Mario Teaches Typing
1
The voice script for Mario Teaches Typing contained multiple negative remarks towards the player such as "That wasn't very good" and "That was a failure". Voice actor Charles Martinet objected to these remarks, and was given permission to write his own, more supportive lines such as "That was great, but you can do even better!" and "Great try!"

The Great Giana Sisters
2
Attachment It is often rumored that Nintendo filed a lawsuit or sent a C&D to Rainbow Arts over The Great Giana Sisters' resemblance to Super Mario Bros. - however, according to the game's music composer Chris Hülsbeck, this is not true, Nintendo only sent Rainbow Arts an "angry letter" over the UK release's cover art containing the phrase "the brothers are history!", leading to the game being pulled.
The Great Giana Sisters
2
In-game, the hero of The Great Giana Sisters is called "Gianna" with two N's, while on promotional materials and all future Giana Sisters games, she is called "Giana" with one N. Gianna was the intended name for the character, but the name Giana was a typo on the box art which had to stick due to there being too tight a deadline for the box to be reworked.

Attachment Koopa Troopa's voice is an incredibly short snippet of "HUMAN, YELL - YELL: FEMALE", a sound effect of a woman gleefully screaming from the Best Service Voice Spectral Volume 1 sound pack, modified into different pitches and speeds. In some games, a different snippet from the sound effect is used to represent Koopa Troopa screaming or being frustrated.

Efforts had been made to contact the programmer of Wabbit, referred to as "Ban Tran" by her former coworkers. Her real name was discovered to be Van Tran in 2022, and she was interviewed by the Video Game History Foundation in May of that year.

Wabbit was the only game Tran developed for Apollo, as they went bankrupt in November 1982. She later went on to work on the Atari 2600 port of Solar Fox before retiring from the video games industry.

1
The Coolmath Games website has often been noted for a severe lack in mathematics-related content. The title of the website has often been theorized to have been chosen in order to bypass school browser filters - however that is not the case, instead it was named that as a spin-off of the Coolmath website, which was math-themed.

Midnight Mutants
1
Attachment The box art of the game uses an image based on Al Lewis' portrayal of Grandpa Munster in the 1964 The Munsters sitcom and claims the game to be "Featuring... 'GRAMPA'". Despite this, neither Al Lewis nor The Munsters are mentioned on the game's copyright disclaimers and credits, and 'GRAMPA' is not the player character.

Pokémon Emerald Version
3
Because the developers were still experimenting with Double Battles where the player teams up with an NPC ally to fight two NPC opponents, this lead to a glaring oversight during the battle where the player joins with Steven Stone to battle Maxi and Tabitha where, during the battle, Steven's Level 42 Metang can actually gain a substantial amount of EXP simultaneously with the player's own Pokémon. This essentially means that, with careful manipulation, the player can have the ally Metang get enough EXP to grow to Level 43. This so far is the only instance in the entire Pokémon main series where, in a non-scripted battle, an NPC's Pokémon can gain EXP and level up.
Enchanted Portals
2
When the game was first announced, it received numerous negative comparisons to the game Cuphead, namely for having a similar choice of gameplay (a 2D sidescroller with run-and-gun elements and an emphasis on boss battles) and artstyle (which appears to reference 1930s Rubberhose animation), with some people even going as far as to accuse the game of being a Cuphead ripoff.

The Ren & Stimpy Show: Buckeroo$!
1
Attachment In the SNES version of The Ren & Stimpy Show: Buckeroo$!, there is an unused sprite of the character George Liquor in his appearance as the Sheriff of Dodge City from the episode "Robin Höek". Around the time the game was being made, show creator John Kricfalusi was fired from Nickelodeon for inserting inappropriate content and missing deadlines while producing episodes, and was allowed to retain the rights to the George Liquor character, who he would continue to feature in cartoons for the rest of his career, meaning that he could not be used in Ren & Stimpy media from that point onward. Liquor would still appear in the NES version of Buckeroo$! however.
