The Poison Mushroom in the Super Smash Bros. games is the only item borrowed from another series whose appearance does not stay true to its original franchise. In the Mario series, poison mushrooms have distinct colouring from other mushrooms, but in the Super Smash Bros. series, they look similar to Super Mushrooms in order to purposefully confuse players. The differences between the two items are nearly unnoticeable - the Poison Mushrooms are only slightly less red and have a meaner look.

Attachment The boxart for the North American Wii version of the game includes a spelling mistake on the back of the box, with the word "perform" misspelled in the line "and preform new moves".
Fallout: New Vegas

The Feral Ghoul Roamer has an error when loaded into editing software. While this doesn't cause conflicts in the game itself, the model attempts to draw from a model labeled "mmm_ghoulvariant_v1.nif". This is one of the feral ghoul variants that appeared in the Fallout 3 mod Mart's Mutant Mod, suggesting that Obsidian Entertainment had experimented with it while developing new features for the game.

Attachment The back of the game case shows four games with a screenshot on the top left supposedly from Sonic the Hedgehog 2. However, it's actually showing the image of the Flame Craft fight during Angel Island Zone Act 1 from Sonic 3 & Knuckles.

In the "Answer" part of the game ending, Mitsuru can be heard saying the line "Is that it?". However, the game shows Akihiko and his portrait while the line is being said.

During the final cutscene of the game, Spoiler:Sonic rushes past Elise, which then causes her to look around. After not seeing anything, one of the girls behind her asks "Miss Elise?". Although Elise replies "It felt like... someone was calling me," the subtitles accidentally include an extra "like" in the sentence.

In the Super Smash Bros. series, Yoshi is usually one of cast's heavier fighters. He even outweighs Samus and Ganondorf in the PAL versions of Super Smash Bros. Melee and is on par with Wario in Super Smash Bros. Brawl. Despite this, Yoshi is usually labelled as a lightweight or middleweight character in the Mario Kart and Mario Tennis spin-offs.

The name of Wei faction character Xu Zhu is actually misspelled in the English version of the game. The proper Romanization of his name, "許褚", should be Xu Chu. In Chinese 'Zhu' and 'Chu' are pronounced similarly.

When you encounter Steve Fox and Hwoarang in Hugo and Poison's story mode in the Japanese dub, Hugo says "I am number one!" while clearly pointing to himself. This was replaced with "We are number one!" in the English dub.

In the June 14th, 2015 Direct, Sakurai stated that the Suzaku Castle stage was the first in the series to have music that changes as the timer gets close to zero. This is false, as the Mushroom Kingdom stage of Super Smash Bros., and the Mushroom Kingdom and Mushroom Kingdom II stages of Super Smash Bros. Melee all have music that changes as the timer gets close to zero.

The stage in which the player would fight the boss character Cao Cao is called the "Great Hall - Shang Yong". It was supposed to represent Cao Cao's capital. In reality, Cao Cao's capital was in Xu Chang, considerably further north than Shang Yong.

Attachment The Japanese version of the game is called "悪魔城ドラキュラXX Akumajō Dorakyura Daburu Ekkusu" - which in English literally translates to "Devil's Castle Dracula Double X". The American version, however, is simply called "Castlevania: Dracula X". Likewise, the Australian & European versions were renamed to "Castlevania: Vampire's Kiss".

There are also several differences between the titles screens of the three games. The Australian/European version does not have blood on its title screen. The Japanese version also uses two religious crosses in its "Double X" title, rather than a single red X used by the American version. The "PASS WORD" option is also spelled incorrectly in the Japanese version, but was later corrected as "PASSWORD" in the International version.

In the Australian/European & Japanese versions, the boomerang cross weapon looks like a religious cross, but was replaced with a regular X-shaped cross in the International version. This was likely done to avoid any religious complaints.
