Call of Duty: Modern Warfare 2

There are multiple cut game modes, as revealed by the existence of sound files for them within the game's data. Some of these modes can even be played by hacking. These include:

Arena: A game mode with no respawning. The objective was to kill all of the players on the other team, but if there was only one person left alive on each team, a flag would randomly spawn and the first person to capture the flag wins.
Arms Race: Unknown. Only sound files remain.
Defcon: There would be 5 Defcon phases which would count down. Play is normal at five. At four, each player goes into "Last Stand" upon death. Three makes weapons deal more damage. Two reduces the requirements for a kill streak by one kill. Defcon one disables respawning. After another minute, if one side has not eliminated the other, a nuke detonates, bringing the game to a draw.
Die Hard: Team Deathmatch with Last Stand available for all players.
Global Thermonuclear War: Involved protecting a nuke from the enemy team until the match was over.
One Flag: One team protects the flag, while the other team captures it.
VIP: Like Capture The Flag, except protecting a random player on the same team.

Attachment There is an unused red portal that can be found in the files of the game.
Final Fantasy IV

In the original SNES English release, the Cockatrice/Cocktric summon is dummied out of the game for reasons unknown. Its name, like many other dummied out items within the game, is "Dummy" in both the summon spell and the item it was learned from.
Mario Tennis

The Ring Tournament mode was removed from the Wii Virtual Console version of the game due to the online leaderboard being shut down. Its icon is still there, but it is skipped over in the menu. It was implemented again in the Wii U Virtual Console release.
Super Smash Bros. Brawl

Fox and Falco both have unused animations that appear to be their blasters misfiring.
Double Dragon Advance

A lot of content planned for the game was cut during development, including a playable prologue, additional cutscenes, improved enemy AI, lots of weapons and special moves, "Marian's Escape" mode in which the player would play as Marian escaping from her captors, "Renegade Abobo" mode in which the player could play as Abobo helping Marian escape, and the three main characters from Combatribes being playable in Survival Mode. In addition, Marian would have appeared in Survival Mode after every 20 beaten enemies to refill some of the player's health.
Call of Duty: Black Ops II

Attachment There was a mode cut from the game called "Hacker". The objective would have been to disable and hack neutral A.G.R.s (Autonomous Ground Robots). Points would be earned both for hacking the A.G.R.s and killing other players, with using the A.G.R.s to kill other players rewarding the most points. The only thing that remains of this game mode, however, is a cut menu icon.
The Legend of Zelda: Majora's Mask

Through use of Gameshark or Action Replay cheats, it is possible to get "Saria's Song", as well as the "Sun's Song" from Ocarina of Time. The "Sun's Song" has no difference from Ocarina of Time, and fast-forwards time to the next day. "Saria's Song", however, doesn't do anything.
The Sims 2

Many features introduced in the expansion packs, like weather from Seasons and driving cars in Night-life, were going to be in the base game. However, due to an office server room fire in Maxis, the data for these and many others were lost.
Mortal Kombat: Shaolin Monks

A hidden survival mode can be found in the foundry stage. It was apparently never finished, and was removed from the main menu. However, it can still be accessed in-game through a long and complicated procedure.
Thief

Valerie Bourdeau stated that the developers removed quick-time events, as shown in the original E3 trailer, due to the negative feedback that the gaming community gave.
Harvest Moon: Magical Melody

Attachment The Gamecube version of Magical Melody gives the player an option to begin the game as either a girl or a boy.
This gameplay feature was removed from the Wii port, so that only the male protagonist is playable. Any reference to the female protagonist was also removed from the cover art.
Platform: Nintendo 64

Nintendo removed the Nintendo 64's ability to use RGB output (SCART/Component), but earlier versions of the console still have the ability to do so. An RGB signal can be achieved on the N64 by simply jumping a few wires on the motherboard and adding a few more electronic components.
Call of Duty: Black Ops

There is a cut perk for the Zombies mode called "Candolier". It was most likely going to let players carry extra magazines, like the multiplayer perk Bandolier.
LEGO Universe

There is a lot of cut content that never made it in to the game, due to its closure. Some of the more interesting cut content includes a Kingdoms World, which would have included Guilds, PvP, Mounts, Crafting,and Animal Transformations. Capture The Princess, a PvP mode that would have been added to the Kingdoms world, and would have been much like Capture the Flag, and Nexus City, a world that would have eventually surrounded Nexus Tower.
Super Mario Bros. 3

Attachment As seen in early screenshots, Koopa Troopas and Hammer Bros. were going to host two mini-games. The minigames were a Question Block and a Dice Block game respectively, which also would have marked the first Mario game to have the Koopas standing instead of on all fours. These mini-games and their hosts seem to have been replaced by Toad.
Dark Souls

There is an unused Miracle called "Escape Death" in the game's files that acts as a Rare Ring of Sacrifice, letting the player die, but also keep all of their souls and humanity. It is likely that this was removed due to developers noting that it would remove the penalty of death, making the game unbalanced.
Mega Man Battle Network 5: Team Protoman

Attachment In the Team of Blues release, the Japanese version of Team ProtoMan, there exists a menu for using the Wireless Adapter, rather than the Link Cable, for multiplayer play. It's possible to access this menu through the use of hacking tools, although the feature is not actually programmed. Both the ProtoMan and Colonel versions can use the Wireless Adapter for the Crossover Battle with Boktai 2: Solar Boy Django, so it's unknown why this feature was dropped from the main multiplayer section of the game.

Attachment The game's files include an unused Fionna Button which is stored with the rest of the menu buttons. It's possible that it was intended for the game to allow the player to use her as a character. The game also includes an unused Boss Rush Mode.
Activision Anthology

There were originally plans for the game to allow you to download additional games from the game's website and play them on your system, but it was never actually implemented. According to the games producer, it was requested by Sony to be removed at the last minute.
