Rayman Legends

Attachment A leaked trailer for Rayman Legends suggested that it would feature special Near-Field Communication functionality on the Wii U, allowing new gameplay opportunities by placing certain objects on the controller. Ubisoft later confirmed that this trailer was merely demonstrative and consisted of prototype material intended only for their internal production teams, and did not represent what features the final game would have.
Donkey Kong Circus

Attachment The Game & Watch title Donkey Kong Circus, released 3 years after the original Donkey Kong, may be set as a prequel to the original Donkey Kong. It is said that it was Mario who unwittingly started the conflict by mistreating his pet, and the scene depicted in Donkey Kong Circus may be an example of this mistreatment, if Mario is taming him to perform rather than being a mere audience member. However, the relationship between the two in this instance seems to be good-natured.
Yoshi's Cookie

Attachment In the Game Boy version of the game, it's possible to access a debug menu using Game Genie codes. The top of the debug menu reads "Hermetica Debug Mode", suggesting that the title was originally to be called Hermetica, having no affiliation to the Mario series.
Grand Theft Auto IV

Attachment In the Ballad of Gay Tony trailer titled "Meet Luis Lopez", Luis is seen wearing the brown and black Perseus suit, but with blue jeans. This implies that it may have been one of Luis' already available outfits.
Super Smash Bros. Brawl

Attachment An unused HUD element named "InfAdvLevel0000" was apparently intended to display a shrinking bar for 50 frames (5/6 of a second) with a "level number" in a circle. The label "pp" is even more mysterious. The black areas of the textures would have been transparent in-game.
Biohazard i Survivor

i Survivor is the only game in the series to add new S.T.A.R.S. members to the franchise since the original game. This likely makes it non-canon, despite its otherwise rigorous adherence to the information found in the main entries in the series.
Tearaway

Attachment In one of the early trailers for Tearaway, iota is seen holding a pink and yellow swirly cone shaped item that would blow Scraps away whenever the player shouted into the Vita's microphone. The item functions similarly to The Squeezebox, an item that sucks in as well as blowing Scraps away when the L or R buttons are pressed.

The reason this item was removed is unknown, however it could have been replaced with the Squeezebox. Both items have similar functions, and the Squeezebox would be less tedious to use as the user wouldn't have to shout.
Grand Theft Auto V

At the Yellow Jack Inn, there are bloody footsteps around the 8 ball pool table in the back. This is a possible reference to the serial killer Merle Abrahams and his obsession with the number '8'.
Manhunt

It is likely that Manhunt is set within the same universe as the Grand Theft Auto series, which are both published by Rockstar. There are multiple references to stores and locations in both games, and the setting for Manhunt, Carcer City, is directly referenced in multiple Grand Theft Auto games.

Plants vs. Zombies

In the "Achievement" menu, continuing to scroll down the page, past the list of achievements, will reveal a number of characters and items referencing previous Popcap games.

At the very bottom of the page are Zombies dressed in Chinese attire situated in front of a pagoda, implying it is the other side of the Earth.
The Stanley Parable

At the beginning of the game, while the the narrator is telling the player to go to the meeting room, it is possible to look through a window and see an unnamed man walking down a hallway, if the player moves quickly. It is unknown who this man is, but it is believed to be either a fellow co-worker, or Stanley himself.
The Legend of Zelda: Ocarina of Time

After killing a Redead, any other Redeads near it will flock around the felled enemy and sit. It's speculated that the Redeads are either mourning the loss of a comrade, or are eating the dead corpse, explaining why it disappears.
Bionic Commando: Elite Forces

Attachment Inside the game's files are graphics from the Game Boy Color remake of Crystalis, which was released 5 months after Elite Forces. From the images, it appears that the title logo for the Crystalis remake would have resembled the original NES version, but the title screen used in the final product is completely different.

The most likely reason for the presence of these images in an unrelated game is because both games were developed by the same Nintendo subsidiary: Nintendo Software Technology.
Ultimate Marvel vs. Capcom 3

Several characters mention Mundus, the main antagonist of Devil May Cry, before or after fighting one of the characters from that series. Before the battle with Trish, Ghost Rider will say "I eat demons like Mundus for breakfast.". Phoenix (Jean Grey), after defeating Vergil, will say "Perhaps if I'm not too busy, I'll take care of Mundus, too." Dormammu also says to Dante that he should stick to fighting Mundus, implying not only that Dante is not strong enough to defeat him, but that Dormammu is more powerful than Mundus.
Fallout 2

The in-game description and model suggest that Bozar was intended to be a Sniper Rifle, rather than an inaccurate, automatic, heavy support weapon with a scope. It's likely that it was either repurposed during development, or mistakenly given the statistics of an automatic weapon and never fixed.
Gradius

Attachment Holding A+B while starting the game will display a hidden message: "KONAMI ONE GRADIUS ONE"

This appears to be a diagnostic screen, as some people have reported that it may say "TEN" instead of "ONE" on one line or the other. It is theorized that this screen displays the result of some kind of code verification algorithm or checksum, with each line representing the status of a different portion of the game's code.
Epic Mickey

Attachment In an early version, Gus was to be caged like all the other Gremlins in the game as opposed to being free by default. This may explain why Gus never introduces himself to Mickey, as the scene where he would have done so was cut and never replaced.
The Legend of Zelda: A Link to the Past

Attachment In the Swamp of Evil, where the Misery Mire dungeon is located, there are three enemies that do not load properly, rendering them invisible. Despite this, they can still attack and take damage, though will remain unseen by the player.

These invisible enemies are Ku, some of which can be seen in the Swamp of Evil. The ones that do load properly are positioned in the deep water of the swamp, while the invisible ones are located in shallow water. It's likely the enemies were only programmed to function in deep water, and thus either the terrain was different during development, or the enemies were simply misplaced.
