and is also considered beautiful by many characters in the film.

Attachment Diddy Kong Racing DS is somewhat infamous for introducing the newly redesigned Tiny Kong, who despite being originally conceived by Rare as Dixie Kong's younger sister in Donkey Kong 64, is now depicted as a taller teenager. This design would carry over into future titles such as Donkey Kong Barrel Blast and Mario Super Sluggers.

Contrary to popular belief, as revealed on a December 20, 2007 edition of Rare's Scribes, the decision to redesign Tiny into a teenager was not Rare's call, but Nintendo's, which is appropriate considering Nintendo has retained full ownership of Rare's Donkey Kong characters since the latter company were acquired by Microsoft in 2002, and thus now have full creative control on how they're presented and portrayed.

One detail that is worth pointing out, and may provide an explanation to why Nintendo had no qualms with redesigning a character who was meant to be Dixie's younger sibling into a teenager, is that the Japanese localization of Donkey Kong 64 reportedly makes zero mention of Tiny being related to Dixie, instead just being described by the manuals as just a "Charming girl Kong". It's also worth pointing out that every game that has featured Tiny Kong since 2002, such as Mario Super Sluggers, Super Smash Bros. Brawl, and Super Smash Bros. For Wii U has also made zero mention of Tiny's relationship with Dixie Kong, which adds credence to the theory that Nintendo opted to ignore and essentially retcon Rare's initial lore of the two being siblings.

Shen has a strong resemblance to Joe Musashi, the ninja from the Sega game series Shinobi. The two ninjas also share the same walk animation and dash.
Diddy Kong Racing DS

Attachment Dixie Kong and Tiny Kong replaced Conker and Banjo in the DS remake. This is thought to be due to a legal issue, as both IPs belong to Microsoft after Rare were bought by Microsoft in 2002.

Attachment There seems to be a blank sign and four little rectangular prongs floating right under the Bathysphere. It could be used for distance between the ground at the end, or a placeholder for when the Bathysphere was to be completed. It follows you from the start, all the way to the end of the little trip. It has no physics or textures.
Grand Theft Auto V

The mission "Grass roots - Trevor" is thought to be a reference to a scene in the movie "PeeWee's Big Adventure". Both scenes are hallucinations, contain clowns, and both have the same background music.

Attachment In the Suburban store that you visit in the mission 'Friend Request', one of the shirt stalls allows you to buy a parrot tropical shirt, similar to the one worn by Max Payne in Max Payne 3. Also, if you give Michael just a light scruffy beard while he's wearing his grey suit, he looks oddly similar to Max Payne at the beginning of Max Payne 3.
One Piece: Pirate Warriors
1
Attachment The first boss in the game, Pacifista, is the only boss in the game to take visible battle damage throughout your fight. Because of this, it's possible that early in the development of Pirate Warriors, they had intended to have visible battle damage appear on bosses/characters, much like another Tecmo game, Fist of the North Star: Ken's Rage.
The Elder Scrolls V: Skyrim

The quest "Mind of Madness" has parallels with the story "Alice in Wonderland", where Alice is instructed by The Mad Hatter to do several menial tasks while he has a tea party. When you do finish the quest, you get the staff of "Wabbajack", a reference to the poem "Jabberwocky".
Kirby 64: The Crystal Shards

Shiver Star, the game's 5th world, appears to be a frozen-over version of Earth, containing several uninhabited urban areas. Nintendo has confirmed that Shiver Star was once inhabited, but the climate drove them to evacuate the planet. This has led fans to speculate that Shiver Star is a post-apocalyptic version of Earth. Though Nintendo's intentions during the making of Shiver Star remains unknown, it can be inferred that the world is either a message about climate change, or the suggestion that the "Kirby" series takes place in our own universe.

Attachment There is a ring in both games called the 'Gridiron'. The design of the ring matches the designs of the football teams, the Green Bay Packers (IB1) and the New-York Giants's (IB2) rings, both of whom won the Superbowl in 2011 and 2012, respectively.
Metal Gear Solid 3: Snake Eater

A voice casting sheet for Metal Gear Solid 3, which was leaked prior to E3 2004, stated that The Boss was a Green Beret and Navy SEAL, and that she was of British descent. These details were never mentioned in-game.

Attachment The character Utsuho Reiuji has several sprites and animations that bear resemblance to the character Thouzer/Souther from Fist of the North Star. This includes one of her Spell Cards in the game, which bears a resemblance to Souther's Fatal K.O., "Nanto Hō-ōh Ken Ougi: Tenshou Juuji Hou."
Touhou Shinkirou: Hopeless Masquerade

Attachment One of Koishi's alternate color schemes resembles that of her sister, Satori. Interestingly, inverting Koishi's default colors bears a striking resemblance to her sister's clothing as well. The same can also be done to their official artwork from Subterranean Animism.
BlazBlue: Continuum Shift

In various joke endings throughout the game's story mode, it is shown that protagonist Ragna the Bloodedge has a fear of ghosts. This could possibly be referring to ghosts being immune to Ragna's brutish attacks, which rely on physical weaponry. It also works as a reference to Ragna's rivalry with main antagonist Yuki Terumi, who had inflicted psychological damage to him as a child.
